Tadalafil is a medicine that is used to treat sexual ailments in men. It is used to treat symptoms like erectile dysfunction and sexual impotence. It does this by increasing the amount of blood that ultimately reaches the penis. This medicine works by relaxing the muscles around the genital area of men. It does this by preventing Phosphodiesterase-5 from working on the body. This further increases the amount of cyclic guanosine monophosphate (cGMP) present in the body, which ends up relaxing smooth muscles inside blood vessels.

This medicine can also be used to treat problems of the prostrate gland and of passing urine. However, it will not help you in protection against and treatment of sexually transmitted diseases like syphilis. This medicine does not require a doctor’s prescription, but you should talk to a urologist about the dosage you specifically require, as any kind of overdose may be extremely dangerous. It may even cause death. Before you obtain the dosage, make sure to inform your doctor of your medical history and of any drugs you are currently taking. After the prescription, you may take this medicine with or without food and resume sexual activity after about 60 minutes of consumption.

This medicine is prescribed in cases of erectile dysfunction which makes having intercourse difficult for males. Sexual stimulation combined with this medicine can help sustain an erection. Pulmonary hypertension mainly occurs due to narrowing of the arteries of the lungs as a result of which the flow of blood is restricted. In this kind of condition, blood fails to carry oxygen to your heart and thus your heart gets adversely affected. The blood pressure of your body will get increased to a great extent if this situation remains untreated, thus leading to bdreadful consequenses, specifically situation like heart failure, serious pulmonary diseases, blood clots in lungs and congenial heart defects, etc.

There are certain forms of pulmonary hypertension, which are serious in nature and worsen with time, so much so that they are even fatal at times. It also includes forms which are non curable in nature, however, in order to improve the quality of life, symptoms can be reduced with proper treatment. The treatment for pulmonary hypertension is often complex and it takes some time to find the most appropriate treatment and requires extensive follow-up care. Your doctor might also need to change your treatment if it's no longer effective. However, when pulmonary hypertension is caused by another condition, your doctor will treat the underlying cause whenever possible.

Common Treatments for pulmonary hypertension:

Medications:

  1. Blood vessel dilators (vasodilators): Vasodilators open narrowed blood vessels. They are one of the most commonly prescribed vasodilators for pulmonary hypertension is epoprostenol (Flolan, Veletri). The drawback to epoprostenol is that the effect lasts only for few minutes. This drug is continuously injected through an intravenous (IV) catheter via a small pump that you wear in a pack on your belt or shoulder.
  2. Ventavis: Another form of the drug, iloprost (Ventavis), can be inhaled six to nine times a day through a nebulizer, a machine that vaporizes your medication. Because it's inhaled, it goes directly to the lungs.
  3. Treprostinil (Tyvaso, Remodulin, Orenitram): It is another form of the drug, which can be given four times a day and can be inhaled or can be taken as an oral medication and can also be administered through injection.
  4. Endothelin receptor antagonists: These medications reverse the effect of endothelin, a substance in the walls of blood vessels that causes them to narrow. These drugs may improve your energy level and symptoms. However, these drugs shouldn't be taken if you're pregnant. Also, these drugs can damage your liver and you may need monthly liver monitoring.
  5. Sildenafil and tadalafil: Sildenafil (Revatio, Viagra) and tadalafil (Cialis, Adcirca) are sometimes used to treat pulmonary hypertension. These drugs work by opening the blood vessels in the lungs to allow blood to flow through more easily.
  6. High-dose calcium channel blockers: These drugs help relax the muscles in the walls of your blood vessels. They include medications, such as amlodipine (Norvasc), diltiazem (Cardizem, Tiazac, others) and nifedipine (Procardia, others). Although calcium channel blockers can be effective, only a small number of people with pulmonary hypertension respond to them.
  7. Soluble guanylate cyclase (SGC) stimulator: Soluble guanylate cyclase (SGC) stimulators (Adempas) interact with nitric oxide and help relax the pulmonary arteries and lower the pressure within the arteries. These medications should not be taken if you're pregnant. They can sometimes cause dizziness or nausea.
  8. Anticoagulants: Your doctor is likely to prescribe the anticoagulant warfarin (Coumadin, Jantoven) to help prevent the formation of blood clots within the small pulmonary arteries. Because anticoagulants prevent normal blood coagulation, they increase your risk of bleeding complications. Take warfarin exactly as prescribed, because warfarin can cause severe side effects if taken incorrectly. If you're taking warfarin, your doctor will ask you to have periodic blood tests to check how well the drug is working. Many other drugs, herbal supplements and foods can interact with warfarin, so be sure your doctor knows all of the medications you're taking.
  9. Digoxin: Digoxin (Lanoxin) can help the heart beat stronger and pump more blood. It can help control the heart rate if you experience arrhythmias.
  10. Diuretics: Commonly known as water pills, these medications help eliminate excess fluid from your body. This reduces the amount of work your heart has to do. They may also be used to limit fluid buildup in your lungs.
  11. Oxygen: Your doctor might suggest that you sometimes breathe pure oxygen, a treatment known as oxygen therapy, to help treat pulmonary hypertension, especially if you live at a high altitude or have sleep apnea. Some people who have pulmonary hypertension eventually require continuous oxygen therapy.

