It is because of waste products collection in the blood which is not allowing oxygen to reach to the organs and omcells..

As I can understand, your patient is already on dialysis. Most of the times it is due to excessive wt. Gain between 2 dialysis. Other causes could be related to heart, severe anemia or uncontrolled bp etc. Hence if it is happening regularly and your patient is near his or her dry weight, then would require to undergo evaluation in form of echo for heart and hemogram to look for anemia or cardiac issues. Hence you should consult your nephrologist for further evaluation.
