Hi Dr. .what would you prescribe for pilonidal sinus .went to a surgeon friend he just listened did not see the area and prescribed cefuroxime tabs fot 7 days .i have a boil like thing between the buttocks for quite sometime heals ans come up again.

don't go to friends for treatment. thorough examination and investigation is required for proper diagnosis. if it really is a pilonidal sinus then antibiotics will only give a temporary relief. cure requires some procedure, laser or surgery etc.

Need to be examined to know whether it is pilonidal sinus or farunculosis, as pilonidal sinus is rare at this age.

The location suggests pilonidal sinus but examination is required to confirm the same. Cefuroxime will help if it is infected.

Did your surgeon tell you that it is a pilonidal sinus? If it is, then that is the course of the treatment. Initial course of antibiotics, then after the infection subsides, we have to do imaging studies with a mri to understand the course of the sinus and if there is an underlying abscess. Once this is understood, we can plan for sinus excision with z plasty surgery to minimise future recurrence. About the boil, can be treated with betadine ointment, and add a painkiller to reduce the pain.
