For last six months I am not able to sleep properly. After 1 hour of going to bed, I get my sleep and it only last 4-5 hours. What should I do?

Sleep with all lights off. Have a glass of warm milk before going to sleep. Do not keep any electrical stuffs or mobile phones on your bed. Do meditation for 15 minutes daily. Avoid tea and coffee after evening.
