Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Sacralization of l5 vertebrae is x ray finding and can be seen in many people. Si joint pains are also different from lower back pain although both can happen in same patient. I will actually need: 1) a detailed neurological (motor, sensory, deep tendon reflexes) charting done by a trained physical therapist 2) your mri images and reports in an attachment this will help us in determining the cause of your weakness and maybe pain. Also please get esr, crp, serum vitamin d tests empty stomach and send all reports across to me. Do not hesitate to contact me if you need any further assistance.

The conservative management would be to keep ice in that inflamed area during one time in a day and also to sit in a hot water bowl where the penetration of the heat would help to reduce the inflammation. And to reduce the weight falling on the inflamed surface we suggest patients to use air cushion pillow (also called as doughnut pillow) where in the buttock's weight will not fall exactly on the painful pubic bone and that would automatically reduce the pain.

Hi Mr. lybrate-user first of all diagnosis has to be done. For that you have to consult orthopaedic surgeon and have x-rays done for back and shoulder. In the mean time continue with hot fomentation (if you are not diabetic), avoid shoulder movements and forward bending, squatting and sitting cross legged. If need be you may require mri. But let your doctor decide.

I have pain in my right venus dimple. By venus dimple I mean the two indentations or small holes which are present near the sacro iliac joint. What could be the reason for it? I have consulted a physio and he is telling that I have anterior pelvic tilt on the right side. Can someone please explain properly to me the entire thing in detail?

An anterior pelvic tilt is when your pelvis is rotated forward, which forces your spine to curve. It?s often caused by excessive sitting without enough exercise and stretching to counteract the effects of sitting all day. If you have an anterior pelvic tilt you may notice that the muscles in the front of your pelvis and thighs are tight, while the ones in the back are weak. Your gluteus and abdominal muscles may also be weak. All of this can cause: lower back pain hip and knee pain incorrect posture forced hip and knee rotations. IF YOU HAVE IT YOU CAN TEST IT BY THOMAS TEST AS FOLLOWS- Sit on the edge of a sturdy table. Lie back onto the table so that your legs hang off the table at the knee. Pull one of your legs in toward you, holding under your knee and bending your leg until it rests against your chest. Repeat with the other leg. If your pelvis is correctly aligned, the back of your resting leg will touch the table when you get into this position. If you need to extend the resting leg or rotate your leg or hip in order to touch the table, your front thigh muscles are tight. This likely signals a tilted pelvis.
