Dear Doctor, I am planning to start a muscle gain program, which will have calorie surplus diet as well as intense weight training and would like to take Creatine Monohydrate 5gm before workout and 5gm after workout for a period of 2 months. Consumption of Creatine Monohydrate will be 5 days in a week only on workout days followed by 2 days rest, Please let me know if it is ok?

You can have but remain checking your your blood parameters for urea and creatinine every 4-5 months interval. And if possible keep a gap between the each courses.
