Like good health and youth, most of us take our locks for granted, until they're gone. For many people, a hair transplant can help bring back what looks like a full - or at least a fuller - head of hair.

A hair transplant is a procedure in which a dermatological surgeon moves hair to a bald area of the head. The surgeon usually moves hair from the back or side of the head to the front or top of the head. Hair transplants typically occur in a medical office under local anesthesia.

Receiving a hair transplant can improve your appearance and self-confidence.

Good candidates for a hair transplant are:

  1. men with male-pattern baldness
  2. women with thinning hair
  3. anyone who has lost some hair from a burn or scalp injury

Hair replacement isn't a good option for:

  1. women with a wide-spread pattern of hair loss throughout the scalp
  2. people who don't have enough 'donor' hair sites from which to remove hair for transplant
  3. people who form keloid scars (thick, fibrous scars) after injury or surgery
  4. people whose hair loss is due to medication like chemotherapy

PROCEDURE-

  1. Planning and Assessment before the Operation.
  2. Harvesting Methods: The patient is sedated using mild sedation and local anaesthesia after which the transplantation is performed. Common hair transplant methods to choose from are:
    • Follicular unit transplant (FUT)
    • Follicular unit extraction (FUE)

Care Post Operation:
After the surgery, your scalp may be very tender. You may need to take pain medications for several days. You will be prescribed an antibiotic or an anti-inflammatory drug to take for several days. Most people are able to return to work 2 to 5 days after the operation. The treated area should be protected from the sun.

Everyone has heard of or even seen hair transplant horror stories, maybe in a magazine, online or even worse seen the obvious 'plugs' in an elderly friend or relative. However hair transplant surgery has jumped leaps and bound in recent years to make it a viable, safe and not too risky option for those suffering from male pattern baldness, many celebrities such as Nicholas Cage, Brendan Fraser Mather MOcanhey, Dwayne Johnson ('the Rock', WWE) Salman Khan (Bollywood) and even Italian Prime Minister Silvio Berlusconi are believed to have had hair transplantation surgery, although such is the stigma attached that none of them has admitted to it publicly, it only becomes obvious when studying before and after pictures.

History of Hair Transplant

The origins of hair transplant surgery stem from Japanese dermatologist, Dr Okuda, who in 1939 published a revolutionary method in a Japanese medical journal of using small grafts that were similar to the way hair transplantation is performed today. This method involved using hair transplant grafts to correct lost hair from various body areas, including the scalp, eyebrow, and moustache areas.

In the late 50's one physician, in particular, Dr Norman Orentreich began to experiment with the idea of relocating or transplanting the hair on the back and sides of the head to the balding areas. Dr Orentreich's experiments showed that when bald resistant hairs from the back and sides of the head were relocated, they maintained their bald resistant genetic characteristic regardless of where they were transplanted.

This principle is known as "Donor Dominance", which highlights that hair could be transplanted from the bald resistant donor areas to the balding areas and continue to grow for a life time. This laid the foundation for modern hair transplantation. During the '60s and '70s hair transplants grew in popularity. However, the standard of care involved the use of larger grafts that were removed by round punches and often contained many hairs.

In the '80s hair restoration surgery evolved dramatically, as the large punch grafts were gradually replaced with a more refined combination of mini and micro-grafts. This "combination mini micro-grafting" hair transplantation procedure no longer used the punch to extract the bald resistant grafts. Rather a strip of bald resistant hair was surgically removed from the back of the head and then trimmed into mini and micro-grafts.

Types of Hair Transplant

There are two main types of hair transplant surgery. These mainly include-

1. Follicular Unit Transplant, which is commonly known as FUT or 'strip surgery'

2.  Follicular unit extraction, commonly known as FUE.

The main difference is the method of extracting the transplanted hair. FUT and FUE are really complimentary forms of HT, not competing methods of which one must make a choice.

FUT versus FUE

The '90s saw the gradual introduction of a very refined surgical procedure now known as "follicular unit hair transplantation" or "FUT". This exacting and labour intensive procedure transplants hairs in their naturally occurring one, two, three, and four hair "follicular unit groupings" in which they grow naturally.

The concept of creating the entire hair restoration using exclusively follicular units was proposed by Dr Robert Bernstein and was described in the 1995 Bernstein and Rassman publication "Follicular Transplantation". Critical to the success of the follicular unit hair transplant procedure was the introduction of the binocular microscope by Dr Bobby Limmer of San Antonio Texas in the late 1980s.

