You can use your antibiotic for some other reason while your having breastfeeding but if there is breast engorgement if you are going to use antibiotic for it it might cause antibioma so make sure you don't have engorged breast while taking antibiotics.

You can see dolo is paracetamol but the dose is important. Paracetamol can be taken in 500 mg too. And other doses. But do try to have gap of atleast 6 hours between two pcm doses.
In my opinion you'll need proper assessment of wound along with debridement/dressing as needed. Kindly consult a surgeon in person if not so then consult online with wound photos and all reports with previous treatment history. It should get better soon.
