You seem to be having anal fissure, which is a crack in the wall of the anus and it pains when stretched due to friction of passing hard stools. Constipation and hard stools are the main culprits. Use sitz bath (sitting in a deep bowl of lukewarm to hot water for about half an hour) daily which helps in the local healing. Green leafy vegetabes and lot of salads provide necessary fiber to give soft stools. Consult a surgeon if pain is excruciating and not responding to above measures.

Looks like a fissure in ano. Fissures may be caused by constipation, passing of large, hard stools, by prolonged diarrhea etc. It has been seen that fissures are associated with increased resting anal pressure due to spasm of internal sphincter muscle of anal canal. You need a surgical consultation for evaluation & treatment.
