Definition

Uterine fibroids are noncancerous growths of the uterus that often appear during childbearing years. Also called leiomyomas (lie-o-my-O-muhs) or myomas, uterine fibroids aren't associated with an increased risk of uterine cancer and almost never develop into cancer.

Uterine fibroids develop from the smooth muscular tissue of the uterus (myometrium). A single cell divides repeatedly, eventually creating a firm, rubbery mass distinct from nearby tissue. Fibroid location, size and number influence signs and symptoms:

TYPES

Submucosal fibroids: Fibroids that grow into the inner cavity of the uterus (submucosal fibroids) are more likely to cause prolonged, heavy menstrual bleeding and are sometimes a problem for women attempting pregnancy.
Subserosal fibroids: Fibroids that project to the outside of the uterus (subserosal fibroids) can sometimes press on your bladder, causing you to experience urinary symptoms. If fibroids bulge from the back of your uterus, they occasionally can press either on your rectum, causing a pressure sensation, or on your spinal nerves, causing backache.
Intramural fibroids: Some fibroids grow within the muscular uterine wall (intramural fibroids). If large enough, they can distort the shape of the uterus and cause prolonged, heavy periods, as well as pain and pressure.

Symptoms

In women who have symptoms, the most common symptoms of uterine fibroids include:

Heavy menstrual bleeding

It is very common for women to have heavy and painful periods or have a feeling of fullness in the lower abdomen. Although, it may not sound very alarming these could be the symptoms of uterine fibroids. These are the most common types of benign tumours found in women. The fibroids are basically some tissues and muscle cells that grow within the uterus, outside the uterus, or along the wall of the uterus. The fibroids are usually benign and asymptomatic and do not require any treatment unless they cause problems.

Know the causes

Though the exact cause of fibroid formation is not known, it is believed that the female hormones estrogen and progesterone have a role to play in their formation. Fibroids are formed only when a woman is producing these hormones and they are not seen in women in non-reproductive age i.e. before starting of menses or after stoppage of menses (menopause). If fibroids are persisting even after menopause or especially if increasing in size then it is an alarming sign. Such a fibroid needs to be taken care of immediately.

What are the symptoms?

Fibroids often remain quiet for long periods of time. They cause nonspecific symptoms in the pelvis and abdomen including:

Fullness in the abdomen
Low back pain
Irregular menstruation
Cramping with menstruation
Painful sex
Increased urgency to urinate
Anemia, leading to tiredness and weakness
Infertility Diagnosing the fibroids
When these symptoms are recurrent, it is good to confirm the diagnosis. This can happen with a pelvic exam followed by ultrasound scanning to confirm the size and location of the fibroids. A blood test also may be done to confirm anemia, which is common due to heavy periods.

Treatment Options

Management of fibroids can range from doing nothing to periodic monitoring to surgical removal.

If pain and heavy bleeding are the only symptoms, then pain killers like ibuprofen should suffice for symptomatic relief. Anemia, if severe, may require iron supplementation.
Embolization is an option which shrinks the fibroid, at the same time preserving the uterus. The blood flow to the fibroid is cut off, thereby preventing its further growth. It takes about 1 to 3 hours and requires some bed rest after the procedure. There could be some pelvic pain and vaginal bleeding, which will gradually subside. The fibroids may grow back, but the benefits of keeping the uterus are definitely there. This is suitable in case of single fibroid with specific blood supply.
The next surgical option is myomectomy, where the portion of the uterus which contains the fibroid alone is removed. This is done in women who still wish to get pregnant and in women who would like to retain the uterus. This can also be achieved laparoscopically( key hole surgery). In such cases, fibroids are cut into small pieces and then removed from the body. This procedure should be done by a doctor specialised in advanced gynecological endoscopy as all this cutting should be done in a bag to avoid any spillage of cells inside the abdominal cavity. This procedure is known as Laparoscopic Myomectomy with In-bag Moecellation.
In women who have crossed their pregnancy phase, hysterectomy or complete removal of the uterus is advised. In these women, the bleeding and pain may not have subsided even after years of treatment with hormones. The growing fibroids could be pressing on the adjacent organs, causing pressure. This is the only definitive treatment and should be done in women have completed their family and don t desire to be pregnant.
Myomectomy and hysterectomy may be done laparoscopically or with an open method depending on the overall health, the size and location of the fibroids in the uterus.

