Hello, Thank you for reaching out and sharing your struggles. I can understand how challenging it must be for you to try to quit alcohol, especially when facing the difficulties and temptations that come with it. Firstly, I want to commend you for taking the courageous step of seeking help and starting medication to support your journey towards sobriety. It's important to acknowledge that overcoming addiction is a process that takes time and persistence, and it's okay to ask for support along the way. While medication can be a helpful tool in managing withdrawal symptoms and cravings, it's also essential to address the underlying factors contributing to your alcohol use. Counselling or therapy can provide you with the necessary support, guidance, and coping strategies to navigate this process successfully. In addition to medication and therapy, here are some suggestions that may further support your efforts to quit alcohol: Establish a Support System: Surround yourself with understanding and supportive individuals who can encourage and motivate you on your journey. This could include friends those who don't drink, family members, support groups, or a counsellor. Identify Triggers: Take some time to reflect on the situations, emotions, or people that may trigger your urge to drink. Once identified, work on developing healthy coping mechanisms to manage these triggers effectively. Engage in Healthy Activities: Find alternative ways to occupy your time and distract yourself from thoughts of alcohol. Engage in activities that bring you joy, such as exercise- yoga, walking, hobbies, spending time in nature, or pursuing creative outlets. Practice Self-Care: Prioritize self-care activities that promote your overall well-being, such as getting enough sleep, eating nutritious meals, practising relaxation techniques (e.g., deep breathing, meditation), and engaging in activities that reduce stress. Set Realistic Goals: Break down your goal of quitting alcohol into smaller, manageable steps. Celebrate your successes along the way, no matter how small, and be gentle with yourself if you experience setbacks. Incorporate Daily Routine: Establishing a daily routine can provide structure and stability to your life. Set aside time each day for activities such as yoga, pranayama, and meditation, which can help reduce stress and promote mindfulness. Remember, recovery is a journey, and it's okay to seek help and support along the way. You've already taken the first step by reaching out for assistance, and I'm here to support you every step of the way. If you'd like to discuss further or explore additional resources, please feel free to reach out. You're not alone in this, and together, we can work towards a healthier and happier future. Keep up the great work, Manoj! Your commitment to overcoming challenges and improving your well-being is truly inspiring. Remember, each step you take towards sobriety is a testament to your strength and resilience. Take care and stay strong,

Hello, thanks for the query. I have gone through the information given. The glucose levels that are mentioned seem to be quite good. However, the way the numbers are given it is dificult to know which are fasting and which are pp levels. It would have been etter if you had presented the details in a tabular manner. Still the levels are quite good, suggesting there is good glucose control. Please look at the hba1c% if you have completed over 3 months with the drug that you have started. As regards variation in numbers, it is bound to occur as food intake and physical activity tends to vary every day. If fbg is closer to 100 mg & pp < 150 mg then one has very good control. So I do not see any reason to worry. Please check the hba1c%. If that is < 6.5% then you have achieved really a good control. If you have any questions please do come back. Thanks.

Try to adjust your calorie intake according to your calorie expenditure. If this does not lower your hba1c to 7, then your dose of medication will need to be increased. Consult a physician for the same.

Hello, thanks for the query. I have seen the details mentioned. With h/o acute pancreatis there is no insulin secretion. Hence no oral medication can work. Because the oral drugs act either by stimulating beta cell to secrete insulin (by different methods) or by reducing insulin resistance. But because there is no insulin secretion at all so those drugs can not act. Hence, insulin injections is the only option. That to multiple doses may be required (long acting & rapid acting). The drugs like sglt-2 inhibitors like dapagliflozin, empagliflozin and canagliflozin may be tried as they function at kidney level. Thanks.

Hello, thanks for the query. I have sen the details mentioned. The very fact that you glucose shows 244 mg & 279 mg (in different glucometers), is the sign of uncontrolled diabetes. Checking in different machines with a gap can show up variations in levels. But what is significant is that random glucose levels are high. This suggests your current treatment is not adequate. There is no mention of what medications you are on, the dosage of various drugs, fbg, pp glucose & hba1c%, your dietary pattern, weight, height, extent of daily exercise, lipid profile and bp readings. Without having those important details it is difficult to comment any further or suggest anything specific. So you may have to come back with all the above information preferably in a private conversation then I will be able to give specific advice and even prescribe any medication that may be required. If not please contact your physician and get necessary changes done. At your age ideally fbg should be < 100 mg, pp 150 to 160 mg & hba1c% < 6.5%. If this happens then you are able to lead a healthy life and prevent or postpone long term complications associated with diabetes. If you have any questions please do come back. Thanks.

Hello, thanks for the query. I have seen the details mentioned. Please let me know your fasting, after food glucose levels & hba1c%. If you are able to control blood glucose with insulin & istamet, then one can look at dropping dapagliflozin 10 mg. Since non of the necessary details are given it is difficult to comment or give any suggestions. Please come back with following information: fasting, pp glucose, hba1c%, dietary details, extent of daily exercise & lipid profile. Preferably in a private conversation, then I will be bale to guide specifically. Thanks.
