YULIPRIST 5MG TABLET is a Selective Progesterone Receptor Modulator (SPRM) that is used as an emergency contraceptive and a medication for uterine fibroid. It is used for the prevention of pregnancy in case any regular method of birth control failed.

The use of YULIPRIST 5MG TABLET should be avoided if you are already pregnant or if you are allergic to it or any ingredients in it. You should also avoid using it if you are breastfeeding. It is advised that you consult a doctor, before taking YULIPRIST 5MG TABLET, if you experienced your last menstrual period more than four weeks ago or have a history of tubal (ectopic) pregnancy, or you are already on some medication. It should also be avoided if your menstrual period has not yet started, or if your menopause has already passed.

The reported side effects of YULIPRIST 5MG TABLET are menstrual pain, nausea, stomach pain, tired feeling, dizziness, and headache. You should immediately visit your doctor if start experiencing allergic reactions like rashes, difficult breathing, swelling in tongue, throat, lips, face, or if you encounter extreme pain in the lower abdominal area after 3-5 weeks of using YULIPRIST 5MG TABLET .

YULIPRIST 5MG TABLET is available as 30mg tablets that has to be taken orally. It should be taken within 5 days (120 hours) after suspected contraceptive failure or unprotected intercourse.

In addition to its intended effect, YULIPRIST 5MG TABLET may cause some unwanted effects too. In such cases, you must seek medical attention immediately. This is not an exhaustive list of side effects. Please inform your doctor if you experience any adverse reaction to the medication.

Diabetes mellitus is caused by a deficiency in the secretion of insulin.

Predisponsing factors:

the various predisposing factor are:

 

  • heredity: a familial tendency to diabetes has been found to exist genetic factors are involved when the disease develops before the age of 40 years
  • age: the disease occurs at all ages but a greater percentage of the cases occurs in persons over 45 of age.
  • sex: the disease affects both sexes. the incidence of the disease is somewhat greater in men tha in women.
  • obesity: the incidence of diabetes is greater among obese persons than in persons of normal weight.
  • stress: stresses of various types,ex. frequent intections, worry irregular meals, etc. may also contribute to the development of the disease.

Metabolism in diabetes mellitus:

The metabolismof carbohydrates, fat and protein in diabetes mellitus is briefly discussed below:

  • Carbohydrate metabolism: due to decreased oxidation of glucose in the tissues, the level of blood glucose is increased markedly, leading to glycosuria. the quantily of glucose present in urine may very from  0.5 to 6% depending on the severity of the disease. the volume of urine is increased (polyuria).
  • Fat metabolism: since, glucose is not utilised, there is increased breakdown of fats to acetate and ketone bodies (acetoacetic acid, acetone and β-hydroxybutyric acid. the keto acidosis which develops may lead to dehydration and coma in severe diabetes. the serum cholesterol and triglyceride levels are high.
  • Protein metabolism: since carbohydrates are not utilised for the production of energy, there is increased breakdown of tissue proteins and increased oxidation of dietary proteins for use as energy. hence, there is an increased in nitrogen excetion leading to negative nitrogen balance wasting of muscles is a common symptom in chronic diabetes. hence, diabetics need more proteins (about 2g/kg body weight) rgan normal subjects. the protein allowances should be corresponsingly higher (50% higher) for daibetic children and expectant and nursing mothers. further, increased protein intake may hepl to increase insulin production.


Clinical types of diabetes:

the classification of diabetes from the clinical point of view is as follows :

  • juvenile daibetes
  • adult diabetes

 

  • juvenile daibetes: this includes all daibetics below the age of 20 years. the disease is primarily due to deficiency of insulin. the subjects are generally undernourished and ematiated. they require a high calories, high protein diet and insulin for maintaining the blood sugar level within normal limits.
  • adult diabetes: (maturity onset diabetes) a majority of them are pbese subjects. the treatment will consist of a reducing diet rich in proteins and carbohydrates along with oral anti-diabetic drugs to maintain the blood glucose within normal limits. these subjects require less insulin than juvenile diabeyes.

Treatment of diabetes mellitus

the principles of treatment are as follows:

  • diet
  • moderate exercise
  • insulin in juvenile daibetes and sever adult diabetes and/or oral anti-daibetic drugs in mild to moderate adult diabetes.

diet: only weighed quantitis of food should be consumed according to the diet schedule prescribed by the physician. the most important consideration should be given to:

  • calories needs
  • the proportion of calories from carbohydrate, fats and protein.
  • calorie requirements: the calorie requirements should be about 5% less than the actual requirements for the patient’s height and ideal body weight.