Surgeries:

  1. Atrial Septostomy: If medications don't control your pulmonary hypertension, this open heart surgery might be an option. In an atrial septostomy, a surgeon will create an opening between the upper left and right chambers of your heart (atria) to relieve the pressure on the right side of your heart.
  2. Lung Transplantation: In some cases, a lung or heart lung transplant might be an option, especially for younger people who have idiopathic pulmonary arterial hypertension.

Reducing Complications:

  1. The complications need to be reduced by taking proper health care. Healthy diet needs to be taken regularly along with proper medications. Smoking should be stopped and exercising sessions should be attended daily without any fail

In Erectile Dysfunction, a person is unable to maintain an erection necessary for a sexual intercourse. A man's confidence and self-esteem often take a beating in such cases. Can Viagra Cause Skin Cancer?

Sildenafil (Viagra), whose function is to inhibit a substance called Phosphodiestarase-5 or PDE5 - was initially used only for the treatment of angina pectoris (a cardiovascular problem) and pulmonary hypertension. Incidentally, it was discovered that it could help the issue of erectile dysfunction too, and thus became hugely popular in the late 1990s. The little blue pill, as it was called, ruled more than 50% of the sexual dysfunction market for more than 15 years. However, as its popularity and usage increased, so did the scientific information about this drug, and now there is an increased speculation that it could be contributing to skin cancer, specifically melanoma. This is considered as one of the most dangerous forms of skin cancer and so the safety of Viagra sildenafil came into question.

Research has shown that users of Viagra sildenafil are at a higher risk for developing melanoma. There are reports stating that about 45 million men are at an increased risk of developing melanoma due to associated Viagra sildenafil use. The risk of developing melanoma was found to increase in men using ED drugs by 21%. Though there is no conclusive proof as yet that sildenafil is the cause, research is increasingly pointing out its role in melanoma. For example, PDE5 inhibitors (sildenafil, tadalafil, vardenafil) are known to promote melanin synthesis, which may exacerbate melanoma development.

The analysis of biochemical pathways of this correlation suggests that Viagra sildenafil triggers a special biochemical action involving the growth-stimulating cGMP (cyclic guanosine monophosphate (an intracellular signaling molecule) which in turn promotes the growth of malignant melanoma. In normal cases, the enzyme phosphodiesterase type 5 (PDE5) regulates the cGMP preventing any adverse cellular activity. Sildenafil inhibits the production of this enzyme, and potentiates a specific cGMP-related biochemical mechanism (called cGMP-cGKI pathway) , and thereby contributes to malignant melanoma. This has been proven to happen in mice and is yet to be proven in humans.

