She is 85 yes old. She is being detected with acute pancreatitis and her serum lipase and serum amylase are on higher side. She is also having gall bladder stone. She is diabetic and had bypass surgery long back. She is having high blood pressure as well. Rt now she is in ICU. Her bp is controlled. Her sugar levels is 78. Got her ct scan done. There is inflammation in her pancreas. Doctor is keeping her on IV and antibiotics. But her high serum lipase and amylase are concerning. What should I do, shall I continue with the same treatment of IV fluids and antibiotics and wait or is there some other alternative treatment available.

Hi, the cause of acute pancreatitis is probably the gall stones, which slips down to common bile duct and block the pancreatic duct. It is recommneded to remove the gall bladder within 48 hours of admission, however, now its more than 48 hours, she should settle down with conservative line of management as been provided. Don't chase the amylase or lipase level, this will come down within next 2-3 days. Amylase or lipase level does not reflect the severity.

Higher generation antibiotic is the only option to control her symptoms! It will take at least 4 week to come on normal side of all the parameters of acute pancreatitis like serum lipase and serum amylase! After four week she can go for Laproscopy of gall bladder stone!
