Hello
the plaque that has hardened is calculus. Get scaling (cleaning) done by your dentist. The yellow discolouration caused by it would be gone, calculus and stains will be removed. Get scaling done whenever feasible.
Maintain oral hygiene
brush twice daily
do warm salt water rinses.

Hello, you can undergo scale if once to remove tartar. Brush your teeth twice everyday, avoid or reduce food which can stain the tooth like coffee,tea,tomato based sauce etc to prevent build up of tartar

Hello, tartar/ plaque develops throughout our life even after getting it cleaned once by scaling it can develop again and thus a professional round of scaling should be done every year, and yes deposits between the teeth can also be cleaned by the dentist. After scaling, brushing in morning and night and rinsing with mouthwash after the meals should be followed.

Tartar or calculus is nothing but the calcified deposit which needs to be cleaned professionally by a dentist. However for future visits I would say even a dentist gets his/her teeth cleaned once in a year because formation of mild to severe calculus will occur in every individual. So maintain your oral hygiene but do get scaling or cleaning done by a dentist once in a year.

Tartar and plaque deposits in your mouth r due to poor oral hygiene maintanence,reasons could be many.
There is no home remedy for it to be removed at this stage.
Go for scaling.
After that u can maintain oral hygiene by home remedies.