However, there are also reports which suggest that this does not happen due to Viagra sildenafil per se, but due to greater exposure to sun, which even otherwise increases the chances of skin cancer in general, and melanoma in particular. The countering theories also say that the population in which skin cancer was detected, comprised mostly of high income individuals who spent a significant amount of time holidaying on beaches and are not connected to the extensive use of Viagra per se. 

Therefore, though there is a correlation that has been established, there is no definite proof that Viagra sildenafil causes skin cancer. It is however, advisable to use ED drugs after medical consultation. If a patient has a genetic predisposition to develop melanoma, it is advisable to seek medical advice before the use of ED drugs. Occasional use should not be harmful. A bent penis can be a source of genuine concern for a man. Yes, a little curvature in the penis is common, but when the curvature is severe, it can create both physical and emotional problems. Most men with a severely bent penis are considered to have Peyronie's disease, named after the doctor who first described it medically. Since sexual and penis health can be impacted by Peyronie's, it helps to be aware of what treatments are out there for this condition.

In 2016, the World Journal of Men's Health published "Nonsurgical Interventions for Peyronie's Disease: Update as of 2016," an overview of the existing treatments for a bent penis. Following is a distillation of the information contained in that article.

Background

First, a little about Peyronie's disease. The Mayo Clinic describes this disorder as "the development of fibrous scar tissue inside the penis that causes curved, painful erections." This scar tissue usually develops due to injury to the penis. For example, a man might be struck in the penis by a baseball, might slam his erection into a hard surface or might simply experience repeated rough (if pleasurable) handling during sex. Such activities create trauma in the penis; as it heals (even from small traumas like rough handling), scar tissue forms. If that tissue forms repeatedly in one area, it prevents the skin from stretching properly, causing the penis to bend during an erection.

Treatments

Among the treatment options are:

1. Oral options. A wide range of pills are sometimes used in treating Peyronie's, including vitamin E, tamoxifen, L-carnitine and tadalafil. Such options are typically employed first, as they are the easiest treatment available. Typically, these therapies show results in cases where the scar tissue is less intense. Some of these options are also available for topical application directly on the penis skin.

2. Injections. More persistent cases may respond to the injection of medication directly in the area where the scar tissue build-up is pronounced. As of this writing, only one drug is approved for this method in the U.S. Many men resist this treatment as, understandably, the idea of injecting a needle into the penis does not appeal to them.

3. Traction. Traction therapy essentially works to pull the penis back into shape. A man wears (for several hours each day), a device that attaches to the penis and exerts force to pull it in the opposite direction of the curve. Impotence is not a matter of joke. Medically called erectile dysfunction or ED, this condition affects a man’s ability to perform in bed and achieve or maintain an erection, and/or ejaculate. This condition restricts or completely forbids penetrative sex.

Though impotence can occur in men of all ages, its risk increases with age. The common causes of impotence range from the emotional, psychological to the physical like:

  1. Diabetes: It can causes nerve damage, impairs blood flow and screws up hormone levels.

  2. Vacuum erection devices are a mechanical way of producing an erection. They are preferred by men who do not want or cannot use ED drugs. These devices work by making the penis rigid by the use of a vacuum pump sealed around it that draws up blood. The blood is then prevented from leaving the penis by the use of an accompanying rubber band which is rolled to the base of the penis before penetrative sex can begin.
  3. The other treatments include testosterone replacement, blood vessel surgery and psychological counselling. It is especially useful for younger men who become impotent due to anxiety and stress. Alternate therapies like acupuncture have also been very effective for treating impotence. Lifestyle changes like denouncing drugs, smoking and alcohol can improve the condition significantly. So can exercising more and eating healthy.

The good news is that impotence is a treatable condition. But the challenge is not to take it lightly and ignore it because it can signal serious underlying problems like heart disease and diabetes.
