Dear Sir/Madam I would like ask you that after my physician change me the prescription From diabetic drug galvusmet 59/1000 to Glucosphage 850 mg with trajenta I feel symptom of heart rate disorder and high blood pressure he also give me depress drug like Amtriptiplin to make me feel not good. Just I want I ask you can I comeback to use galvusmet 50/1000 mg because I feel good with this medication but if I use it single it can't well control of diabetic which my HB1C around 7.2 so I would like to ask you that which is drug suitable use with Galvusmet? Such as Diamicron or other? Please advice me if I use another medicine with it.

Mr. lybrate-user, Tab Galvusmet (50/500) is definitely a good combination medicine. However, if adequate control is not achieved then there is most likely a need for some add on treatment. Since there is no mention about blood glucose levels, exercise, diet etc, there in no way to give any suggestion or give any comment. Plus in this communication we are supposed to give only general guidelines, no prescriptions. If you come in a private conversation with all above details I will definitely be able to give specific guidance including prescriptions. Thanks.

lybrate-user, Ideally FBG should be closer to 100 mg. FBG 140 mg is high, but can surely be controlled. Appropriate selection of drug combination plus diet and exercise is needed to achieve this. Since there is no mention about PP levels, HbA1c%, drugs being taken and dosage plus dietary pattern and extent of daily exercise it is not possible to give any suggestions. If you are looking for achieving better control please come back with all the above mentioned details, then I will be able to guide. Thanks.

Hello, If one has diabetes then it is preferable to avoid either sugar or sugar free tablets along with tea, coffe or milk. The sugar substtutes are not really safe. Their intake increases craving for sugar. They may not giving calories, but can be harmful. Thanks.

lybrate-user, During menstrual cycle the thick uterine lining breaks down and bleeding occurs. Chemicals to break clots are also secreted, but if flow is more, some clots do escape. That is quite common. It has no bearing on fertility. Thanks.

Mr. lybrate-user, With hypertension some times there can be pedal (lower limb) edema. Some times certain medications can also produce the swelling. Keeping BP 120 - 130 / 70 to 80 mmHg is good. Plus restricted salt intake and exercise will help in remaining free from it. Thanks.

Mr. lybrate-user, In the winter months skin becomes dry and there may be increased itching. There may not be any specific problem like infection or allergy. You may consider using moisturising creams or oils or white petroleum jelly applications, to maintain good deal of moisture in the skin. That should help. Fasting glucose should ideally be around 100 mg, for good control. PP should be 1j0 to 180 mg/100 ml. Thanks.

lybrate-user, Canyou clarify what you mean by" I decreased intake of sugar & salt" Because if you talk only added sugar or sweets that one eats contains GLUCOSE, then it is wrong. All the food that we consume like wheat flour, jowar flour, bajra flour, rice, potato, bananas, bread, pastries other bakery items etc everything get converted to GLUCOSE in the body after digestion. Secondly actual salt intake should not exceed 2 to 3 grams a day. But every food preparation made at home, processed marketed or packaged food contains a large quantity of Sodium Chloride (table salt). Therefore, one can hardly reduce the intake unless there is very strict diet control with severe calorie restriction. Plus by reducing salt intake blood pressure does not go down. By salt restriction rise in pressure can be reduced. Thanks.

lybrate-user, Fasting 189 mg is high. Ideally when in control blood glucose should be: FBG:closer to 100 mg, PP 150 to 160 mg & HbA1c% <6.8. Good blood glucose control depends on - appropriate drug & dose selection, pluse daily regular exercise, weight control and exact quantity of food intake. A doctor can only guide, but it is the patient who has to follow every single instruction meticulously. In your case it is necessary to first target the fasting glucose level and bring it closer to 100 mg, by adjusting night time insulin dosage. Then we will have to look at PP & pre lunch and pre dinner levels. Plus a close look has to be given to diet and extent of exercise. Thanks.

Mr. lybrate-user, With TSH value of 8 mU/L, you have sub-clinical hypothyroidism.(SCH). Usually SCH does not need any treatment. But weight gain is there and other symptoms also are present then treatment with thyroxin is advocated. As regards the second part of your question it is difficult to give a clear answer. Usually such kind of things do not happen simultaneously. Thanks.

