BHMS
Homeopath,
I am suffering with tight foreskin of penis and foreskin stuck to down side. what to do? Please advise me.
Dear Lybrate User, You may be suffering from a clinical condition called PHIMOSIS. The most conventional method to get rid of it is surgery. However, if you are unwilling to undergo surgery then you can try the following process at home. 1) Buy Betamethasone cream from local allopathic shop. During buying you should check the composition of the cream to confirm whether it contains 0.05% Betamethasone in it. If you cannot find 0.05% Betamethasone cream then you can buy any local Anaesthetic Gel (e.g. Lidocaine, Lignocaine, Xylocaine) from allopathic retail shop. 2) Apply a little amount of that anaesthetic gel at the lower part of the prepuce of penis where the foreskin remains attached. 3) After application of the gel, retract the foreskin backwards as far as you can. Due to the anaesthetic property of the gel you will be able to retract the foreskin backwards to some extent more than you can do in normal condition. 4) Release the foreskin when you feel pain 5) Again repeat the same process. Do it continuously for at least 30 minutes in a day. Within a month you will feel that your foreskin has become loose

I can't take the head of my penis out of its skin when it's horny. Not even when I masturbate. I only can take it when it's not horny. But why? I've tried many times to take the head of my penis when its horny but it hurt so much and I couldn't even take a move. So all the time I masturbate with the skin on it. But I can take the head out of its skin when its not horny but it struggles when I try to put it inside. But once I took it out of the skin when it's not horny and I made it horny but it hurt so much and I couldn't even move the skin cause it was so tight. And then I put it inside when it wasn't horny but the whole skin didn't cover the head that time. Like some of the skin has rolled it or something. Whys that? Is there any solution for this? How can I take it out when it's horny? What should I do?

How to use and when to use Manforce stay long gel for men.
What are the benefits.
Manforce Staylong Gel, a local anesthetic drug of the amino amide type, relieves pain of normal intact skin and numbs skin to pain from injections and other medical procedures. It works by blocking nerves from transmitting painful impulses to the brain. In its injectable form, it is used in dentistry. It is also often used in combination with lidocaine as a topical preparation for dermal anesthesia, for treatment of conditions like paresthesia. It is also commonly used for intravenous regional anaesthesia due to its low cardiac toxicity. The recommended dose for adults and teenagers is 1 to 2.5 gm, applied as a thick layer to the skin area. Dosage for children is based on their body weight and must be determined by doctor. Take care.

Liposuction - Which Type Should You Go For?

Liposuction - Which Type Should You Go For?

Liposuction is a type of cosmetic surgery that removes excess fat from some parts of the body to improve your overall body contour. Essentially, this cosmetic procedure involves insertion of small, thin but blunt-shaped needles under the skin in those areas that have excess fat. Fat is then suctioned out through these needles by moving them from one place to another under the skin. Also called as “lipo”, this type of cosmetic surgery addresses the following body parts;

  1. Thighs
  2. Hips and buttocks
  3. Fat located on abdomen and waist
  4. Fat on the upper arms
  5. Fat on neck, chin and cheek area
  6. Fat deposited on the back, inner knee, chest, calves and ankles

Liposuction can be performed along with other cosmetic surgeries such as facelift, breast reduction, tummy tuck etc. Please note that liposuction cannot be used to reduce obesity, diabetes and other weight-related disorders.

Types of Liposuction
In recent years, this surgical procedure has been made less painful by developing newer techniques. These are:

  1. Tumescent Liposuction- This involves applying a local anesthetic on the area where the procedure has to be done. A combination of chemicals e.g. lidocaine and epinephrine is then inserted under the skin. This numbs the precise area where liposuction will be done.
  2. Ultrasound liposuction: This procedure involves melting of the undesired fat by ultrasound. This form of liposuction is suited for fat deposited on the neck, upper abdomen, sides and the back area.
  3. Laser liposuction: Low-energy waves are sent under the skin to liquefy the fat that is later removed through the cannulas.

Liposuction is not an elaborate procedure and does not require overnight hospitalization most cases. The patient is administered a local anesthetic so that she does not experience any pain. Overnight hospitalisation may be required if the volume of fat to be removed is large.

