Your problem could be due to dental cavities per se or due to temporo- mandibular arthritis. Get a dental consultation first to find extent of cavities and whether they are causing pain. Temporo mandibular arthritis is arthritis of joint forming the jaw. This condition can cause severe facial pain aggravated with jaw movement like speaking or eating. This condition requires consultation with a pain specialist as it an be treated by a procedure in which drug is injected in the affected joint under X Ray guidance.
