Quitting smoking is the one major change that can help you. Which you have done. Apart from that continuing the blood thinners and leading a healthy life with a good diet and exercise should give you a near normal life. Ideally stopping the beer would help since it can lead to increased fat deposition. There is a risk of another stroke or a cardiac event occurring considering your past history but that can be reduced with a healthy lifestyle.
