A antispasmodic, Ranispas Old Tablet is used for the treatment of a certain type of intestinal problem called irritable bowels syndrome. It reduces the symptoms of intestinal and stomach cramping. This medicine works by slowing down the natural movements of the gut and by relaxing the stomach and intestine muscles.

This medication must not be used in children younger than 6 months old and in women who are pregnant or breastfeeding. Before using this medication, inform your doctor if you have a history of glaucoma, enlarged prostate, urination problems, high blood pressure, nervous system problems, myasthenia gravis, liver, heart, thyroid, intestinal or kidney problems.

This medicine can cause a range of anticholinergic side effects such as dizziness, weakness, dry eyes, blurred vision, dry mouth and abdominal bloating, and, at higher doses, serious effects like decreased sweating, flushed skin, irregular heartbeat, slurred speech, loss of coordination, mood changes, difficulty urinating and decreased sexual ability.

The medication is to be taken by mouth, with or without food as instructed by your doctor, usually 4 times a day. The dosage is based on your age, medical condition, and your response to the medicine. Your doctor may direct you to start this medication at a low dose and gradually increase your dose to decrease the risk of side effects.

In addition to its intended effect, Ranispas Old Tablet may cause some unwanted effects too. In such cases, you must seek medical attention immediately. This is not an exhaustive list of side effects. Please inform your doctor if you experience any adverse reaction to the medication.

I am 20 Male. I have been having a weird feeling in my testicles from last few days. It is sometimes leading to pain. I think it's mostly a muscle or vein related as I get a light sensation when I pull/push my muscle there. I went to an urologist. He told me these are common for my age and examined my testicles. He applied pressure there and asked me if I am having a burning sensation which I had a very slight one. He told me it looks fine and mostly it will go away and gave me a tablet called signoflam and told me to use when ever I get pain. Should I go for further diagnosis or use the tablet? I have been under stress too for last few weeks. Could that have caused the problem?