Dr Limmer found that by using the microscope to examine the donor tissue he and his staff were able to successfully isolate and trim the naturally occurring follicular units into individual grafts. Dr Limmer shared his techniques and findings with his colleagues and together with Drs. Bernstein, Rassman and Seager, was a persuasive advocate for the follicular unit hair.

The process involving follicular hair transplants is considered to be the most effective among hair restoration methods. In a follicular hair transplant, the surgeon transplants hair from the permanent zone in the back of the scalp onto the affected areas.

If you need a large area covered, then you most likely want to go for FUT because it is the more economical in terms of the number of grafts for the price paid. If you absolutely, positively don't want strip surgery as you are worried about scarring, then FUE is your only alternative. Contrary to popular belief, both methods leave scars. FUT will leave a narrow line across the back of your head, whereas FUE will leave little unpigmented dots across the back of your head. The only difference is that the FUE scars are not concentrated together and therefore harder to detect when wearing hair very short.

There is no doubt that FUE procedures are harsher on the grafts than FUT procedures. Because of this, the final growth yields tend to be lower for FUE compared to FUT. But BOTH procedures work for the most part.

Hair Transplant Cost

FUT surgeries are performed by reputable surgeons around the world however FUE is mostly available through a number of specialists, the vast majority of reputable ones that I know of are in the United States and Canada. Pricing for FUE from these reputable surgeons is currently close to $7- $11 US dollars per graft for FUE, and depending on what level on the Norwood Scale, a scale used to measure the progression of male pattern baldness, can be quite costly. Pricing for FUT from these same surgeons is normally in the range of $4-6 per graft, however, in Asia and Eastern Europe prices can be even lower.

Does cheaper means better?

Many of my clients do sometimes look to cut costs by using other surgeons especially in countries such as India, Pakistan, Thailand and Eastern Europe. Although I'm sure there have been success stories via this route please remember in these countries there is little guarantee or action available to you if something does go wrong. I have seen hair transplant using the FUT method, although not a horror story in terms of scarring or suffering, it did not yield the expected results of a more established surgeon placing the same number of grafts (2,000). He is therefore left with poor hair coverage but now has lost the option to 'shave it all off' due to huge smiley-faced scar in the back of his head!

Check, Check and Check

It is recommended that you should consider the hair transplant surgery option only after having tried the non-surgical treatments on the market. Then when you certain you wish to get a hair transplant I would advice thoroughly researching reputable doctors, of course, price is always an issue, but in the case of hair transplants, it should not be the most important factor. Indeed the money saved is nothing compared to the trauma of having to live with a badly scarred scalp for the rest of your life. If you decide to go for more budget surgeon, please do insist on seeing photos of the surgeon’s previous work and do not be afraid to ask for contact details of their previous patients.

What is Hair transplantation?

Hair transplantation is a method of Hair restoration surgery used to treat baldness and hair loss. Follicular unit transplantation or FUT is the latest method of Hair restoration surgery whereby follicular units are transplanted from the donor area (i.e. area where there is abundant hair usually in the back of the head) to the recipient area( balding area usually front or top of scalp). The follicular units occur naturally in groups on the scalp and contain 1,2,  3or 4 hairs along with oil glands, nerve and blood vessel bundle and are enveloped by a sheath of tissue.

How many types of hair restoration surgery are available?

Follicular Unit Transplantation or FUT is of 2 types: Strip method and FUE ( follicular unit extraction). They both differ in terms of how the follicular units are harvested. In the strip method of harvesting, the strip bearing the follicular units is removed from the back of the head and this is dissected into individual units under magnification. After the strip is removed, the edges of the skin are sutured. This leaves a scar in the back of the head running from one ear to the other. In the FUE method, the units are harvested directly from the back of the head using a tool that makes punches. There is no scar with this method and the healing time of the wound is quicker.

What factors does the result depend on?

There are several factors that influence who will have a better outcome from the transplantation. Not only the method of surgery chosen but factors like hair density, waviness of the hair, colour contrast between the hair and scalp, thickness of the hair, size of the donor area, elasticity of the scalp are all important. Besides this different practitioners have different results, so the skill of the surgeon is also a factor.

What does the surgery involve?

