Trichomonas Vaginalis - How To Avert It?
Trichomonas Vaginalis, a virus, which affects almost 7.4 million men and women each year. This virus can cause a sexually transmitted disease upon contact with a person who may be carrying the same. This condition is more common in women than it is in men and causes a variety of symptoms. Let us learn more about this infection.

Symptoms
This disease does not have any obvious symptoms until the problem becomes pretty severe. The symptoms for men include irritation inside the penis, discharge, and a burning sensation upon urination and ejaculation. The symptoms for women include a strong odour filled discharge that may also be greenish yellow in colour, pain in the lower abdomen in case the infection is extremely severe, itching and irritation in the vagina, and pain or discomfort during sexual intercourse.

Diagnosis
The gynaecologist or the general physician will usually carry out the diagnosis with the help of blood and urine samples that will be sent to the lab for testing. These tests will help in determining the spread and severity of the infection. The doctor will also use vaginal or urethral fluid in order to diagnose the issue with the help of further lab tests. Most doctors will tell you that it is harder to detect the presence of the virus in women than it is in men.

Treatment
The treatment of this disease is done with the help of metronidazole, which can be used to treat trichomoniasis in case the patient is pregnant. It is usually recommended that the sexual partner of the patient should also be treated for the infection so as to stem its spread. Also, the doctors will recommend abstinence from sex for the duration of the treatment. Antibiotics will also be prescribed for both partners. Usually, one will be asked to wait for all the symptoms to disappear before becoming sexually active again.

Complications
There is a risk of contracting the HIV virus in case pregnant women do not get treated for this disease on time. It can also cause premature rupture of the various membranes that protect the foetus, which can, in turn, lead to premature delivery of the baby. Further, women who are suffering from this disease are at higher risk of passing on the HIV virus to their partners. This happens when the woman is also infected with the HIV virus.

Prevention
There are various ways to prevent this disease including the proper use of condoms and limiting the number of sexual partners. Genital inflammation and pain should be checked by a doctor immediately to rule out the possibility of this STD. Female dyspareunia refers to painful sexual intercourse. It is estimated that 8 to 21% of women experience this at some point in their lives. Pain during or after intercourse is disturbing and restricts partners need for intimacy. It leaves the partners with an unpleasant experience and may lead to frustrations, anxiety and fear of sex. There is little awareness among couples about dyspareunia and thus a painful experience is sometimes confusing for the other partner and often invites fights and arguments rather than support and understanding.

Women with dyspareunia suffer pain in genitalia or deeper in the pelvis, vulva or vagina. It is more common among women after menopause; however, many women experience pain right from their first sexual attempt or just after initiation while some at deeper penetration. Causes of Dyspareunia could be medical, psychosocial or both. Medical conditions can be cured but often there are underlying personal and psychological factors which need to be addressed usually by counsellors or sexologists.

Diagnosis of the problem begins with a physical examination of the vulva. Apart from this an internal pelvic examination may also be required. A doctor takes into account the nature, extent and duration of pain in determining possible causes and deciding the mode of treatment. During interaction with the patient, various other psychosocial factors are also revealed that must be addressed for holistic treatment.

What is vaginal dryness and how can it affect you?
Vaginal dryness is often a result of menopause and it is capable of affecting women of any age. Although it feels like a minor irritation, it can have a huge impact on a person's sex life. Normally, the vaginal walls are lubricated with a layer of clear fluid. The 'oestrogen' hormone is used to maintain this thin layer of fluid, thus keeping the vaginal lining thick, elastic and healthy. However, menopause brings about a condition known as vaginal atrophy, which is a drop in the levels of oestrogen and this greatly reduces the level of vaginal moisture that is available. Improper lubrication of the vagina can result in painful sex which in turn affects libido, arousal and desire.

Treatment: The most common treatment for this condition resulting from low levels of oestrogen is Topical oestrogen therapy. In this, hormones which are no longer secreted by the body are replaced by the topical oestrogen. Even though Topical Oestrogen Therapy is helpful in relieving the symptoms of vaginal dryness, the treatment pales in comparison to oral oestrogen hormone therapy (HT).

Usually, women mostly use one of the three types of vaginal oestrogen listed below:

Vaginal oestrogen tablet: A tablet is inserted into the vagina using a disposable applicator. The procedure is carried out once daily during the initial two weeks of treatment, after which it can be inserted twice a week.
Vaginal oestrogen ring: A soft and flexible ring is inserted inside the vagina, either by the patient or the doctor. Oestrogen, in the form of a steady stream, is then released directly to the vaginal tissues by this ring. Replacement of this ring is done every three months.
Vaginal oestrogen cream: An applicator is used to apply cream to the vagina daily. The frequency of application can be reduced as per doctor's advice.
Side effects and aftercare: Side effects such as breast pain and vaginal bleeding are common when any form of oestrogen product is used. Vaginal moisturisers should be used to keep the vagina lubricated. Use of water-based lubricants during sex can make it more enjoyable. Use of bubble baths, scented soaps, douches and lotions around the sensitive area of the vagina should be avoided as these products can worsen the problem.

Vaginismus can be primary or secondary depending on the concerned woman s previous sexual experiences. Vaginismus can be linked to an earlier sexually painful experience, sexual trauma or psychological factors. In some cases, the pain tends to exist without any direct physical cause.
The causes of the pain might vary from women to women and might occur in some cases with only one specific partner and not with others.
Sometimes the pain might stem from underlying infections or any other underlying complications.

Treatment-
The following treatments can be useful to treat Vaginismus:

If the pain arises whenever something penetrates the vaginal walls, progressive desensitization can be useful to treat the pain. The process aims to loosen the vaginal muscles so that you don t experience any pain during sexual intercourse.
Kegel exercises (at least 20 of them) can help in the initial stage to relax the vaginal muscles. After you exercise for a few days, you should try inserting one finger inside your vagina while doing the exercises. Try doing it gently and if it still hurts, try doing it with a lubricating jelly. This continuous procedure will help in the contraction of the vaginal muscles which eventually relax.
