Restless Leg Syndrome - How It Can Be Controlled?

MCh Neurosurgery, MS - General Surgery, MBBS

Restless Leg Syndrome - How It Can Be Controlled?

Restless leg syndrome is a condition where the nervous supply to the lower legs (and sometimes hands) is affected. The person has a constant urge to move the legs due to the tingly sensation or the pins and needles sensation and so this condition is called as restless legs syndrome (RLS). It is almost never a standalone condition and is often associated with other nervous system conditions.
It is a progressive condition and aggravates with time. The patient may experience episodes of this disease. Most people might experience this problem at night due to which a lot of patients complain of disturbed sleep and restlessness while sleeping.

While there is no permanent solution to this condition, there are various home remedies and techniques which can help control the condition. Read on to know more.

  1. Trigger agents: Some people are able to identify triggers like caffeine or chocolate which induce the condition. The first step would then be to avoid using these, especially before sleeping.
  2. Exercise: Any form of exercise, which gives a good workout for the leg muscles is recommended. This could range from a 30-minute brisk walk to stretches depending on the person’s overall health status. This is known to induce a good sleep, reduce muscle tension and relax the mood. Choose yoga, gym, cycling, or any form which suits your overall health and schedule.
  3. Massages:Hot or cold massages, often used alternately, can help in relaxing the muscles. This also helps in reducing tension and improving overall sleep. The ideal time for the massage is the evening so that RLS does not affect during nighttime.
  4. Diet: A diet rich in vitamins and minerals can prove to be helpful. This is because proper muscle and nerve function requires various minerals like zinc, potassium, calcium, sodium, etc. Diet rich in these helps in improving muscle and nerve function and thereby control RLS.
  5. Sleep pattern: Sound sleep practices like having an early dinner, a quiet room free of digital media, optimal temperature, and relaxing ambience helps in a better sleep. This has shown to reduce the incidence of RLS and evidence of sound sleep.
  6. Medications: RLS can also cause mild to moderate pain in the affected legs, and so pain killers can be used once in a while if the pain is not bearable. These should be used only rarely and continued use can lead to stomach ulcers and bleeding. The above mentioned nonmedical methods should be tried before trying medications. Anticonvulsants have been used to control RLS if pain killers alone are not adequate. In more severe cases, antiparkinson drugs are used with great success. If you wish to discuss about any specific problem, you can consult a Neurosurgeon.

SINGLE BEST EXERCISE FOR BAD BACK

EXERCISES FOR TREATING A BAD BACK

Once the stage is set with adequate rest and any necessary drug therapy, the following exercises reverse the structural and functional changes of the motion segment. Basic theoretical treatment regimens, using various combinations of the exercises for specific back disorders.

ROCKING THE KNEES TO THE CHEST

The least taxing and therefore least frightening exercise in the early loosening of a jammed segment is rocking the knees to the chest. It is performed in the horizontal supine position to eliminate compression of the spine. The primary function of the knee rocking exercise is to fan open the posterior compartments of the spine like flaring out a deck of cards.

The action stretches the muscles down the back of your spine when their tonic hold has pulled the inter spaces shut. Releasing the muscles un-jams the facet sand releases the pincer effect on the inter-vertebral disc. Thus the passive stretching inhibits the additional closing down effect across the inflamed vertebral segment. By providing ‘active’ decompression it provides the first glimmer of the spine lifting off the compressed segments.

It is a very efficient first base exercise.Like spinal rolling which you will read about below, rocking the knees to the chest is very effective if you have just jarred your spine or hurt it in some way. Rocking has an immediate neuro-physiological ‘switching-off’ effect which defuses the alarm and pre-empts the local muscles locking up the spine.

The to-and-fro rocking action familiarizes your back with movement again so it doesn’t have time to get stiff. It keeps your back moving in anon-threatening way and encourages the fundamental physiologic-unhindered blood flow and proper drainage- to resume.

In the acute phase the pull-and-release evacuates the inflammatory products from the site of injury. With more chronic conditions, the main benefit is the physical stretching of the tight structures.

The non-weight-bearing loosening of all the soft tissues of the posterior compartment, including the back wall of the disc, immediately gives the tight segment more freedom to move.Although rocking the knees to the chest is most effective for segment stiffness of L5 and L4, it is also effective higher up in the lumbar spine where there may be some rotation of the segment as well.Simple as it sounds, rocking the knees is often difficult to get started, let alone do well.

7 Exercises to Relieve Back Pain

7 Exercises to Relieve Back Pain

If you are suffering from back pain, you should know that the tiniest of movements can trigger the same. Back pain may occur due to several factors or reasons and it hampers your daily activities and standards of life to a great extent. However, you need to become active instead of being immobile for relieving back pain. There are several exercises which help in strengthening your back muscles and performing these on a regular basis will provide you relief from back pain drastically. Here are 7 common exercises which you should carry out:

1. Pelvic tilt: You have to lie on your back and keep your knees bend, but touching. Your feet should stay flat on the floor. By flattening your lower back against the wall and tilting your pelvis downwards, you must maintain the position for 20 to 40 seconds. Meanwhile, you must breathe slowly and deeply and then release. This stretch requires small movements for reducing tension, thereby easing your back pain.

2. Lumbar stretch: You need to sit on a chair, roll your head, chest, neck and lower back forward till your head gets between the knees. Hold the position and take deep breaths. Slowly, get back to the sitting position. Repeat this twice.

3. Cat position: You have to kneel on your fours and keep the knees and hip width separate. Keep the stomach muscles tense, arch your back like a cat does and maintain the position for five seconds. Repeat this and then allow your stomach to drop towards the floor direction and hold it for five seconds again.

4. Curl ups: For this exercise, you should lie on your back with the knees bent and your feet kept flat on the floor. Placing your hands behind the head will make the stomach muscles tense. Then, lift your shoulders, head and upper back from the floor. Do this around 10 times. This is ideal for strengthening your back.

5. Dry swimming: Begin by lying on your stomach, keeping a rolled-up towel under your belly for support. By tightening your buttocks, raise one arm and the opposite leg simultaneously. Switch your arms and legs and perform this for about two minutes.

6. Leg lift: For leg lifts, you should lie on your back with the knees kept bend and the feet kept flat on the floor. You have to press your lower back to the floor and straighten a leg with the knees aligned. After bending your leg, return to the starting position and repeat the exercise on the other side.

7. Cobra stretch: The cobra stretch is another ideal movement for stretching your tight abdominal muscles and lower back. You need to lie on your stomach with your legs extended and your palms planted on the sides of your head. If you wish to discuss about any specific problem, you can consult an orthopedist.

I am 28 years old and having pain in my backbone. What should i do? Please advise.
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

When I wake up early In the morning my feet pain very much. Please can you help me out.
This is because of plantar fascitis. Sometimes may also be because of overweight. You seem to be overweight. But that causes pain on prolonged standing. Do exercises for stretching tendo achilles, ankle and toe and calf stretching. This works wonders.

If, there is pain on touch, then plaster cast should be applied again for 2-3 wks. Gentle physiotherapy will help to regain mobility. Kindly do no massage.