Fibroids are benign (non-cancerous) tumours of the womb (uterus). They are also known as myomas. They are growths of smooth muscle and fibrous tissue. The size of fibroids can vary from as small as a pea to that of a melon. At least one in five women develop a fibroid at some stage of their life, usually between the ages of 30-50 years old.

Fibroids are named according to where they are found in the womb. There are three types:

Intramural fibroids are found in the wall of the womb and are the most common type of fibroids found in women.

Subserosal fibroids are found growing outside the wall of the womb and can become very large. They can also grow on stalks (called pedunculated fibroids).

Submucosal fibroids are found in the muscle beneath the inner lining of the womb wall.

Symptoms

The majority of women with fibroids show no symptoms. However, if symptoms develop, they may experience one or more of the following:

Heavy or painful periods- in some cases this can lead to anaemia

Discomfort, or swelling, in the lower abdomen, particularly if the fibroids are large

Urinating frequently, usually, if the fibroids are pressing on the bladder,

Constipation which can be caused by the fibroids pressing on the rectum (large intestine leading to your anus), and

infertility Very rarely, fibroids can cause problems during pregnancy and labour

Diagnosis

As fibroids rarely have symptoms, they are often found during a routine gynaecological (vaginal) examination. If fibroids are suspected, an ultrasound scan can be used to confirm a diagnosis. A trans-vaginal scan is sometimes used to diagnose fibroids. It involves a small scanner being inserted into the vagina to take a close-up image of the womb.

Fibroids near your inner lining, and those within the cavity of your womb can be seen directly using a hysteroscope (small telescope used to examine the inside of your womb). To look at the size and shape of the outside of your womb a laparoscope (small flexible tubing used to look inside the abdomen) is used.

If you do not have any symptoms from your fibroids, treatment may not be necessary. Some women with minor symptoms, such as heavy periods, choose not to have treatment as their day-to-day life is not affected. After the menopause, fibroids often shrink, and your symptoms will either disappear or ease slightly.

Treatment

To treat fibroids, your GP may recommend medication. However, in more severe cases, surgery can be considered.

Treatment with medication
Tranexamic acid these tablets are taken from the start of your period for up to four days. The tablets work by helping the blood in your womb to clot, which reduces the amount of bleeding.
Anti-inflammatory medicines such as ibuprofen and mefanamic acid help to ease your periods and are taken for a few days during your period. They work by reducing your body s production of a hormone-like substance, called prostaglandin, which is linked to heavy periods. They are also painkillers but are not a form of contraceptive. Common side effects include indigestion and diarrhoea.
The contraceptive pill is often taken, during your period, to prevent your menstrual cycle (period) from occurring. Some contraceptive pills also help to reduce any period pain you may experience.
Levonorgestrel intrauterine system (LNG-IUS) is a small plastic device that is placed in your womb and slowly releases the progestogen hormone called levonorgestrel. It prevents the lining of your womb from growing quickly so your bleeding becomes lighter. Possible side effects of LNG-IUS include; having irregular bleeding that may last for more than six months, acne (inflamed skin on the face), headaches, and breast tenderness. The most effective medication to treat fibroids is an injected hormone medicine called
Gonadotropin releasing hormone agonist (GnRHa). GnRHa works by preventing your menstrual cycle (period), but it is not a form of contraceptive. However, it does not affect your chances of becoming pregnant after you stop using it. Common side effects include menopause-like symptoms, such as hot flushes, increased sweating, and vaginal dryness. Osteoporosis (thinning of the bones) is a less common side effect.
Surgical procedures -Surgical procedures, for treating fibroids, are usually considered for large fibroids (>5cm) or if medications are ineffective in relieving symptoms. Common surgical procedures that are used to treat fibroids include:
Myomectomy involves surgery to remove the fibroids from the wall of your womb. A myomectomy is an alternative to having a hysterectomy, particularly for women still wishing to have children. However, the procedure may not always be possible as it depends on your individual circumstances, such as the size, number and position of your fibroids.
Hysterectomy involves surgery to remove the womb. A hysterectomy is not usually necessary unless the fibroids are very large or you have severe bleeding. A hysterectomy may be advised in order to prevent fibroids recurring.
Endometrial ablation is removal of the womb lining. It is usually only carried out if your fibroids are near the inner surface of your womb. The affected womb lining is removed, which may be done in a number of ways, including using laser energy, a heated wire loop, microwave heating, or hot fluid in a balloon.

