Ammonia Questions

My mother is going through intense dry cough attacks at night especially. It all started 2 years back with coughing at night which used to get ok with macberry cough syrup but since a month now nothing is working. Coughing keeps her awake. Chest x ray is normal and complete blood test is normal also ammonia breath test is also normal. Going by the symptoms I think it is gerd even though she does not has heartburn. Please suggest proper medicine for this gerd induced cough. She takes rabeprazole capsule whenever her stomach is upset.

I'm a 23 year old sexually active female. I've been experiencing sharp pain inside vagina during sex. And having smelly discharge. That sometimes smells like ammonia. I am planning to visit a gyne this week. What can be the reason this is happening. Is this something very big I need to worry about. What what will the gyne do during the check up. What sort of test are expected to be done. Thanks in advance.

During your check up the gynec will do an internal. Check with a speculum .it doesn't hurt at all. If required she might suggest you to do a PAP smear. Which is a test of the cervix. To check for active infections and to rule out cancer. Depending on the Infection she might advice to vaginal tablets to insert.

I got Jaundice on 18 Nov when bilirubin was 5.10 and typhoid as the report says. Went to Doctor on 04th December as I wasn't feeling sick, it was just that my eyes was yellowish. On 4 the doctor took my blood samples and said I got Obstructive jaundice with total bilirubin showing 20.5, direct as 15.3, indirect 5.2 And also on the next day he advised me to do Ammonia test, Viral card test and sonography. The reports of sonography and ammonia was normal. On 7 my report of viral card came showing Anti HBe Antibody - Non Reactive (1.99) Anti HBs (total) - Reactive (484.48) Anti HCV Antibody - Non Reactive (0.084) HAV IgM antibody - Reactive (7.51) HBC IgM - Negative (0.0) HBC Total -Negative (2.26) HBE Antigen. -Non Reactive (0.329) Hepatitis Marker -Non Reactive (0.675) Profile. And showing total bilirubin 18.8 Direct as 12.17 and indirect as 12.17 After which the doctor said I had hepatitis A and B. And kept me admitted for a week so that my bilirubin decreases. After I got discharge at a bilirubin of 13.5 on 10th Dec and was advised to be in home with regular medicines and in a complete bed rest. He advise me to do MRCP test and consult to gastroenterology. Later I went on 29dec to a specialist and sonography and bilirubin showing 4.3 while normal in sonography. As the last reports of 28.01. 2016 the bilirubin was 1.5 and my other new doctor (gastroenteritis) says no need to do any more tests, like MRCP or any Hepatitis Test as said by him that I was having Hep A and not Hepatitis B At present moment, my weight is 52kg. I am Going to my college, should I need to worry about any of this Hepatitis A, Hepatitis B? Can I start eating healthy food with Masala, Meat, Eggs, Cheese, etc etc? Please answer precisely about appetite. Thank You Best Regards.

Hi my husband is 52 he was drinking 200 ml of alcohol from since 5 years everyday, he is diabetic since 10 years is on insulin 28 unit twice was taking viboliv and ursocol 450 once with vibrator, prolomet am50 and Janumet 50/1000 till march his sugar level was high 8.1 hba1c, his liver function test, lipids except triglycerides was normal, his sonogram all normal showed only grade 1 fatty liver. This condition was ok even till July, he started losing weight, and along with that his sugar level started coming to very normal 61hba1c and fasting 85 all of sudden pp 120. Then again sonogram and fibscan showed everything normal, his liver function test was quite normal so the gen physician stopped ursocol. Now in two months he has lost almost 3 kg, lot of inches, but in my back of mind I felt that something is wrong so got tested for ammonia and bun which show 80 ugdl and bun 3.2, we live in Assam which is very backwards medically, even the gastro said nothing wrong as there was not any mental confusion even ammonia was high. Then a visiting doctor suggested ct scan which indicated diffuse parenchymal changes with .portosystemic collateral in perisplenic region seen. We went to hyderabad (few minutes with Dr. Nageshwar reddy) where again a MRI was done which showed the same result as ct scan with no lesion. Doctor said his liver has 10 percent damage, condition is reversible if he does not take any alcohol. He has been advised bilefix 600 and viboliv twice along with his earlier medication. He has stopped 200 ml of scotch daily but to avoid relapse is taken 2/3 can of light beer (5/) alcohol beer alternate days and is trying to quit that also in 1 month time. Very worried is it leading to cihrossis. - [ ]

Hi, I can understand your worries. please don't panic and stick to doctor's advice. Tell your husband to have high protein diet and less spicy and fat free diet. Do regular blood tests and other tests. Take information from Google but don't conclude them on your own. If you have any doubt clear them with the doctor.

Good morning, It seems like a very complicated scenario and your attempt to describe his symptoms and problems seems to be very clear. It is very likely that cirrhosis has made his medical condition worse. I believe you need to closely monitor his kidney and liver functions and day to day activities. If in case any problems arise due to his condition becoming bad (as per your observation) he would need immediate admission. However, for now, it seems like his condition is stable on medications. If he is not able to sleep or has other withdrawal reactions from stopping or going down on the amount of alcohol, you might need to consult a psychiatrist ASAP. Look forward to hearing from you. If you need help with booking an appointment online with me on this website either text, audio or video appointment for a confidential and private consultation you may do so by contacting the help desk of Lybrate.
