The patient here appears to have problems either with the lungs (asthma or copd which might have occurred as a complication of allergy or smoking or atmospheric pollutant's exposure) or the heart (coronary artery disease or valvular disease or cardiomyopathy) or maybe a combination of both worsening with chest infection of cough and cold. Diabetes should also be adequately controlled because it worsens the all these diseases.
