Kshar Sutra & Fistula - How Can Former Handle The Latter?

The modern lifestyle, along with providing comfort, has also made us susceptible to a host of diseases and ailments. The regular and indiscriminate intake of junk foods, high-salt content foods, and sugar-laden fizzy drinks deprives the body of necessary fiber and essential nutrients. These habits can cause the condition known as the fistula. What more, stress and sedentary lifestyles also figure among the primary causes of the disease.

Problem
Fistula can generate extreme pain and bleeding during or after passing stool, blood spots in the stool, and an uncomfortable feeling of something hanging from the anus. The disease can trigger a discharge of pus from the anus or the anal region, along with itching and a burning sensation. Constipation is an associated symptom.

Therapy
Traditional medicines, such as Ayurveda, recommend the Kshara Sutra treatment which is designed to offer both preventive care and relief from fistula. Kshara Sutra is a minimally invasive surgical procedure which helps patients and Ayurvedic practitioners to manage anorectal diseases. It is a safe and cost-effective means to treat fistula, hemorrhoids, and other similar diseases. Indian sages such as Sushruta and Charaka have attested to the efficacy of such Ayurvedic interventions. In modern times, the efficacy of the procedure has been re-evaluated by the Department of Shalya Tantra in the Benaras Hindu University. The technique is widely practised in India and some other countries.

Procedure
Patients undergoing Kshara Sutra are anesthetized with either local or general anesthesia. Then a malleable probe is passed through the external opening of the fistula to the internal aperture in the anal canal. The probe is taken out gently through the anal opening along with the Kshara Sutra. Later, both ends of the sutra are tied together. The process is repeated once every week and gradually helps in healing the infected tract.

Benefits
The treatment helps to cut, drain, and heal the fistulous track which causes the discomfort. It also destroys and helps in the removal of unhealthy tissues, thereby facilitating fast healing of the fistula tract. The Kshara Sutra therapy also controls infection through its microbicidal action. It separates the debris and completely cleans the wound.

Also, this simple and safe para-surgical process is ambulatory, and patient need not stay overnight in the hospital. They may freely pursue their day to day activities immediately after the process is administered. The treatment does not trigger any complications, such as incontinence or stenosis, or strictures. This makes it suitable for patients of all ages.

The procedure being natural, and promises prevention and cure, is recommended to patients that suffer from fistula. However, patients must be advised to seek consultations from trained medical practitioners.

Can Fistula Be Treated Without Surgery?

Anal fistulas or fistula-in-ano generally cause discomfort due pus or watery discharges and sometimes pain. This is a condition where a nodular swelling appears in area surrounding the anal opening. Intermittent pus discharged is noticed from this nodular swelling. Instead of a nodular swelling; there might be simply an opening in this region. An anal abscess is one of the most common causes for this condition. Here are seven things you should know about these anal fistulas:

  1. It is curable through Ayurveda only: Till date many treatment options are tried to cure Anal Fistula which include Anal Fistula Surgery/ Glu technique/ cutting seton/ Colostomy/ Fistula plug/ VAAFT and many more. But all of these treatment methods have pretty high recurrence rate of about 20-25 %. However with Ksharsutra treatment for Anal Fistula; chances of recurrence are negligible. Other complications generally seen in surgery like bleeding, incontinence (inability to hold stool); are not seen with Ksharsutra Treatment. Clinical trials conducted by Indian Council for Medical Research (ICMR); at AIIMS New Delhi and PGI Chandigarh in 1992 have proved that Ksharsutra treatment is more convenient and more effective than Surgery in the patients of Anal Fistula. 
  2. There are symptoms you can look out for: Some of the symptoms that indicate anal fistulas include appearance of a nodular swelling in area surrounding Anal opening. Intermittent pus discharge is also observed from this swelling. Usually pain subsides and opening closes after the discharges; and patient feels relief. But after certain interval of time again pus accumulates; swelling re-appears with some amount of pain which is again relieved after the discharge. This cycle is repeated at varying interval in the patient of anal fistula.
  3. Medication can only provide temporary relief: Anti biotics and painkiller might give temporary relief from the symptoms. But after some time there will be again pus formation and associated swelling & pain in area surrounding anal opening. 
  4. Anal fistulas can recur after surgical treatment: As discussed above all the treatment options tried by modern medicine (including most recent treatment VAAFT) have high incidence of recurrence (about 20-25%) Whereas there are negligible chances of recurrence after Ksharsutra Treatment for Anal Fistula. . 
  5. Anal fistulas treated by Graded Ksharsutra do not recur: Ayurvedic treatment for anal fistulas is known as Kshar sutra therapy. This involves the use of medicated thread being ligated in Fistula Tract. The medicines present in this thread act as a strong debriding agent and induces healing of Fistula tract. In few weekly sittings of Ksharsutra change; Fistula gets healed completely and it do not recur. Pt. maintains his normal routine life during treatment period. 
  6. Sitz bath can be beneficial: This involves sitting in a tub filled with warm water so that the affected site is submerged. An antiseptic and astringent agent like Triphala decoction may be added to the water if needed. A sitz bath soothes the skin and provides relief from pain. This also helps keep the area clean and thus reduces chances of infection
  7. Oily and spicy foods should be avoided: When dealing with anal fistulas, you should try and take a diet that keeps your stool soft and regular. Hence use of oily and spicy foods should be reduced. Eat plenty of fiber and drink lots of water. Alcohol should also be avoided. 

