My mother, 78 years old, has blood sugar level of 216. Her liver and kidney are beginning to show damage. Would you advice her to control it by diet and medicine or insulin injections.

It depends on the extent of liver and kidney damage. If its severe, insulin is the safest option as oral medicines may not be allowed. But if the problem is not major, she can be controlled by tablets. In either case, dietary changes are also required.
