I am a diabetic and have been taking metmorfin in the form of galvys met 50/500 regularly along with triglismisave. However the sugar levels does not seem to reduce, in spite of all other parameters of balanced diet and regular exercises being followed. Do we need to take them continuously or this mandates a change?

Dear, galvus met is a combination of vildagliptin and metformin. Your bmi is 24.5 which is just fine. A proper measure for knowing and managing your diabetes will be to ensure a hba1c below 6.5%. Regarding diet, we indians have high carbohydrate diet. You have to closely observe your plate and you can come up with a better diet plan for yourself. To simplify it, identify the items which are carb rich in your plate and try to compensate the same with protein and fibres, for eg cut down a roti and increase sabji, dal or salad. To have a better control you can do a monthly self glucose monitoring 3 times a day and keep a track of your food with it. Check fasting sugar, sugar one hour prior your lunch and 2 hrs post lunch, and note down the details of your breakfast and lunch items. Modify as per my earlier suggestions and within a week you will see your blood sugar level responding. Best of luck.

Heart attack first aid

A heart attack is a medical emergency. Call ambulance or your local emergency number if you think you or someone else is having a heart attack.

The average person waits 3 hours before seeking help for symptoms of a heart attack. Many heart attack patients die before they reach a hospital. The sooner the person gets to the emergency room, the better the chance of survival. Prompt medical treatment reduces the amount of heart damage.

This article discusses what to do if you think someone may be having a heart attack.

Causes
A heart attack occurs when the blood flow that carries oxygen to the heart is blocked. The heart muscle becomes starved for oxygen and begins to die.

Symptoms
Symptoms of a heart attack can vary from person to person. They may be mild or severe. Women, older adults, and people with diabetes are more likely to have subtle or unusual symptoms.

Symptoms in adults may include:

Changes in mental status, especially in older adults
Chest pain that feels like pressure, squeezing, or fullness. The pain is usually in the center of the chest. It may also be felt in the jaw, shoulder, arms, back, and stomach. It can last for more than a few minutes, or come and go.
Cold sweat
Light-headedness
Nausea (more common in women)
Numbness, aching, or tingling in the arm (usually the left arm, but the right arm may be affected alone, or along with the left)
Shortness of breath
Weakness or fatigue, especially in older adults and in women
First aid
If you think someone is having a heart attack:

Have the person sit down, rest, and try to keep calm.
Loosen any tight clothing.
Ask if the person takes any chest pain medication, such as nitroglycerin, for a known heart condition, and help them take it.
If the pain does not go away promptly with rest or within 3 minutes of taking nitroglycerin, call for emergency medical help.
If the person is unconscious and unresponsive, (or your local emergency number), then begin cpr.
If an infant or child is unconscious and unresponsive, perform 1 minute of cpr, then call ambulance
Do not
Do not leave the person alone except to call for help, if necessary.
Do not allow the person to deny the symptoms and convince you not to call for emergency help.
Do not wait to see if the symptoms go away.
Do not give the person anything by mouth unless a heart medication (such as nitroglycerin) has been prescribed.
When to contact a medical professional or your local emergency number immediately if the person:

Does not respond to you
Is not breathing
Has sudden chest pain or other symptoms of a heart attack
Prevention
Adults should take steps to control heart disease risk factors whenever possible.

If you smoke, quit. Smoking more than doubles the chance of developing heart disease.
Keep blood pressure, cholesterol, and diabetes in good control and follow your doctor's orders.
Lose weight if obese or overweight.
Get regular exercise to improve heart health. (talk to your doctor before starting any new fitness program.)
Eat a heart-healthy diet. Limit saturated fats, red meat, and sugars. Increase your intake of chicken, fish, fresh fruits and vegetables, and whole grains. Your health care provider can help you tailor a diet specific to your needs.
Limit the amount of alcohol you drink. One drink a day is associated with reducing the rate of heart attacks, but two or more drinks a day can damage the heart and cause other medical problems.

Sir, I went through your report on Vit B6 deficiency. Surprisingly, I have almost all the problems that is in the report like changes in mood - depression irritability and anxiety - confusion, muscle pains, chronic fatigue, I have also this tendency to procrastinate and has to bring myself hard to take bath and clean myself although I do it. Why is it so? I also suffered CAD in 2008 and has BP and DM-II for the past 13 years. Though my food intake is nutritious am I still lacking in vit B6?

Stress patients worry too much. No need to do so. Relax Nothing is going to happen to you. Take regular exercise daily Stop alcohol and smoke No need to worry. Take nexito 10 mg daily Yoga and breathing exercise All the best B6 deficiency can cause these but rare treat your anxiety.

Dear , getting upset easily means you are emotionally not stable. Emotions are supposed to be vent out. You could be stressed or due to tension you are getting emotional. Tension means in one way anxiety or stress. You are aged 26. There could be many reasons to get stressed or to be anxious about. Stress is part of everyday life. We need know the stress and learn to live with it. Fighting the stress causes anxiety. Anxiety and apprehension further causes negative and obsessive thinking. Any shampoo which will be effective on dandruff? Currently I am using Himalaya anti dandruff but still problem of dandruff persists.

Hi Shampooing your hair everyday will not reduce your dandruff and hairfall at all instead it will worsen this problem.So reduce washing your hair 2times a week.First before washing hair apply oil in the morning or previous night.Don't use anti-dandruff shampoos at all bcoz this will increase the amount of dandruff and hairfall .Don,t eat too spicy and oily food.Include salads in diet and fruits. revert back for treatment

Our ears has selfcleaning property. Only thing you have to do is after bathing clean ear with a fingertip inserted in cloth.

Please better start insulin please consult nearby physician and ask him to start insulin along with insulin, give him tab diabecon ds by himalaya 1 1 before food.

Hello Your Daily food intake should ideally consist of – a. Have plenty of nuts, dry fuits and seeds. Almonds, walnuts, sunflower seeds, pumpkin seeds, figs, dates and raisins. They are full of healthy fats needed by a human body. b. Good amount of fruits and vegetables – Bananas, apples, pomegranate, green leafy vegetables. Fruits and vegetables helps in absorption of food in the body and together, completes your daily nutrition need. c. If you are a non- vegetarian - Eat eggs, chicken etc. d. Eat whole grains – Try to have multi grains spread throughout the day. Wheat, brown rice, oats, bajra, daliya etc should be taken in multiple meals in a day. e. Eat Sprouts – Chana and moong which are full of protein and nutrients. f. You can supplement your diet with good quality multi-vitamin capsules. If you are lacking in protein, you can buy a high quality whey protein and have it with banana shake twice a day. g. Continue push - up. First start protein and calcium rich diet. Ex. Pulses, milk, eggs, chicken, fish. Take maximum green vegetables. Start yoga.

It is hard to tell just by the limited description given. I would recommend yourself to be seen by a hand specialist to rule out carpal tunnel syndrome (pressure on the nerve in the wrist), which can be treated initially with therapy but in some cases may require injections or surgical release.
