Double Force 100Mg Tablet is a phosphodiesterase type 5 inhibitor that helps to relax as well as dilate the blood vessels in the body. It helps to increase the flow of blood in certain parts of the body. This medication can be used for treating erectile dysfunction among men. It also treats pulmonary arterial hypertension (PAH), and eventually improves the exercising capacity in men as well as women. It can be taken orally or injected in the body by your doctor.

Double Force 100Mg Tablet helps the muscles that are found around your blood vessel?s walls to relax, thus increasing the blood flow to specific areas of your body. It is also used for treating erectile dysfunctions in men. The reason could be impotence or the inability to keep or get an erection. Apart from that, it can be also used for improving the exercising ability in adults suffering from pulmonary arterial hypertension (PAH). This phosphodiesterase type 5 inhibitor (PDE5) relaxes and dilates your blood vessels that are present in your lungs, thus improving your exercising ability.

Before taking this medicine, it is mandatory to consult your doctor to confirm whether it is safe for you or not. It is advisable to take the medication as prescribed. It is usually injected in your body by the doctor in your clinic. You can also take it at home, provided you are trained or acquainted with the proper method of administration. Also, keep in mind to taking more than the required time frame. When taken for PAH, it is generally taken 3 times in a day, with a gap of 4-6 hours. For problems related to erectile dysfunction, you should take them only when you need it, probably half an hour to one hour before any sexual activity. You should not take this medication more than once in a day in this regard.

It is advisable to continue using Double Force 100Mg Tablet till required without missing any doses. Also, you should not stop using it all of a sudden or change the doses without prior consultation with your doctor.

There are certain things that you must keep in mind when you are taking Double Force 100Mg Tablet. Firstly, avoid drinking alcohol as there can be possible side effects. Secondly, grapefruit products may have a reaction with Double Force 100Mg Tablet, resulting in side effects. Therefore, you must avoid consuming any grapefruit products. Also, ensure that you do not use any other medicines related to treating impotence, along with this drug. If you do so, consult with your doctor first.

The common side effects of Double Force 100Mg Tablet are dizziness, headache, blurred vision, stuffy nose, pain in the back or in the muscles, stomach upset, trouble sleeping, etc. However, if you experience symptoms like pain in the bladder, bloody urine, indigestion, bone pain, migraine headache or increase in the frequency of urination; it is better to consult your doctor immediately.

Erectile Dysfuntion
Double Force 100Mg Tablet is used for the treatment of impotence; where achieving and sustaining an erection is a problem while having sexual intercourse. However, it will work only if there is sexual stimulation.
Pulmonary arterial hypertension
Double Force 100Mg Tablet is used to treat high blood pressure of arteries present in the lungs and right side of the heart. It also improves exercise capacity in such patients.
Double Force 100Mg Tablet is not recommended for use if you have a history of allergy to sildenafil or any other component of this medicine.
Nitrate containing medicines
Double Force 100Mg Tablet is not recommended if you are using any medicine containing nitrates.
Riociguat
Double Force 100Mg Tablet is not recommended for use if you are consuming a medicine named Riociguat (Popular trade name is Adempas).

The risk of permanent tissue (penis) damage is high in such patients and hence caution is advised while administering Double Force 100Mg Tablet.

Double Force 100Mg Tablet should be used with caution in patients suffering from a disorder of the retina in which color discrimination is impaired. Use of this medicine should be avoided in this cases.

If it is the first time it is happening don't bother. You might have injured the gums while brushing your teeth or when you forcefully sneeze / cough there is a likelihood of the small blood vessels rupturing hence the blood in the sputum while you spit or cough.

The very simple two letter word No appears to be in my professional (and sometimes personal experience) a rather tough word to formulate. On the surface of it, it seems to be simple and straightforward. However, in reality, it can be very hard to say. As humans, we are constantly trying to seek validation and acceptance from our peers and society in general. Which is why people a lot many times agree to things that they do not want to do. This can in part be due to the inability to disappoint someone they love and care about. It is also easier to not say no as it helps us maintain connections with people. Even though the end result could overburden us.

The repercussions of this absence of disagreement is critical. By over-committing ourselves, we are actually consuming the majority of our positive energy and using it somewhere where we might not want to. This takes away from our own goals and needs. It leaves us tired and we greet each day like it is simply one more battle or hurdle to cross. We also give up our own basic needs for space, stillness, relaxation and imagination and are bound to our committed state.