Post-liposuction care
Post this surgery, expect your body to swell. The parts of your body where liposuction has been carried out will display swellings, bruises etc. You may even experience pain in those areas. To alleviate discomfort, patients are wrapped up in bandages, support hoses, girdles etc. Some patients may report fluids oozing from the areas affected by liposuction. To prevent infection, doctors prescribe antibiotics to such patients.
Swelling and bruises may continue for the next 7- 10 days post this surgery. Patients may have to wear the bandages, girdles, wraps etc. for the next 3-4 days.

Effectiveness
Liposuction results in a better shape if the volume of fat removed is small. If the patient regains weight after liposuction, fat reappears in those areas which have not been treated.

Herpes Zoster

MBBS, DHMS-Harvard, Masters in Clinical Dermatology
Dermatologist, Chandigarh
Herpes Zoster

Herpes zoster or Shingles is a viral infection that causes a painful rash. Although shingles can occur anywhere on your body, it most often appears as a single stripe of blisters that wraps around either the left or the right side of your torso.

Shingles is caused by the varicella-zoster virus — the same virus that causes chickenpox. After you've had chickenpox, the virus lies inactive in nerve tissue near your spinal cord and brain. Years later, the virus may reactivate as shingles.

While it isn't a life-threatening condition, shingles can be very painful. Vaccines can help reduce the risk of shingles, while early treatment can help shorten a shingles infection and lessen the chance of complications.

Symptoms

The signs and symptoms of shingles usually affect only a small section of one side of your body. These signs and symptoms may include:

  • Pain, burning, numbness or tingling
  • Sensitivity to touch
  • A red rash that begins a few days after the pain
  • Fluid-filled blisters that break open and crust over
  • Itching

Some people also experience:

Pain is usually the first symptom of shingles. For some, it can be intense. Depending on the location of the pain, it can sometimes be mistaken for a symptom of problems affecting the heart, lungs or kidneys. Some people experience shingles pain without ever developing the rash.

Most commonly, the shingles rash develops as a stripe of blisters that wraps around either the left or right side of your torso. Sometimes the shingles rash occurs around one eye or on one side of the neck or face.

Treatment

There's no cure for shingles, but prompt treatment with prescription antiviral drugs can speed healing and reduce your risk of complications. These medications include:

  • Acyclovir
  • Valacyclovir

Shingles can cause severe pain, so your doctor also may prescribe:

  • Capsaicin topical patch
  • Anticonvulsants, such as gabapentin
  • Tricyclic antidepressants, such as amitriptyline
  • Numbing agents, such as lidocaine, delivered via a cream, gel, spray or skin patch
  • Medications that contain narcotics, such as codeine
  • An injection including corticosteroids and local anesthetics

Shingles generally lasts between two and six weeks. Most people get shingles only once, but it is possible to get it two or more times.

Here Are Innovative Liposuction Techniques To Get Rid Of Genetic Fat!

Here Are Innovative Liposuction Techniques To Get Rid Of Genetic Fat!

Your genetic background influences where your body fat is stored. In case, your mother or father has fat stored in the abdomen, hips, waist or in the thigh, it is likely for you to develop deposits of fat in the same regions.

Circumstances of Genetic Fat

  • You may have a proper or a balanced BMI (body mass index) but still tend to develop bulges in areas, which are likely to make you look disproportional.
  • You may become obese by development of excess fat accumulation in the lower abdomen region, referred to as pannus. Extra fat may accumulate in your lower back and hips as well, which cannot be lost in spite of taking weight losing measures.

Liposuction to get rid of extra fat:

  • Liposuction is a cosmetic surgery that allows you to remove genetic fat or any extra fat from the body, in order to change the body's shape. Liposuction does not remove cellulites and tightening of loose skin cannot be done.
  • Liposuction removes excess fat from the body by suction. Small, narrow tubes with blunt tips known as the cannulae are inserted through incisions in the skin surface. The tubes are moved under the skin to target the suction of fat deposits.

There are several types of liposuction techniques:

  1. Tumescent Liposuction: In this form of liposuction, a wetting solution such as lidocaine with a local anesthetic is injected into the patient before surgery. This causes the shrinking or constriction of blood vessels due to which it allows liposuction to be performed while the patient is under local anaesthesia. Blood loss and post surgical pains are reduced.
  2. Suction Assisted Liposuction: This mode of liposuction also known as SAL is the most primary form of liposuction. It involves drawing out of fat using a vacuum.
  3. Power Assisted Liposuction: This liposuction technique called PAL in short involves the process of SAL with an extra tool for increasing the motion of the cannula. The process is faster.
  4. Ultrasound Assisted Liposuction: This process (UAL) involves the transmission of energy via a specialized hand piece for loosening and melting of fat. A greater volume of fat can be removed via UAL.
  5. VASER: It is a variation of the UAL method where an ultrasonic cannula with grooves helps in energy disruption to improve removal of fat.