THE PROPORTION OF PROTEIN, FAT AND CARBOHYDRATE IN THE DIET:

the proportion of calories derived from carbohydrates, fats and proteins in the diet will depend on the type of diabetes as indicted below in table:

proteins: since, diabetics in general are in negative nitrogen balance, they should receive about twice as much proteins as normal subjects. the proteins should be of high biological value and provide about 20 to 25% of the calories in the diet.

fluid: a liberal intake is desirable.

carbohydrates: the daily intake of carbohydrates should provide about 40% of the calories to prevent ketosis. the carbohydrate intake should not exceed 40% of the total calorie intake (except in the case of obese diabetics as indicated in above table) as otherwise it will be difficult to control the blood sugar level.

Uterine Fibroids - How They Can Be Managed?

The uterine fibroids are one of the most common gynecologic problems, with over 40-50% of women having them at some point in their life. The number and size of the fibroids, the age of onset, associated symptoms like dysfunctional bleeding and pelvic pain will determine the management of fibroid. The following is a series of management measures, starting from the most conservative to the most invasive. 

1. Wait and watch: In women who are asymptomatic, it is best to watch them for the development of symptoms without any intervention. Also, in women nearing menopause, it is best to just watch the fibroids as they just shrink once menstruation ceases. 

2. Medical therapy: In some women, menstrual cycles could be heavy or irregular and require hormonal replacement. They could also have occasional pain, and so may require painkillers. 

  1. Tranexamic acid is a non-hormonal medication that can be used on heavy bleeding days only to ease the heavy menstrual flow. 
  2. Nonsteroidal anti-inflammatory drugs (NSAIDs) are useful in pain control. Iron and vitamin supplements are also useful in managing anemia and weakness associated with heavy bleeding. 
  3. Oral contraceptives and/or progestins are also used to control abnormal menstrual bleeding. They do not have any effect on fibroid size though. 
  4. Progestin could also be delivered through an intrauterine device (Mirena) to relieve heavy bleeding and prevent pregnancy
  5. Gonadotropin-releasing hormone (Gn-RH) agonists: They block the production of oestrogen and progesterone, and so induce a temporary postmenopausal state. This can also be used to shrink the fibroid so it is easier to remove it surgically. This is not used for more than 3 to 6 months as they can cause hot flashes and bone loss. 
  6. Newer drugs like Mifepristone and Ulipristal acetate are being used to shrink the size of fibroids. 

3. Noninvasive procedure: MRI-guided focused ultrasound surgery is when the women are inside the MRI scanner and an ultrasonic transducer is used for treatment. The exact location of the fibroids are identified and sound waves are used to destroy the fibroid. Done on an outpatient basis, it is safe and effective. 

4. Minimally invasive procedures: Uterine artery embolization is where the arterial supply is cut off to reduce fibroid growth. Myolysis is where laparoscopically heat or cold waves are used to lyse off the fibroids. Endometrial ablation is where the uterine lining is destroyed through high-intensity heat or cold waves. 

5. Invasive procedures: Abdominal or Laparoscopic myomectomy is where the fibroids are removed through an abdominal incision or keyhole surgery. If fibroids are extensive and completed family, hysterectomy is the choice of treatment. Given their high incidence, fibroids require management based on symptoms, age, and other considerations as discussed and decided between the patient and the doctor.

Emergency contraception refers to method of contraception that can be used after sexual intercourse

Methods of emergency contraception are

  • 1insertion of cu-t within 5 days of sexual contact
  • 2 emergency contraceptive pills --tab levonorgesterol (i-pill, unwanted 72) to be taken within 72 hours but are more effective if taken within 24 hours
  • Tab ulipristal acetate 30 mg
  •  3.3-5 tablets of oral contraceptive pill.

Tab levonorgesterol 1.5 mg single dose is most commonly used emergency contraception. Here we will be discussing emergency contraceptive pill -levonorgesterol (ecp)
 
Ecp prevents pregnancy by delaying ovulation (egg release). It does not interrupt an existing pregnancy

 Ecp should be used in following conditions -

  • Unprotected intercourse
  • Concerned about regular contraceptive failure. Like condom tears or slips. Missed pill for regular 3 days etc
  • Incorrect use of contraceptive
  • Sexual assault

Ecp prevents 95% of pregnancy if taken within 72hours of unprotected intercourse

There are no medical contraindication to use of ecp but it should not be used regularly, after the use of ecp a regular contraceptive method should be resumed. Like oral contraceptive pill or iucd or injectable contraceptives

Side effects of ecp use are-

 If periods are delayed after intake of ecp urine pregnancy test should be done

Thus emergency contraceptive pill should not be considered an alternative to regular contraceptive method. Regular contraceptive method should be adopted and ecp should be used only in emergency.