Mr. lybrate-user Sunder, From your query nothing is clear except knowing that you have diabetes (type not mentioned), actual BP readings not given, nothing can be made out about the issue with your eyes. So please give following details: current fasting, PP glucose levels, HbA1c%, present treatment with dosage, dietary pattern, extent of daily exercise and preferably come in a private conversation, then I will be able to guide. lybrate-user, Your BMI is 29.54 kgs/M2, close to being grade 1 obesity. This shows hypothyroidism is not in control. Hair fall is also a major symptom of hypothyroidism. When on treatment TSH should be around 3 to 4 mU/L, then only all the symptoms are in check. So please check TSH values, if high, then dose of thyroxine may have to be increased 125 mcg and then TSH should be checked after 6 weeks to see if the desired level is achieved. With proper control efforts to control weight will be fruitful, plus hair fall also will be controlled. Thanks.

Mr. lybrate-user, If blood glucose levels go hay wire, then repeat infections tend o occur. Therefore, it is absolutely essential to maintain blood glucose levels in good control. Fasting should be closer to 100 mg, PP 160 to 180 mg & HbA1c% <7. If the control is not please se that it is regained with appropriate changes in the treatment. Thanks.

The reason for advising that is, banana contains high concentration of sugar, gets rapidly converted to glucose and raises blood glucose fairly rapidly. Eating a small banana once a while is ok. But regular consumption is not advised. Fruits like mango, banana, grapes and chikkoo as those are too sweet, hence better to be avoided. Other fruits like papaya, pomegranate, pear, guava, apple, water melon, orange, sweet lime and other melons are allowed. But only about 100 g a day should be taken with good control of blood glucose.

Mr. lybrate-user, Hypothyroidism produces following symptoms: weight gain, tiredness, early fatigue, slow heart rate, swelling of face and lower limbs, dry skin, dry hair and hair fall, joint pains, muscle pains, low sweat, irregular menstrual periods, excess flow, infertility, anxiety and depression etc. Treatment with adequate dose of thyroxine will usually makes patient quite normal. TSH should be maintained between 3 to 4 mU/L. Thanks.

Mr. lybrate-user, Unless hypothyroidism is well controlled with TSH level brought around 3 to 4 mU/L, all the symptoms will not be in check. So you will have to check TSH levels after 6 weeks and if still high, then dose of thyroxine will have to be increased. It I appears that starting dose given is too low. It should have been over 100 mcg. Thanks.

As a Type II Diabetic on Insulin with good diet control is it enough that blood sugar test be done ONLY for (1) Morning Fasting, and (2) Evening Fasting 6.30 to 7 pm pre-dinner (3) Thus omitting Breakfast and Lunch PP blood sugar tests?

lybrate-user, If you have really good & consistent blood glucose control then you may be able to manage with less number of tests a day. However, if glucose levels are bad then more tests are required to adjust the dose of Insulin. Since, no levels are mentioned and no mention is made of HbA1c% it is difficult to give exact advice. Mr. lybrate-user, It is difficult to answer about how many grams of sugar and carbs to be eaten. Basically if one is in a pre-diabetes state, then over all caloric intake has to be restricted to meet the exact requirement based on - Ideal body weight X 25 calories per kg / day. In your case IBW should be around 60 kgs. So 60 X 25 cal /Kg = 1500 calories, that means your total dietary intake should be 1500 cals per day. It best to avoid sugar or jaggery or sweets in total. Honey is also similar to sugar, there is hardly any difference. It is supposed to give some additional anti-oxidents. But only a small quantity should be used. Complex carbs like whole grain products are best. Do not use maida, pasta, bakery items, junk foods, sugary aerated drinks, pastries etc. Avoid deep fried foods and fructose corn syrup containing products and highly processed packaged products. Consume more of green leafy vegetables and salads and fruits. Daily exercise of at least 1 hour is necessary. Thanks.

It produces thyroxine hormone, essential for development & day to day functioning of brain, for all the vital metabolic activities in the body. Deficiency of the hormone causes hypothyroidism where patient experiences weight gain, tiredness, early fatigue, slow heart rate, dry skin. Dry hair and hair fall, irregular menstruation, infertility, joint pains, mood swings, depression etc. Hyperthyroidism causes loss of weight, anxiety, increased heart rate, palpitation, sleeplessness, etc. Getting a thyroid assay will help in knowing whether thyroid is functioning normally or not. lybrate-user, Both Glycomet 500 mg and 500 mg SR tablets contain same medication Metformin 500 mg. The difference is SR has a formulation which releases the drug over 6 to 8 hours. One of the advantage claimed is SR tends to be better tolerated. Some patients complain of gastrointestinal side effects, then SR is advised. As regards drug action both are same. Thanks.
