My brother is suffering from schizophrenia and he had a relapse since a month now and he is still agitated and experiences hallucinations and delusions. He is on medicine olimelt 20 mg, etizola beta 0.5 mg and joy kem 150 mg at bed time and joykem 150 mg in the morning. I do not understand what to do. He is irritated and speaking of hallucinations and delusions. Tomorrow is eid and we are so depressed. Feel like dying. Please help someway.

Please take him for a quick review to his doctor, and if he is not on some therapy, introduce him to that immediately. Relying only on medication alone will not suffice for this condition. Because it is a psychotic condition he needs to be made to relate to reality often by confronting his distortion of his environment. He may be given to delusions, hallucinations and disordered thinking and speech as you have described. He may be emotionally unresponsive, suffering from social withdrawal, extreme apathy, and lack of initiative and drive. If he is allowed to go internal he can strongly relate to his distortions and in fact believe in them to be true. His cooperation with the rehabilitation and medical intervention is often not as desirable as it should be. In which case someone has to monitor him constantly and enforce some compliance. He must be kept functional to get him to be aware of his surroundings and build on a steady and trusting relationship with at least one member of the community. He will be difficult to handle, and he himself will not find it easy to comply with therapeutic expectations. With someone strong and firm he could be made to follow a tight schedule and do many activities and exercises to prevent him from succumbing to this illness. You are important for his and his recovery. If you are not there, what will be his state?! Besides, your life is very important too and must be preserved at all costs. There is hope if you blend medication with the therapy. This is the best combination because this is debilitating disease and everything useful has to be thrown into the ring.
