Infertility is a global health issue in current world health dynamics, that has far-reaching and serious socioeconomic consequences. A declining trend in male fertility indicates an acute need of accessible as well as economical diagnosis and treatment. Conventional Assisted Reproductive Technologies are useful but the success rate has become constant over time.

Current Sperm Selection Techniques:

Conventional clinical sperm selection techniques, like density gradient isolation methods, physiological intracytoplasmic sperm injection (PICSI) method, and morphology-based selection methods, provides reliable solutions for isolating normal sperm from semen samples. Sperm morphology is an important predictor of fertilization. Morphology-based selection is generally employed. Unlike morphology-based selection, PICSI is a biologically inspired technique that mimics the natural attraction between mature sperm and ovum, depending mainly on sperm motility for selective fertilization.

In recent times, paper-based microfluidics have emerged as a low cost, simple, faster, and effective alternative to conventional techniques of sperm analysis, much similar to pregnancy testing kits at home for semen analysis. It is a major breakthrough and is expected to reach a wider population as it is easily accessible to all.

Here are some of the advantages of microfluidics for sperm selection method:

  1. Selection of quantitatively sufficient and qualitatively healthy sperm is an extremely important sample preparation process in the treatment of male infertility. Microfluidics help in recovery of motile sperm from semen, from highly heterogeneous mixtures, and reduce skill required for purification of sperm.

  2. Microfluidic technologies are emerging as low-cost and rapid diagnostic alternatives for male fertility testing.

  3. Microfluidics mimic in-vivo process and enable a selection of high-quality sperm, promising near-term advances in understanding male infertility and clinical execution.

  4. Paper-based microfluidics is also possible at home and are easily accessible to many users.

  5. Microfluidics can work with small volume samples leading to reduced costs, by minimizing the volume of expensive reagents. Thus, it is a cost- effective solution.

  6. Microfluidics can help improve the efficiency of sample preparation, enable consistent cells and embryo culturing, and reduce human error.

For the benefit of couples suffering from infertility, modern medical science has introduced several innovative procedures. Some of the popular procedures are In-vitro Fertilization (IVF), Intra Uterine Insemination (IUI), Gamete Intrafallopian Transfer (GIFT), Intracytoplasmic Sperm Injection (ICSI), donor eggs and embryos and so on. In addition to these, there are several drugs and surgical procedures that help the couple in getting rid of infertility. Among all these procedures, IUI has gained popularity in the field of gynaecology and infertility treatment procedures. IUI procedure in brief:

In fact, compared to the IVF procedure, IUI is cost-effective.
  2. In order to enhance the level of success, the gynaecologist may suggest IUI procedure every month. In some cases, the gynaecologist may also prescribe a few medicines to simulate the ovulation procedure. Except this, IUI may not involve extensive medication.

Intracytoplasmic Sperm Injection (ICSI) is an artificial procedure similar to IVF. This technique is mostly preferred in case of severe male infertility issues like an ejaculatory failure, low sperm count and Azoospermia. ICSI is a refined version of IVF where a single sperm is strengthened and injected into female ova (egg) to initiate conception. The diagnostic and screening process is similar to IVF. ICSI is recommended when:

  • The shape of the sperm is abnormal
  • The previous IVF cycle fails to produce any fertilized egg. In this scenario, ICSI technique may be used for the next IVF cycle
  • The sperm is not strong enough to reach the female egg through natural means
  • A male who had vasectomy previously needs sperm to be extracted to fertilize the female egg

ICSI-IVF Procedure:

  • Ovarian fertility drugs are given to a woman by an embryologist to produce healthy eggs for fertilization and follicles are examined by ultrasounds and blood tests until eggs get matured.
  • Eggs are retrieved from the female ovary with the help of a tiny needle that is attached to the ultrasound probe.
  • Fresh sperms are collected on the same day of fertilization and placed in a culture with eggs.
  • Each egg is then injected with sperm and left for fertilization.
  • The embryo thus formed, if found good enough, is transferred to the uterus through the catheter with ultrasound.

Advantages

  • As mentioned earlier, ICSI could help in producing a genetic child even when the male has fertility issues.
  • There is a greater chance of fertilization and embryo formation in ICSI compared to IVF.
  • The success rates of ICSI and IVF are similar and ICSI can be away when the cause of infertility cannot be determined.

Risks involved

  • The side effects in ICSI are similar to those associated with IVF.
  • Multiple pregnancies, OHS (Ovarian Hyperstimulation Syndrome) due to stimulating drugs administered for IVF and embryo growth failure.
  • Since ICSI requires precision while injecting sperm, there is a rare possibility of damage to the egg if not done carefully.
  • The male offspring produced through ICSI may have infertility issues passed genetically.