I am a Gastroenterologist and would be answering your query. As per you query answers are- Firstly for Irritable bowel syndrome. Irritable bowel syndrome is a common intestinal disorder. There is some problem in the functional ability of the gut that leads to the symptoms of irritable bowel syndrome. The structure of the intestines is normal, only functional abnormality occurs in this condition. So frankly speaking (IBS) represents a functional disorder of gastrointestinal tract without the presence of an anatomic defect. Recent studies by French scientists have indicated that the most important mechanisms include visceral sensitivity, abnormal gut motility and autonomous nervous system dysfunction. The interactions between these three mechanisms make bowel's function susceptible to many exogenous and endogenous factors like gastrointestinal flora, feeding and psychosocial factors. Further data indicate that according to the above mechanisms, the influence of genetic factors and polymorphisms of human DNA in the development of IBS is equally important. So in lay mans language till date we don't know the exact cause of this problem. So the million dollar question is how to repair a system when you don't know which part of it is faulty (Some says it can be brain other syas it can be hormone anxiety age etc etc). Some patients have attacks of diarrhea and bloating and some may have constipation. Other symptoms that may be present in irritable bowel syndrome include nausea, vomiting, headache, loss of appetite, muscular pains, heartburn, and weakness. Some patients have occasional symptoms whereas others may have symptoms for a long period of time. So as seen above every patient is different. Intensity of symptoms in every patient is different as so is the response to medicines and life style change. Every drug is not effective in every patient so we have to try different therapies till we find the drug which suits best our patient. Dicyclomine 20 mg four times a day is to heavy a dose for 80 year old women. Dicyclomine ios an anticholinergic drug. An anticholinergic agent is a substance that blocks the neurotransmitter acetylcholine in the central and the peripheral nervous system. Anticholinergics inhibit parasympathetic nerve impulses by selectively blocking the binding of the neurotransmitter acetylcholine to its receptor in nerve cells (In simple words it messes up with both your peripheral and central nervous system) Mebeverine is an antispasmodic medicine and is very useful in IBS and is well tolerated by patient with minimal side effects. Mebeverine works on certain muscles in the wall of your intestines, causing them to relax. Clidinium bromide is an anticholinergic (specifically a muscarinic antagonist) drug. It may help symptoms of frequent stools. Chlordiazepoxide works by increasing the action of GABA, a chemical messenger which suppresses the abnormal and excessive activity of the nerve cells in the brain. Can be used in combination with above mentioned medicines. Another OTC medication is Bismuth sub-salicylate. It belongs to a class of medications called antacids and adsorbents marketed by name of Pepto Bismol. Doses up to 1gram can be taken daily to provide symptomatic relief. Loperamide can be used but should be avoided. Diphenoxylate and Atropine combination (Lomotil)-Atropine/diphenoxylate is an antidiarrheal and anticholinergic combination. For bloating and gas take Tab Nexium 40 mg twice daily. Take a probiotic daily with food as proper digestion of food will prevent excess gas formation. Food pattern recommended is- Eat simple and natural home cooked food. If possible try detox diet (Raw vegans) for atleast 3 month. Take normal food in lunch while in supper take uncooked raw natural form of food. This can play wonders. Avoid taking excessive fibres. A total fiber intake of about 20-30 grram per day would be good for you. Any thing in excess is good for nothing. No pizza, Burger,Red meat, and dairies for few weeks. No alcohol or smoking. Take natural food rich in probiotics like Yoghurt, Kimchi,etc. This doesnot mean that you have to take them as much as you can. Follow your body increase their uptake gradually. Drink lots of juices and fresh water daily. Avoid over spicy and heavy food. If possible go for a detox diet but again don't force your body rather let your body adopt gradually to things. Anal fissure and fistula- An anal fissure is a small tear or crack in the lining of the anus so in lay mans language its an internal wound. Certain treatments can promote healing and help relieve discomfort, including stool softeners (you already have soft stools) and topical pain relievers. If an anal fissure doesn’t improve with these treatments, surgery may be required. Salts adviced for trial before going for surgery are- A broad spectrum antibiotic like Cefuroxime to prevent bacterial infection of site. Combination of trypsinogen with serratiopeptidase (Wound healer). Dicofenac as an anti inflammatory. Ointment containing broad spectrum antibiotic to be applied locally over fissures. Vitamin C (wound healer). Folic acid and zinc supplement. However initially an anoscopy should be done by a Rectal surgeon to see the extent of fissure. Best surgical option is Video-assisted anal fistula treatment (VAAFT) is a novel minimally invasive and sphincter-saving technique for treating complex fistulas. As per the standard data of NCBI (National centre of biotechnological information) a facility funded by American govt data's are-"From May 2006 to May 2011, 136 patients were operated using VAAFT. Ninety-eight patients were followed up for a minimum of 6 months. 1) No major complications occurred. In most cases, both short-term and long-term postoperative pain was acceptable. 2) Primary healing was achieved in 72 patients (73.5%) within 2–3 months of the operation. 3) Sixty-two patients were followed up for more than 1 year. The percentage of the patients healed after 1 year was 87.1%. Hermorrhoids- 1) Sitz bath-Sit in about 3 inches of warm water for 15 minutes or so, several times a day. This helps reduce swelling in the area and relaxes your clenching sphincter muscle. 2) A 1% hydrocortisone cream on the skin outside the anus can relieve itching and pain. 3) With an electric probe, a laser beam, or an infrared light, a tiny burn will be made to remove tissue and painlessly seal the end of the hemorrhoid, causing it to close off and shrink. 4Rubber band ligation is a procedure in which the hemorrhoid is tied off at its base with rubber bands, cutting off the blood flow to the hemorrhoid. I have gone through your query and can understand your concern. It's not clear what causes irritable bowel syndrome, treatment focuses on the relief of symptoms. Following steps should be taken 1. Eliminating high-gas foods such items as carbonated beverages, vegetables ? especially cabbage, broccoli and cauliflower ? and raw fruits. 2.Some people are sensitive to types of carbohydrates such as fructose, fructans & lactose, called FODMAPs .FODMAPs are found in certain grains, vegetables, fruits and dairy products such as wheat, rye, barley, onion, garlic, apples, apricots, avocados, blackberries, cherries, etc 3.Taking fiber supplements, such as psyllium or methylcellulose, with fluids may help control constipation. Fiber obtained from food may cause much more bloating compared with a fiber supplement. 4.Anti-diarrheal medications such as loperamide, can help control diarrhea. 5.Anticholinergic and antispasmodic medications. These medications, such as hyoscyamine and dicyclomine, can help relieve painful bowel spasms. They should not be used if you have constipation. 6.Counseling. You may benefit from counseling if you have depression or if stress tends to worsen your symptoms. 7. A stomach ulcer or gastric ulcer refers to the defects or breakdown in the mucosal lining in the stomach. The stomach contains acid, which helps in digestion and any break in the tissue lining may result in severe pain or bleeding in some cases. The pain is usually felt in the abdomen.

Stomach ulcers cause great discomfort and should be treated effectively. Here are the primary ways for treating stomach ulcers:

  1. Antibiotics: In the case of H. pylori infection, you should take a course of more than two antibiotics, to be taken twice a day and must be continued for a week. Antibiotics such as amoxicillin, clarithromycin or metronidazole may be used in case of such stomach ulcers. However it is always recommended to consult a specialized doctor before consuming any of these medicines. After several weeks of using the antibiotics, you should get a test done for detection of H. pylori bacteria. If they are still present, you have to take different antibiotics.

