I am 18 years old. I am unable to sleep. I only sleep about 4-5 hours a day what should I do. I am in stress for exam.

Insomnia due to exam stress and anxiety is completely understandable. In fact unless you have a relaxing sleep, you cannot perform to the best of your abilities. A sleep of 7-10 hours is usually considered to be relaxing for the brain and if you are unable to get that due to stress and anxiety, you may start some medicines to relieve stress and get a relaxing sleep.
