Sir one of my esrd patient showing positive result for anti-hcv test for last 3 months. A test hcv-rna quantitive was performed 3 months back when he was first diagnosed anti hcv positive but hcv-rna is negative. What are the further suggestions for the patient? should I start doing hemodialysis on positive machines or should I wait till hcv-rna is positive. How frequently I must repeat hcv-rna as it is very costly for the patient and not willing for such test. Hcv-rna quantitive or qualititive, which should be more confirmatory test for him. When the patient must start taking treatment for hepatitis-c.

Hi anti-hcv can be falsely positive. If hcv rna is negative then he is not infected with hepatitis c. So dialyse him on normal machines (non positive machines). You can repeat hcv rna qualitative after 6 months one more time and if that is also negative then no need to do again. Treatment has to be started if patient is hcv rna positive.
