Hi, I have low back pain from past 1 year (not pain, somewhat uneasiness). There is tingling, numbness and burning sensation in the feet near edge of fingers, more in right leg, less in left leg. Sciatica pain also there. MRI report shows slight bulge in L4 & L5 disc. I have taken physiotherapy but it has not helped me. English & ayurveda medicine has not given any result. English Doctors say nothing big to worry. Gabapentin & Vitamin d3 medicine also not helping. But I feel somewhat uncomfortable with this pain. Kindly suggest what can be done on this regard.

Hello Mr. lybrate-user, as per your description of symptoms and your MRI outcome, it is obviously 100% confirmed that there is a nerve compression at your L4-L5 level, and you are getting pain and discomfort in the area supplied by that nerve root. Alright, now let me be very clear that in such type of cases no medicine will show good results, as logically, no medicine can pish that bulged disc back to its original position, it can only reduce some pain for as far as it is in blood circulation. Now, as you have mentioned that you have taken physiotherapy sessions as well, but here it is necessary to know what modalities were used, and what the protocol was. Anyways, what I suggest and I usually do to my patients with such a condition is, Intermittent lumbar traction, followed by most important thing i.e. "Manual mobilization of spine" Here I would like to mention that very few physiotherapists use this technique. But trust me it works in almost all such cases. Followed by TENS (nerve stimulation). Along with this, some back extension exercises (Mckenzie technique) helps a lot. With this protocol, I have found great results in 10-11 sessions as it treats the cause. So, my opinion is to find a physio who does spinal mobilization along with the mentioned modalities, and you should be fine within 10 days. Hope you get well soon. Take care.
