An in vitro fertilisation procedure carried out to counter the problem of male infertility is Intracytoplasmic sperm injection. Male infertility factors include poor quality of sperm, low sperm counts, poor sperm movement or the disability to penetrate an egg. Azoospermia is one such rare condition wherein there is no sperm in the semen. Through this injection, a single sperm can be injected into the egg directly. The fertilised egg is placed into the woman’s uterus or the fallopian tubes. The first successful birth using this procedure was on January 1992 after the conception took place in April 1991.

Collecting Sperm
Sperm can be collected from the male through masturbation. If this procedure somehow fails then sperm can be removed through a surgery. Using a small incision, sperm can be withdrawn from the testicle and is used in the procedure. If the ejaculation of sperm is prevented due to a blockage or if the problem arises in the development of the sperm, surgery method is used.

Injecting the sperm
This is performed through the following steps. A specialised pipette or a glass tube is used to hold the egg, a single sperm is picked with the help of a sharp and delicate needle, the needle is then inserted carefully into the cytoplasm of the egg through the egg shell, then the sperm gets injected into the cytoplasm and the eggs are checked to see whether they have been adequately fertilized.

After this process is done, eggs which are fertilised successfully are selected and placed into the uterus with the help of a catheter. The number of embryos an individual needs to transfer is determined depending on the age of the person and other health issues. For this purpose, a thorough full body check-up is carried out and the medical history is checked during the preliminary consultations.

Your doctor might recommend ICSI treatment for you if you have an extremely low sperm count resulting in infertility or other medical sperm conditions such as poor motility or poor morphology. It may also be suggested if prior attempts at IVF procedures have failed in fertilizing the egg or the fertilization rate is extremely low. You must also consider ICSI treatment if you resort to embryo testing or if the sperm needs to be collected from the epididymis or testicles surgically. 

However, you must keep in mind that if your partner has a genetic problem resulting in low sperm count, it could be passed on to your child, if you decide to use your husband's sperm for this process.

The success rate of ICSI treatment depends on your age and your fertility problem. Your eggs are healthier when you are young and as you age, chances are that they will become less healthy.

Intracytoplasmic Sperm Injection (ICSI) is an artificial procedure similar to IVF. This technique is mostly preferred in case of severe male infertility issues like ejaculatory failure, low sperm count and Azoospermia. ICSI is a refined version of IVF where a single sperm is strengthened and injected into female ova (egg) to initiate conception. The diagnostic and screening process is similar to IVF. ICSI is recommended when:

  • The shape of the sperm is abnormal
  • The previous IVF cycle fails to produce any fertilized egg. In this scenario, ICSI technique may be used for the next IVF cycle
  • The sperm is not strong enough to reach the female egg through natural means
  • A male who had vasectomy previously needs sperm to be extracted to fertilize the female egg

ICSI-IVF Procedure:

  • Ovarian fertility drugs are given to a woman by an embryologist to produce healthy eggs for fertilization and follicles are examined by ultrasounds and blood tests until eggs get matured.
  • Eggs are retrieved from the female ovary with the help of a tiny needle that is attached to the ultrasound probe.
  • Fresh sperms are collected on the same day of fertilization and placed in a culture with eggs.
  • Each egg is then injected with sperm and left for fertilization.
  • The embryo thus formed, if found good enough, is transferred to the uterus through the catheter with ultrasound.

Advantages

  • As mentioned earlier, ICSI could help in producing a genetic child even when the male has fertility issues.
  • There is a greater chance of fertilization and embryo formation in ICSI compared to IVF.
  • The success rates of ICSI and IVF are similar and ICSI can be away when the cause of infertility cannot be determined.

Risks involved

  • The side effects in ICSI are similar to those associated with IVF.
  • Multiple pregnancies, OHS (Ovarian Hyperstimulation Syndrome) due to stimulating drugs administered for IVF and embryo growth failure.
  • Since ICSI requires precision while injecting sperm, there is a rare possibility of damage to the egg if not done carefully.
  • The male offspring produced through ICSI may have infertility issues passed genetically.

Some couples have already used ICSI successfully and rate it highly as it is convenient and the possibility of congenital disabilities is minimal. SET (Sequential Embryo Transfer)

Sequential embryo transfer-(SET) is an improved method of increasing pregnancy rates and implantation rates in infertile couples.  According to the  American Society for Reproductive Medicine (ASRM), the average  pregnancy rate is about 20% for patients undergoing in vitro fertilization (IVF) treatment.  Patients undergoing SET have a pregnancy rate of over 50%!     SET is the transfer of two groups of embryos in the same menstrual cycle of a woman undergoing IVF treatment for infertility. One group of embryos-the best looking and fastest growing embryos are transferred to the uterus 2 or 3 days after the oocytes (human eggs) are collected from the ovary.  The second, more highly developed, group of embryos are transferred to the uterus a few days later-when at least one embryo has reached the expanded blastocyst stage-an advanced stage when an embryo is about to hatch from its shell called the Zona Pellucida  and attempts to implant in the lining of the uterus called the endometrium.  SET has an increased implantation rate ( embryos implanting from the total number of embryos).

Not all patients have embryos that survive to the blastocyst stage. Embryos may have arrested development in the first few days after fertilization – some embryos just arrest-no matter what the laboratory conditions are- and some embryos just were not destined to become babies because of inherent problems-such as genetic defects. The window of implantation of  human embryos is between 7 to 10 days after ovulation.  After 10 days, if the embryo does not at least start to implant, the endometrial lining may start to fall apart and not be able to sustain the embryo. Therefore, any early stage embryos placed in the uterus on Day 2 or  3 must depend on the uterus for a few more days until they develop to the blastocyst stage. Only the blastocyst stage embryo can implant in the uterus.

Embryos talk to the uterus!

SET may be better than just one transfer for many patients for many reasons: First,  embryos communicate with the uterus using hormones called cytokines.  The presence of these cytokines can help the uterus prepare for implantation.  The exact makeup of fluids in the oviduct and uterus of each patient may be impossible to determine.  Perhaps the embryo  can tell the uterus what it needs to implant and develop. The presence of one group of embryos on Day 3 may help the implantation of the second group of embryos by getting the uterus properly prepared.  SET also provides a better method of selecting the best of the extra embryos to transfer and decreases the need to cryopreserve (freeze) extra embryos that may not be survivors. Obviously, the ability to decide which embryos are the best increases the chance of implantation and pregnancy.  Blastocysts not transferred fresh can still be cryopreserved and thawed later for a frozen embryo transfer (FET).