Comparative Study: 

Ksharsutra vs Surgery in Anal Fistula Patients- 

  1. Ksharsutra is done under Local Anesthesia. Hence complications of General Anesthesia are avoided. Surgical procedures are done under GA with all impending complications of General Anesthesia. 
  2. Ksharsutra being a parasurgical procedure hence no bleeding/ cutting of tissue is involved. Surgery involves cutting/ bleeding. 
  3. No hospitalization or bed rest is required in Ksharsutra Treatment. In Surgery hospitalization is must. 
  4. In Ksharsutra; patient can continue his normal daily routine Surgery may require bed rest. 
  5. Complications like incontinence (inability to hold stool) and recurrence are seen after surgery
  6. Ksharsutra is a safe procedure and these complications are not seen. 
  7. Ksharsutra Treatment is more economical as compared to VAAFT surgery. VAAFT Surgery is comparatively too expensive.

Treatment Of Fistula-In-Ano With Ksharsutra!

Fistula-in-Ano is a type of an abnormal hollow cavity that is lined with granulation tissue. It connects a primary opening inside the anal canal to the second opening in the perianal skin. There are certain conventional surgical methods like Fistulotomy and Fistulectomy, which have been used but they involve certain complications like delayed healing, narrowing down of the blood vessels or other tubular organs. Modern surgical techniques also come up with similar kinds of issues. Thus modern surgery cannot provide satisfactory modes of treatment for this problem.
Kshar Sutra is basically an ayurvedic parasurgical treatment that involves management of Anorectal disorders for treating Fistula-in-Ano. The technique is minimally invasive in nature.

Kshara Sutra In Fistula - Why It Is Better Than Modern Surgery?

Anal fistula is an anorectal disorder for which different treatment methods have been advocated from time to time. Certain conventional surgical methods like Fistulotomy and Fistulectomy have been used but they are associated with complications like delayed healing, the risk of stenosis (abnormal narrowing of the blood vessels or other tubular organs) etc.

Modern techniques of surgery like the ‘lay-open' method and ‘anorectal pathology' come with a similar number of intricacies like high rates of recurrence and anal incontinence, prolonged hospitalization etc. Therefore, even today this particular disease has no satisfactory method of treatments in modern surgery.

However, in Ayurveda, about 2000 years ago, the great Indian Surgeon Acharya Sushrata first narrated about the ‘Kshara', which was later, compiled into the work ‘Sushrat Samhita'. Kshara Sutra is a minimal invasive ayurvedic parasurgical technique, effective for the treatment of anorectal disorders like hemorrhoids, fistula-in-ano, Anal fissure and pilonidal sinus.

Why is it used to treat Fistula-In-Ano?
In this technique of surgery, a thread also called a Kshar Sutra thread is specially prepared which acts by herbal chemical cauterization (the medical practice of burning a part of the body to remove or close off a part) and cures diseases. Hence, this is the safest and cost-effective method to treat Fistula-In-Ano.