A. In the Strip harvesting method, after the strip is removed, it is sent for dissection to the team of technicians. Meanwhile, the skin is closed with stitches in layers. The surgeon then sits the patient upright and then designs the hairline in the front and on top of the head with a marker pen. After numbing the skin with local anesthetic injections he then makes tiny holes in the areas of balding. The team then starts inserting the follicles or grafts into the prepared holes in the recipient area.

In the FUE method, units are removed directly from the back of the head and are trimmed by the technicians. The rest of the steps are similar to that of the strip method.

How many Transplant sessions will I need?

This will depend on the amount of hair loss. Unless it is a small area of balding, usually there will be 2-3 sessions needed.

Will I have a scar?

With the strip method, there will be a scar on the back of the head but with improved surgical methods used nowadays, it will be a fine scar. Whilst closing the wound, a Trichophytic type of closure is employed which allows hair to grow through the scar that will camouflage the scar. In the FUE method, there is no visible line like scar. There is tina dot like scars from the areas where the punches were taken but this is hardly visible.

What precautions should I take prior to surgery?

  • Make sure you have all your questions and doubts clarified in your consultation with your doctor. Write down on a piece of paper.
  • Get good sleep the night before the surgery
  • Get a cap with an adjustable clasp with you on the day of the surgery
  • Arrive on time. Hair transplantation surgery takes plenty of hours, you don’t want to delay the whole team working for you into the late hours of the evening.
  • Wash your hair on the morning of the day of the surgery.

What happens after surgery?

  • You will be given instruction, painkillers and antibiotics by your doctor.
  • Get your hair washed at your doctor’s if you live locally
  • Redness in the donor and recipient area is common and will take time to resolve.
  • Avoid exposing your scalp to the sun too much.
  • Scabs will form and will remain for about 10 days after which they will begin to fall off.
  • Sutures are removed 10 days after the surgery.
  • Over the next few weeks, the hairs will shed. This means that the grafts are in resting phase and will remain so for about 3 months.
  • After this period over the next few months, the hair will start to grow from the grafts.
  • Take pictures regularly to note the developments.

The needle is targeted at the base of the hair follicles. At this spot, a circular incision is made. Another instrument, similar to tweezers, is used to dislodge the hair follicle from the scalp.

Post- Surgery Management:

Stretches are not needed in Follicular Unit Extraction. Additionally, there is almost no post procedural discomfort. Since the occurrence of any scar is absent, healing time is generally less. However, puncture marks which have a diameter of 1 mm or less are common. These marks heal by themselves within 3 to 7 days. It takes three to six months for the hair to look natural once again. The initial scars heal quickly and daily activities can be resumed the very next day.

Advantages:

Perspective of the surgeon:

  1. Lesser manpower, when compared to FUT, is needed i.e. a single doctor with a maximum of two assistants can run a clinic.
  2. Lesser equipment is needed.
  3. Graft preparation is minimal.

  2. Less post-operative recovery time.

• Carl Zeiss Loupes - The speciality of this type of devices is the ease of working they provide. What Can You Expect With Hair Transplant Procedure?

Look around you and you will certainly notice some of your friends already sporting bald patches or thinning hair even before they reach an age of 40. Until few years back, balding was seen as a sign of age or hereditary baldness but today, there are a number of factors such as stress, environmental pollution or hormonal changes that trigger hair loss or baldness.

For all those suffering from this problem, the good news is that science has found a way to cover these bald patches and thicken thin hair through a surgical procedure known as hair transplant surgery.

Hair transplant surgery is of two types; Follicular Unit Strip Surgery (FUSS or FUT) and Follicular Unit Extraction (FUE). The former involves removing a strip of the scalp from the back of the head and dividing it into thousands of grafts each with a few hair follicles. On the other hand, follicular unit extraction involves plucking the hair follicles out individually to be made into grafts. These grafts are then inserted into tiny incisions on the bald areas of scalp. Depending on the size of area to be covered, this procedure could take 4-8 hours.

Post surgery, the patient is prescribed some medication to relieve the pain and itching sensation. You should be able to return to work within a week. After 2 to 3 weeks of the process, the transplanted hair begins to fall out in order to make way from the new growth which may take up to few months. This is normal so do not stress yourself about this. Most people will notice 60% of new hair growth in 6 to 7 months. Full growth occur in one year. In some cases, patients may be prescribed a hair growing drug such as minoxidil to improve the quality and speed of hair growth.

A hair transplant surgery does not address the cause for hair fall and hence in some cases, a repeat procedure becomes necessary.
