In t2 dm there is insulin resistance but there are no antibodies against it. Insulin resistance can be reduced by maintaining near normal body weight and a bmi of 23kg/m2. Exercising for more than 1 h a day is a good method of improving insulin resistance. Drugs like metformin and pioglitazone also help in reducing the resistance.

Once if a diabetic patient starts insulin. Can he starts to oral medicine? or always on insulin?
If the insulin has been started because of the failure of oral medicines then generally one has to take insulin life long. At times insulin is given prior to some surgery or a person is shifted to insulin because of some complication, then the patient can be shifted back to oral drugs.

Mr. Lybrate-user, if you have t1 dm, then only treatment is insulin injection, no other option. If you have t2 dm and still you have been given insulin that means your diabetes must have been really in bad control. Diabetes occurs either due to lack of insulin or resistance to its action (t2 dm), or due to total absence (t1dm). Best medicine for diabetes in fact is insulin, it has no real side effects, our own islets of langerhans in pancreas produce it.