Procedure-
The Kshar Sutra is a surgical linen thread of size 20 is coated with ayurvedic herbs with no side effects. There are three types of Kshar Sutra based on the number of coatings and the quality of materials used. Each patient gets customized treatment as per his or her own requirements.

First, a malleable probe is made to pass through the external opening of the fistula to the internal aperture in the anal canal. It is then taken out gently through the anal opening along with the Kshar. Later both the ends are tied together.

I Have a fistula problem, I went for kshar sutra treatment now it's cleared but I can feel my anal area wet, what should I do any treatment?

Some time, it happens that doc didn't tied ksharsutra inside the anal canal .cause they are unable to reach quiet inside the anus and thus they only cure fistula from outer area due to which patient may feel some wetness near anus.

Only ayurvedic kshara sutra therapy is a unique ancient technique, which is proved to be an effective treatment of anorectal disorders. It is commonly recommended in patients suffering from piles. According to world health organization (who, kshar sutra therapy is better than any modern surgery for piles & fistula. This is nonsurgical procedure so no need of hospitalization or bed rest, piles disease will never be again in life.

Piles - How Can Ayurveda Help?

Piles are Haemorrhoids which become inflamed and are accompanied with bleeding. Haemorrhoids can be defined as masses or clumps of tissues developing in the anal canal. Haemorrhoids comprise of blood vessels, support tissues, muscles and elastic fibers.

Bleeding piles can be treated using several methods or forms of surgery. The ayurvedic form of treatment called agni karma is an effective, natural cure for bleeding piles. On the other hand, a medical procedure known as infrared coagulation therapy can also be executed for the efficient treatment of bleeding piles or haemorrhoids.

Agni karma- an Ayurvedic Remedy for Treating Piles
In Sanskrit, the word 'Agni' refers to 'Fire'. Agni karma is an ayurvedic therapy, which involves the use of agni or fire. It is also called cauterization. Agni karma is popularly called Dahan Karma. Agni karma is used in the treatment of many diseases of different type. It is a good method for the treatment of bleeding piles.

In this process, any ailing surface of the body or a tissue, such as haemorrhoids, is destroyed and burnt by using a heated instrument using electric current and by freezing. An acidic agent for cutting like kshar karma is also used in the process of agni karma for the treatment of bleeding piles.

The different types of agni karma include:

  1. uchchh dagdha /scarce agni karma
  2. Durdaghda/ inappropriate burning surgery
  3. Aati dagdha /powerful or excess cauterization
  4. Samyak dagdha / appropriate agni karma

Infrared Coagulation Therapy-

Infrared coagulation therapy or infrared photocoagulation is a medical process which is used to treat bleeding piles or haemorrhoids, and can only cure internal haemorrhoids.

During the process, a device is utilized, which produces a beam of intense, powerful infrared light. The heat released by the infrared light causes scar tissue and blood supply to the haemorrhoids to be cut off. This results in the death of haemorrhoid or piles, and a scar forms on the anal canal wall. The surrounding veins are held together by the scar tissue which avoids them bulging into the anal canal.

Agni karma, the ayurvedic treatment, is considered to an effective medical treatment procedure of infrared coagulation therapy. This is because:

  • Agni karma is a much more natural method of treatment for bleeding piles, and no unnatural substance like infrared light beam is used.

Fistula का सबसे उत्तम इलाज़ केवल मात्र क्षारसूत्र से ही संभव है। इस पद्दति से इलाज़ करवाने पर recurrent होने की भी सम्भावना ना के बराबर है। एलोपेथिक सर्जन से fistula की सर्जरी करवाने के बाद इसके दोबारा से उभरने की सम्भावना भुत ज्यादा रहती है। कुछ क्षारसूत्र विशेषज्ञ क्षारसूत्र के साथ सर्जरी का सहारा लेते हैं जोकि उपयुक्त नहीं है। आप जहां से भी क्षारसूत्र विशेषज्ञ से इलाज़ करवाएं; पूर्ण रूप से विशुद्ध क्षारसूत्र पद्दति से ही इलाज़ करवाएं। आपको कोई भी परेशानी नहीं होने वाली। आपको अनावश्यक एलोपेथिक दवाएं खाने की भी जरूरत नहीं, कोई एंटीबॉयोटिक आदि की आवश्यकता नहीं पड़ेगी।
