I am 48 years & wife ages 33. We have a child after 3 years of marriage without any medicines. I have a problem relating to semen (low speed, I don't know exact word) for this taking medicine for 2 months earlier. Now both we are diabetic and we need a second baby. For this as per direction of doctor started folic acid tab, insulin. But 3 months already passed. No sign conception. Her last period starts on 28th of april. Present days started on 27th may. Let me know the possible date of ovulation and advice is it necessary to consult a doctor for infertility.

Try to trace her fertile days with lh kits which are available in chemist shop. Usually in regular monthly cycle it is wise to try alternate days from day 11 to day 18 of cycle. You should start medicines to improve sperm motility. Consult fertility specialist or urologist.

Clofert 25 will not help. Usual dose is 50-100. If you take it, there is need of doing tvs to see if you are responding to this medicine and also to see whether there is excessive response (ohss), which sometimes may become very serious. Therefore, please take it only after prescription pf the doctor and in the correct dose.

Dear Lybrate user, as I can see you are taking a higher dose of clomiphene. Such doses need to be monitored via serial ultrasounds. If you have already taken a 5 days course and can manage to go to your doctor once, go on day 12-13 of your cycle. If its literally impossible for you to do so, try conception with partner every alternate day from day 11 to 16. If you have not yet started the medicine, it's better to let this cycle go and start from next cycle when lockdown norms will be a bit relaxed.

For infertility treatment it's for ovulation. When mature follicle appears on ultrasound, at that time you can take by your consultant's advice.

Letrozole is commonly used for ovulation induction to improve the chance of pregnancy. However at this age the chance of success with letrozole is extremely limited. Considering your age, you have limited period of time for pregnancy. We cannot reduce the age. Therefore we advise to check blood for amh and check your husband's semen as soon as possible. At this age, probably you will need ivf.
