May be you are overloaded with work or information, stressed or preoccupied with some problems. first take a break to relax yourself, take nutritious food to maintain your alertness and energy levels. Then try to organize the information based of the priority with relation to time and space, and create a sequence or reminders for how it should be done. try this and if it does not work consult a psychologist
