What is amblyopia or lazy eye? Amblyopia is decreased vision in one or both eyes due to abnormal development of vision in infancy or childhood. In amblyopia, there may not be an obvious problem of the eye. Vision loss occurs because nerve pathways between the brain and the eye aren't properly stimulated. The brain, learns to see only blurry images with the amblyopic eye even when glasses are used. As a result, the brain favors one eye, usually due to poor vision in the other eye. Another word for amblyopia is 'lazy eye'. It is the leading cause of vision loss amongst children.

The major causes of Amblyopia are as follows:

1. Eye Misalignment: The most basic reason for amblyopia is misalignment of the eyes. In case both your eyes are not aimed in the same proportion or direction, the image from the affected eye is turned off by the brain. Only the better eye functions. Double vision is restricted. The persistence of this defect for a prolonged time, makes the eye lose connection with the brain's visual cortex. Amblyopia develops.

2. Imbalanced error of refraction: If the refractive blunder of both the eyes is not the same, both eyes can't be in the center at the same time. Amblyopia happens when one eye stays out of the center since it is more myopic, farsighted, or astigmatic than the other. One eye becomes dominant. This results in the neglect of the unfocused eye, which neglects to associate with the mind. Treatment with glasses or contact lenses to adjust the refractive blunder of the eye is required.

3. Cloudiness in the visual system: Any kind of obstruction or cloudiness in the normal functioning of the eye tissues can be another reason, which leads to amblyopia. Disorder of any kind which blocks a clear image from being focused is capable of blocking the formation of a clear image on your retina. This may lead to amblyopia. This is perhaps the most severe type of this disease.

How can amblyopia be cured?
For treating amblyopia, a child must be compelled to utilize his affected, weaker eye. This can be done by correction of any disorder of the affected eye such as refractive error, problems in the retina, etc. The goal is to correct the amblyopic eye. The normal eye is usually covered up by a patch, so that only the affected eye can be used for vision. The patching up may continue for weeks, months or years until the best vision gets restored. Lazy eye or amblyopia is a childhood disorder wherein the child s vision does not develop properly, especially in one of the eyes. If the condition is not treated at the very onset, it can lead to impaired vision in the concerned eye. As the child grows up, the brain will fail to pick up images from that eye.

Causes:

This disorder is known to start when one eye has better vision than the other eye. An example would be if one of the eyes is affected by farsightedness (unable to see objects at a closer range) and the other is normal. In the case of a child, when the brain receives images from both the eyes, it will ignore the faulty eye s image. When this situation repeats itself over months or years, it might lead to absolute deterioration of vision in the faulty eye.

Strabismus is another cause of amblyopia wherein there is a significant misalignment in the eye; one of the eyes may turn outward or inward. This does not allow the eye to focus properly on an object, thus leading to double vision. The brain again ignores the image from the faulty eye that causes vision deterioration in that eye.

Treatment:

The treatment begins by correcting problems in the affected eye and then putting a patch on the normal eye. Initially, the child may have difficulties adjusting to this new method, but bear in mind that it is absolutely necessary to wear the patch. The entire treatment process may range over a few weeks to even months. Regular visit to the doctor is of utmost importance.

In mild cases, an eye drop called atropine may be prescribed for the good eye. The child may also require glasses to improve focus. In case of cataracts, a surgery might be needed to treat it.

Crossed eyes are another such common disorder that affects the eyes. In medical terms it is also known as Strabismus. This condition is a result of miscommunication between the brain and the eye muscles that result in misalignment of the eyes. Corrective glasses can also be used to treat this condition in cases where it has been triggered by excessive farsightedness. In some cases, medication may also be injected into the eye to relax the eye muscles.

Surgery to correct this disorder involves correcting vision by strengthening or weakening the eye muscles. To do this, a surgeon must first make a small incision in the outer layer of the eyeball to reach the affected muscles. To strengthen the muscle a small section is removed from the muscle and the remaining part is then rejoined. This makes the muscle shorter and forces the eye to turn towards that side. Alternatively, the doctor may make a partial cut across the muscle to elongate it and let the eye turn further away from it.

Amblyopia or Lazy eye is the eye condition in which vision is reduced and cannot be corrected with glasses or contact lenses. All the images seen by the amblyopic eye are not fully acknowledged by the brain. It always affects only one eye, but may be accompanied with reduction of vision in both eyes. It can cause loss of vision, including loss of depth perception and two-eyed 3D vision. It's treatment can yield improvements at any age, but early detection can offer the best chances for a cure. Recent research has proven that lazy eye treatment can be successful in older children and adults. Comprehensive vision examinations are required for infants, toddlers, and pre-school children for the detection of lazy eye or amblyopia.

Amblyopia is commonly known as 'LAZY EYE'. It is a vision development disorder, which affects eyes and the brain during infancy and childhood. It signifies that the eye does not achieve a normal visual acuity even with refractive correction in form of glasses or contact lens. During normal visual development, the eye and brain learns to 'see' and develop binocularity, that is the ability to perceive depth (Stereoacuity). This occurs in the first 8 to 10 years of life. Each eye transmits a clear and identical image from retina to the brain, which fuses the two images into a single image with 3 dimensions ( adding depth). When the image is formed on the retina of two eyes is too dissimilar, the brain cannot fuse the two images and suppresses the more blurred image. As a result, the worse eye does not learn to 'see' and becomes 'lazy'. This condition is seen in 1-4% of population.

The common causes for dissimilar images or Amblyopia are :

1. Misalignment of the eyes or squint: It is the primary reason for Amblyopia is misalignment of the eyes. In case both your eyes are not aimed in the same direction, the image captured by each eye is different , resulting in diplopia. The brain will see from the dominant eye and suppress image from the other eye. Over a prolonged period of time, the eye dissociates from the visual cortex of the brain.

2. Refractive Amblyopia:

a. Unequal refractive error in both eye. When the eye power of both eyes differ by more than 1.5D of spherical equivalent, which results in dissimilar images between the two eyes. As a result, the visual development in restricted in the worse eye.

b. High refractive error or astigmatism in both eyes, since there is a blurred image in both eyes, the normal vision development does not occur in either eyes resulting in bilateral lazy eyes.

3. Cloudiness in visual system or visual deprivation Amblyopia: Any kind of obstruction or cloudiness in the normal visual axis of the eye tissues can lead to Amblyopia. Disorder of any kind, which blocks a clear image from being focused blocks the formation of a clear image on your retina. Commonly due to congenital or developmental cataracts or corneal opacities or prolonged closure of one eye in early childhood( drooping of one eyelid, eyelid swelling, patching of one eye).

Treatment for Amblyopia

For treatment of Amblyopia, a child must be compelled to utilize his affected, weaker eye. This can be done by correction of causative factor such as, treating refractive error with glasses, surgical correction of misalignment of eyes followed by patching and vision therapy, cataract surgery.

Patching or occlusion of the good eye

This is done to force the brain to pay attention to visual input from the worse eye. This allows nerve connections between affected eye and brain to develop due to, which the eye 'learns to see'. Eye drops such as atropine are also used for blurring the vision of the good eye. Some form of vision therapy may also be required to train both eyes to see together and develop some degree of depth perception.

To be effective, treatment should be implement as early as possible , within the period of normal visual development best within first 8 years of life. Some visual gain has been seen in selected patients up to age of 14 to 18 years. This also highlights the importance of a comprehensive eye examination as a routine in young children.
