Surgery or non surgery depends on symptoms and clinical findings. Injury appears recent with immediate MRI so bone bruising is thr. At this point of time you need to have conservative treatment -with knee brace and ice application with analgesics, then show your doctor after 2-3 wks for reassessment and decision .flattening of patellar surface may be old (Chronic) so assessment required.

Depending on the MRI alone can not give a valid opinion .Since it is three years since the injury clinically healing would have happened. What are your symptoms now? Why the MRI was taken MRI shows the old injury and the residual defects .If you are having any pain or instability now it could be due wasting of the thigh muscles during this last three years due to non usage .A good supervised exercise programme willl solve most of the problems.