To avoid this, we must first acknowledge our own needs and priorities for both mental and physical well-being and joy. For this, one may have to know when saying "yes" will fulfill your requirement for rest, relaxation or self-awareness. When we do this, the unfounded dread of not being socially acceptable for saying "no" gradually dissipates.

Here are three reasons why saying "no" can prove to be beneficial:

  1. You will have more 'Me' time: The extra time made by saying "no" will, at any rate, prevent that forced clock from ticking. How To Deal With The Presence Of Foreign Bodies In Ear/Nose/Throat?

What is a foreign body (FB)?
Foreign body or FB is an object or piece of extraneous matter that has entered the body by accident or design. It is common to see certain items getting stuck in the ear, nose or throat especially, in young children. While most of the time they do not raise an alarm, however, sometimes they can lead to severe consequences like rupture of the eardrum or even choking.

How do FBs affect ear, nose and throat?

  • Children over 9 months develop an ability to pick up objects. As they explore the environment around them, they tend to put items such as peas, beans, marbles, beads, cotton, paper, etc, in their mouth, nose or ears. A similar trend can be seen in adults with mental disorders.
  • Trauma/ fall/accident can force FBs like rock, pebbles into the ear, nose or throat.
  • Adults while riding a bike or sleeping on the ground (campsites) can have insects: flies, ants, roaches fly into the nose or ear.
  • Choking occurs when FBs block the airway, common reasons are: not chewing the food properly, diseases like Parkinsonism, drinking alcohol or certain drugs that slowdowns the process of swallowing.

Managing FB in the ear, nose and throat

Foreign bodies in the ear
Symptoms: Pain in the ear, discharge or bleeding from the ear, feeling of fullness or pressure in the ear and dizziness and nausea in extreme cases.

Management:

  • Make sure the patient is sitting in an upright position, pull the pinna or outer semicircle of the ear towards the back. This can help you see the object clearly and also loosen it to wiggle out. If you can see the object clearly, use a tweezer to grasp the object and gently remove.
  • In case a live insect gets trapped inside the ear lie down and place few drops of warm vegetable or baby oil. Use oil only if symptoms of eardrum perforation like pain, bleeding/discharge from the ear is absent.
  • Tilting the head to the affected side can facilitate gravity to dislodge the object.

Foreign bodies in the nose:
Symptoms: 
Pain and difficulty in breathing from that side of the nose.
Management: Keep the uninvolved nostril closed by pressing a finger against the side of the nose and gently ask the person to blow from the affected nostril.

Foreign bodies in the throat:
Symptoms: Choking, inability to breathe and speak 
Management: Perform Heimlich maneuver: abdominal thrusts, if trained otherwise, it is advised to call for medical help immediately.

How can you prevent foreign bodies from entering your ear, nose, throat?

  • Discourage talking, playing, laughing when food is in the mouth.
  • Do not give whole grapes, popcorn, nuts to children below 3 years.
  • Keep small objects out of reach of small children.
  • Teach children to avoid placing objects into their nose, ears.

Usually, breast cancer affects women in the age range of 40 to 50, but there have been many reports of it occurring earlier. About 48 % percent of the Indian women having breast cancer have been reported to be under 50. This is an almost 17 percent jump compared to figures 25 years earlier.

Alarming Signs of Breast Cancer:

  • The most common symptom of breast cancer is a new lump or mass. A painless, hard mass that has irregular edges is more likely to be cancerous, but breast cancers can be tender, soft, or rounded.
  • Swelling of all or part of a breast (even if no distinct lump is felt)
  • Skin irritation or dimpling
  • Breast or nipple pain
  • Nipple retraction (turning inward)
  • Redness, scaliness, or thickening of the nipple or breast skin
  • Nipple discharge (other than breast milk)

Screening Guidelines for Breast cancer:

  • Mammography: US Preventive Services Task Force (USPSTF) recommends mammography once in every 2 year for females between age group 50-75 years. Women with a family history has a benefit of starting at age 40 years. After 75 years there are insufficient evidence of benefit of screening mammography.
  • Breast Self Examination and annual Clinical examination: breast self examination is easy to learn and and any abnormality should be reported to the oncologist.
  • For High Risk/ Genetic risk factors: screening should start by age 25 or 10 years earlier of youngest relative affected. Annual mammogram and MRI or Doppler ultrasound alternating at 6 month interval can be considered.

