I have hypothyroidism since jan 2019 and treated since then with thyronorm 125 mcg in march 2019 it was 2.41.doc added thyropace in june 2019 to take at night I had one miscarriage last year sep at 8 or 9 weeks not sure of reason. Tsh was 0.28 in aug 19 report continued same tablet till now. And tsh is normalised to 0.55 dec 2019 since trying for conception now I have conceived this month and did anc report results are with doctor due to lighten not able to collect those after looking at reports of anc profile doctor asked for decreasing the thyroid dose to 75 mcg and stop therapies which I used to take at night I never heard of decreasing such dose during pregnancy I am worried about that decreasing thyroid dose can anyone comment this is expected in first trimester.

Target of tsh in first trimester is 0.1 to 2.5 and your tsh is 0.55 so there is no need for reduction of dose continue thyronorm 125 as taking previously there is no need of thyropace its just a vitamin and iodine containing tablet repeat tsh because in pregnancy it should be done in between 4 to 6 weeks interval.

In pregnancy the recommended range of TSH for each trimester are as follows : 1st Trimester : 0.1 to 2.5 mU/L, 2nd Trimester 0.2 to 3.0 mU/L & 3rd Trimester 0.3 to 3.0 mU/L Since your TSH value is not mentioned (when pregnancy was detected), it is difficult to comment. If the value of TSH is with in the above mentioned range (1st Trimester) then there is no need to reduce the dosage. Usually during pregnancy Levothyroxine dosage goes up by almost 25% of the earlier dose. Please check what is the CURRENT TSH VALUE. That is important. Plus during these 9 months you will have to check TSH every 4 weeks till the time of delivery and please see that it is maintained preferably < 2 mU/L. If you have any questions please do come back.
