During initial months of pregnancy some women do lose weight due to nausea but they make up during later months. Regarding night duty, it is not a reason for her weight loss. There are women working in shifts for hospitality, airlines, hospitals, lady doctors & nurses etc and they do have babies, though barring a few jobs where pregnancy itself is a big no (airhostesses but not ground workers for airlines). Let her continue working as scheduled, get an ultrasound done & most probably the growth of baby will be seen as normal and this may give a big boost to the health of mother. No blood tests now as hb will be defective. Give her a tablet pregnidoxin twice a day on empty stomach with little water to dissolve the medicine, wait for half an hour, give ors, toned milk with protein supplements, no heavy fatty foods or spices. This medicine does control cough as it carries some anti allergic effect also. If cough continues, do write again for investigations. No chest x ray. Give her high protein diet like diluted milk, curds, chhachh & supplements of her liking.