Treatment Options:

Some of the common treatment options for breast cancers are:

  • Breast conservation surgery - This is where the tumors are removed without cutting out too much of the surrounding breast tissue.
  • Mastectomy - If the cancer has spread too far then large amounts of breast tissue has to be removed along with the tumor to stop its spread any further.
  • Sentinel lymph node Biopsy - it's part of both surgical procedures, it decreases morbidities of lymph node dissection first draining lymph nodes only are removed in patients with clinically negative axilla. If negative on frozen section further axillary dissection is avoided. It avoids the morbidity of complete axillary clearance.

Cervical cancer ( Carcinoma Cervix)

Cervical cancer is the cancer of the cervix, which is the opening of the uterus and connects the uterus to the vagina. Indian women in particular are susceptible to cervical cancer as more than 1/3rd of the cervical cancer related deaths in the entire world happens here in India, which is about 74000 deaths annually. The leading cause for cervical cancer is HPV or Human Papillomavirus infection, which if left untreated and unchecked will result in cancer.

Alarming Signs of Cervical Carcinoma:

  • Early lesions and precancerous lesions have no symptoms
  • Abnormal vaginal bleeding such as bleeding after vaginal intercourse, bleeding after menopause, bleeding and spotting between periods or longer/heavier periods
  • Abnormal vaginal discharge
  • Pelvic pain
  • Late stage disease may cause lower extremity swelling, problem with urine and/or stool passage

Screening Guidelines:

  • Cervical cancer screening is most effective screening programme of any cancer disease resulting in significant decrease in mortality from carcinoma cervix
  • United state preventive services task force (USPSTF) recommend cervical cancer screening starting at age 21 yrs.
  • Between age 21-30 years cytology recommended at 3 year interval and HPV testing is not recommended.
  • Between 30-65 years cytology every 3 year or cytology +HPV testing every 5 year.
  • If previously adequately screened screening can be discontinued after 65 years.
  • Women with prior total hysterectomy and no CIN can discontinue screening.

Apart from breast and cervical cancers, certain other cancers have also reported an alarming increase such as:-

Ovarian cancer (Carcinoma Ovary)

Starting in the ovaries, diagnosis of this type of cancer is usually delayed due to lack of visible symptoms in the initial stages. About 85% to 90% of ovarian cancers are epithelial ovarian carcinomas. Epithelial ovarian cancers tend to spread to the lining and organs of the pelvis and abdomen (belly) first. This may lead to the buildup of fluid in the abdominal cavity (called ascites). As it advances, it may spread to the lung and liver, or, rarely, to the brain, bones, or skin. The main treatments for ovarian cancer are surgery and chemotherapy.

Cancer of the endometrium (Carcinoma Endometrium)

Endometrium is the lining of the uterine wall and has along with ovarian, uterine and cervical cancer is also increasing in occurrences throughout India and the world.

The grade of an endometrial cancer is based on how much the cancer forms glands that look similar to those found in normal, healthy endometrium.

  • Grade 1 tumors have 95% or more of the cancerous tissue forming glands.
  • Grade 2 tumors have between 50% and 94% of the cancerous tissue forming glands.
  • Grade 3 tumors have less than half of the cancerous tissue forming glands. Grade 3 cancers tend to be aggressive and have a poorer outlook than lower-grade cancers.

Treatment Options:

Cervical cancer along with Ovarian, uterine and endometrial cancer can be treated using the following methods:

1. Surgery: Radical Hysterectomy primarily preferred option for early stage disease. Both open and minimal invasive approaches are suitable.

  • For precancer high grade lesion conisation is also option for compliant patient.
  • In young suitable patient willing for childbearing trachelectomy is also possible in very early lesion.
  • Depending on risk factors after surgery adjuvant treatment may be advised.

2. Radiation therapy or Combined Radiation therapy+Chemotherapy: Alternative to surgery in early stage and choice in late stage.

Prevention:

Certain prevention techniques that would be advisable to detect cancer in its early stages would be,

  • Regular Pap smear tests for cervical cancer
  • Taking vaccines for cervical cancer
  • Regular mammograms and checkups for breast cancer
  • Changing lifestyles to reduce stress
  • Changes in diet to reduce risks
  • Leading a healthy lifestyle with regular exercise