Ectopic pregnancy: When the fertilised egg develops outside the uterus, it is known as an ectopic pregnancy. In such cases, there may be pain in the region during sexual stimulation and activity.
Vaginal infections: Pain during sexual intercourse may also be experienced due to vaginal infections and other yeast infections as well as a condition known as Vaginismus which can lead to involuntary twitches and spasms that can arise because of fear of getting hurt.
Lack of lubrication: Excessive dryness and lack of lubrication, especially during stimulation can lead to pain during intercourse as well, as the opening does not expand or accommodate the male member properly. In such cases, one can use a lubricant.

According to contemporary medical studies, loss of sexual desire is more common in women than in men. As many as 3 in 10 women have reduced or limited sexual interest in any kind of sexual activity. While the medical term for this problem is Hypoactive Sexual Desire Disorder (HSDD), it is generally understood as a loss of libido.

There are multiple factors that contribute to this problem including psychiatric or physical disorders, relationship issues, hormonal disorders etc. Let us look at the causes of this disorder at length:

1. Medical disorders:

a) There are conditions such as endometriosis, fibroids and thyroid disorders that may have a negative impact on the sexual drive of a woman.

b) Certain medicines such as antidepressants, oral contraceptives, and medicines that lower blood pressure may also negatively impact the sexual drive in women.

2. Hormonal disorders:

a) Dryness of the vagina and pain during sexual intercourse may affect sexual drive both mentally and physically.

b) After menopause, drop in the count of testosterone hormone also affects sexual drive which can result in loss of libido.

c) With advancing age, the level of androgens decrease, and hence results in a lack of sexual desire.

3. Lifestyle disorders:

a) In some of the cases, lack of emotional support in a relationship plays a significant role in the loss of libido. Post baby 'coolness' may also act as a driving force for loss in libido.

b) In today's era, changing lifestyle is drastically affecting our lives. Following an unhealthy lifestyle may result in increased weight that may affect self-image, and indirectly may also affect the sexual desire.

c) Stress and job, family pressure and exhaustion may also influence sexual desire of a woman.

4. Psychological factors

a) Increasing cases of depression, anxiety, frustration, stress is also an important cause of loss of libido in the contemporary society.

b) After childbirth, new moms tend to suffer from lack of sleep, insufficient lubrication of the vagina and exhaustion, which may affect her sexual desire.

The first and foremost step towards dealing with it is to make your partner understand your problem. Then, it is important to consult a doctor to find out the underlying cause, thereafter, treatment of the cause can normalize the condition. Since there are multiple causes of the problem, there are different ways by which the loss of libido can be easily managed and treated. Some of them are discussed below:

1. Treatment of hormonal disorders: The doctors will suitably treat problems like thyroids or recommend surgical treatment for fibroids to manage HSSD.

2. Change in medications: Since some of the medications are responsible for the lack of libido, the doctors will replace the medications to reverse this lack.

3. Addressing the psychological factors: Your doctor will either arrange for suitable psychiatric sessions or ask your partner to coordinate mutually.

4. Sex therapy/Relationship counseling or Testosterone therapy: While sex therapy or relationship counseling brings back the interest by effectively counseling the individual, Testosterone therapy involves off-label usage of testosterone hormone since it is not recommended by FDA.