  2. Proton Pump Inhibitors: Proton pump inhibitors or PPIs helps in the reduction of the amount of acid produced by the stomach. This prevents damage to the ulcer and lets it heal naturally. PPIs are generally taken for four to eight weeks. The most common PPIs which are used for treating stomach ulcers are omeprazole, pantoprazole and lansoprazole.

  3. H2 Receptor Antagonists: H2 receptor antagonists reduce the production of stomach acid and function the same as PPIs. The most common H2 receptor for treating stomach ulcers is ranitidine.

  4. Antacids/ Alginate: All measures to treat stomach ulcers take several hours to show results. Your doctor may prescribe antacid medications for neutralizing the stomach acid. This will provide relief for the moment, but the relief is quite temporary and for a short span of time. By this time, the strong medicines most likely start working. There is a certain antacid medicine known as alginate. This develops a protective coating on the stomach lining. Antacids are available at all over the counter medicine stores and you should consult your doctor to get the best antacid which would work effectively on your stomach ulcer symptoms. You should take antacids before going to sleep or after taking meals.

  5. Natural Remedies: Stomach ulcer can also be treated at home using natural remedies. Medicines may sometimes cause further digestive complications, so home remedies are much more easy to follow and do not have side effects. The best way is to follow a proper diet, which is comprised of certain food items that help in curing stomach ulcers. You should consume bananas, honey, garlic, green leafy vegetables like cabbage, licorice and capsaicin.

Disorders of intimate arousal include several aspects of arousal - physical such as genital lubrication, congestion and sensation, subjective such as interest, distress or interpersonal relationship difficulty. Problems of arousal vary among women and can be classified in the following ways:

  1. Primary: The woman has never experienced sufficient arousal, even when she has had sufficient desire and sexual stimulation.
  2. Secondary: The woman has been sufficiently aroused in the past, but currently experiences decreased arousal.
  3. Non-situational: The problem occurs in all sexual situations.
  4. Situational: The problem occurs only in some sexual situations.

Common factors for this problem include decrease of estrogens and androgens; diabetes; thyroid and prolactin abnormality, vaginal and urinary tract infections; surgery, radiation, and medication, smoking, alcoholism; chronic diseases and psychological factors such as depression and anxiety. Other factors are negative sexual attitudes, low self-esteem triggered by body image like small breast size, overweight and sexual and childhood abuse. Lack of communication can also affect sexual arousal like if a woman cannot tell her partner how she prefers to be stimulated.

  1. Partner’s sexual dysfunction is also a major cause, it includes erectile dysfunction or premature ejaculation.
  2. Women with sexual arousal disorder are assessed by medical and detail sexual history and clinical examination, including vaginal and breast examinations. Also talking about distress, attitude to sex, and communication with partner etc.
  3. Testosterone, estrogens, Prolactin, TSH, Sugar, Hb, TC, DC any other test depending on physical examination.
  4. Treatment depends on the cause of the disorder, related to physical, psychological, and interpersonal factors. If abnormalities are found in blood tests they are treated by medicines.
  5. Estrogen therapy in the form of local creams or oral tablets or skin patch is used to improve vaginal lubrication and decrease vaginal dryness. Non medicated lubricants can also be used.
  6. Drugs like Sildenafil, Tadalafil etc increase genital blood flow. DHEA is claimed to improve sexual interest and genital sensation.

Hyperactive arousal Disorders

Some women experience persistent genital arousal called restless genital syndrome. There is increased genital blood flow in non-sexual situations causing distress. It may be related to depression and anxiety there may be physical causes like vascular, neurologic, pharmacologic, and hormonal. Its treatment involves relaxation exercises, topical anesthetics, avoiding use of medications or herbal and nutritional products. Normal pregnancy lasts for 38 to 42 weeks. However, there may be times during these weeks that you may have some physical issues such as vomiting, backache, and loose stools etc which require treatment with medicines.

Therefore, monitoring and managing these physical conditions becomes necessary to ensure a normal delivery. Management consists of both medicines and rehabilitation. Medicines are considered to be the first line of treatment; however, the health care provider needs to take care while prescribing medicines during pregnancy taking into consideration the harmful effects they can have on both – the mother and the foetus.  Medicines pass on via the placenta from the mother to the foetus. Hence, before prescribing any medicine the provider should check the possibilities of the medicine causing any congenital defect. You should avoid medicines from the time of conception till the first 10 weeks, as this is the time when the foetus is most prone to the get permanent congenital deformities. Medicines given in the later stages (after 10 weeks) may cause systemic damage. For example NSAIDs (Non Steroidal Anti-Inflammatory Drugs) may lead to problems during labour or organ defects in the foetus.

Therefore, medicines should always be avoided during pregnancy. Still there are certain medicines, which can be taken but after consulting your physician. Let’s discuss the medicines that are safe during pregnancy.