Laser Assisted Liposuction:
LAL is a newly developed liposuction technology where a laser works like an ultrasound for disruption and removal of fat cells. This mode is more efficient and involves less trauma. Liposuction is an effective way to remove unwanted, genetic fat from your body, which may be harming your body image. However, liposuction is associated with severe side effects.

Medical Therapy For Premature Ejaculation!

Medical Therapy For Premature Ejaculation!

How to manage premature ejaculation - part 4, medical treatment options

As discussed earlier, premature ejaculation (pe) occurs when a man reaches the peak of sexual excitement and ejaculates before he wants it to happen. When pe interferes with the sexual pleasure of a man or his partner, it becomes a medical problem.

Several factors such as psychological problems (eg. Anxiety), penile hypersensitivity, habituation of quick sex / masturbation, alcohol abuse, hormonal imbalance (e. G. Thyroid problem), and even genetic problems may cause pe.

Medical (allopathic) therapy for pe treatment:

Drugs used for the management of pe reduce sensitivity and anxiety, improve blood flow or even affect some chemical mediators present in the brain. These classes of drugs include local anaesthetics, antidepressants, and phosphodiesterase 5 inhibitors.

Anaesthetic compounds were the first medical treatment proposed for pe management. They were applied topically to the surface of the skin. Lidocaine-prilocaine sprays or creams decrease the sensation of the penis and increase the time taken to ejaculate during vaginal penetration. These sprays / creams are applied 10 to 20 minutes prior to sexual activity. Side effects of topical agents include partial loss of sensation of the penis, absorption in vagina resulting in vaginal numbness and irritation.

Treatment medications include selective serotonin reuptake inhibitors (ssris), which interact with a receptor (5-ht2c) present in brain and increases the production of serotonin. They also help in reducing anxiety and depression associated with pe. Through this mechanism, they prolong the time to reach ejaculation.

Several ssris have increasingly become used as" off-label" for pe. Among the available ssris, paroxetine and dapoxetine are more beneficial with lesser side effects as compared to other ssris. These medications are associated with sexual side effects including decreased fertility and erectile dysfunction. Dapoxetine is a recent ssri which acts quickly and cleared rapidly from the body. Adverse effects with ssris are usually minor and include fatigue, mild nausea, loose stools and sweating. Other side effects may include decreased sexual urge and increased tendency to suicide, especially with long-term paroxetine.

Another potential treatment option for pe are classes of medications which increase the blood flow by dilating the blood vessels of the penile region and prolong the ejaculation time (phosphodiesterase 5 inhibitors). But a headache, dizziness, flushing, body aches associated with them have limited their use in pe.

At present, clinicians need to consider all treatment modalities when evaluating a man with pe, as each patient may respond differently and experience variable side effects. Additional and more effective therapies need to be developed having least side effects.

Ayurvedic therapy for pe treatment:

Vajikaran in Ayurveda is an important treatment modality which increases sexual capacity and improves health. In Sanskrit, vaji means horse, the symbol of sexual potency and performance thus vajikaran means producing a horse's vigor, particularly the animal's great capacity for sexual activity in the individual. Vajikaran therapy reenergizes all the seven dhatus (body elements) and restores equilibrium and health. It also offers a solution to minimize the shukra (sperm and ovum) defects.

There are many formulations which are used in vajikaran. Some of the commonly used formulations include vrihani gutika, vrishya gutika, vajikaranam ghritam and upatyakari shashtikadi gutika. Vrihani gutika is one of the potent formulations while vrishya gutika is highly potent aphrodisiac used in the treatment of pe. Vajikaranam ghritam enhances the strength of penis. Upatyakari shashtikadi gutika is useful for the enhancement of the fertility.

Also, chapdraprabha vati and kaunch pak are known to increase sexual vigor and semen consistency, thereby helping to prolong ejaculation time and sex pleasure. If you wish to discuss about any specific problem, you can consult a Sexologist.
