    I am 49 yr old male dm2 + htn since 10 yrs. Pressure normal 130/ 80 with medication repace 25mg + stamlo (amlodipine) 2.5mg and blood glucose control pp 120 with zoryl 1mg at the time of break fast but fasting readings vary between 6.5 - 7.5. I take metformin extended release 1000mg after supper. But the fasting readings never fall below 6.5. I am sports enthusiast and play badminton thrice a week at expert level. With exercise in the evening the reading tend to rise as my intake goes up. Please advise me any long acting glucose control medicine for 24 hrs control well within 5.8. My hb1ac has always been well below 7.0 mostly remains 6.5. If hb1ac readings are well with in the range can ignore the fasting readings or is it equally important to maintain the fasting reading below 5.8 to prevent cardiac issues later. I also take cholesterol lowering drug fibator - half tab to maintain my ldl well below 80. Should you need any further details please write to me. Last but not the least use of sulfonylurea antidiabetic drug in the morning some cause hypo close to lunch time. I am worried of the side effects of this drug in long term. Kindly advise me alternate if any to this. My staple food is rice with daal, curry and curds. Pure veg

    Looking at your details it appears your diabetes is in check. Slight high fasting is no problem. Nowadays hba1c level is looked at as guideline to assess control. You need not change anything. If pp levels go up can add voglibose 0.3 mg three times with each meal. Otherwise you are doing well. While playing if you feel levels are low always carry candy or gluco tabs.

    You are diabetic since 8 years and you are only on metformin total 1500 mg per day. It's time to increase medication. I advice you should do hba1c test to look for your last three months average. And increase 500 mg metformin post dinner and add 50 mg sitagliptin (januvia/zita) bbf so that will take care of post prandial high sugars.

    Sir, I am a male, aged 85. Got mild heart attack about 30 yrs back. Other wise I am quite OK. Every day, I walk for about 30 mts. (about 3 kms equivalent) My body mass is OK. I am a vegetarian, no smoking and no alcohol etc.Other wise I am having a healthy life. I usually consult a good cardiologist about 2 times in an year. My regular medicines are : Morning after breakfast : Imdur 30, Repolal, Razel-10 After noon after lunch : Aspisol-75 Night after meals : Repace H I some times once on 2 to 3 months gets pain in the chest and the position of pain varies time to time. The pain lasts for 2 to 4 hours. More or less acute in the first hour and gradually diminish. The pain may come in the morning, evening or mid night and so on. The only medicine I take is Sorbitrate - 5mg under the tongue. Some 4 to 6 tablets I consume one after the other each time. I suspect this as mild heart attack. After I become normal, I usually get an ECG which is shown to the doctor. I often told the doctor about this and he chides that a cardiologist alone can diagnosis a heart attacks. My cardiologist works in a reputable hospital around 50 kms away from my place. Once, about 8 yrs back, I got a chest pain around 10 AM. I left my place by car and by the time I could meet the cardiologist it took about 4 to 5 hours. By that time there is no chest pain at all. Still, the doctor insisted me to get admitted in the hospital. There were routine check ups, ECG etc. As my chest pain often lasts 2 to 3 hours only and it completely subsides by the time I could meet the doctor, it appears redundant for the trip besides scaring my family and children. If the chest pain occurs during the night, it may take even 6 or more hours by the time I could meet the doctor. So, I almost made up my mind not to disturb my wife and son at the chest pains and manage with the Sorbitrate - 5mg tablets and preying god. Under these circumstances, is there a better alternative ? Please suggest.
