I am 24 years old male and I want to know that my nipples get puffy whenever I wear something on like normally they stay puffy and whenever I take bath or touch them they become normal what tests should I take and who should I consult cause it looks real weird sometimes I don't know it's just chest fat or gynecomastia! I have reduced caloric intake and getting my chest in shape at gym but nipples are pointed outwards and are always puffy! What is the solution.

You seem to be having a condition called gynecomastia for which the only solution is male breast reduction surgery to be performed by a plastic surgeon. Medicines, diet and exercises do not work.
