Right Time to Have a Knee Replacement Surgery

I am Dr Deepak Thakur, senior consultant of joint replacement surgery. My area of specialization includes joint replacements including computer navigations knee replacements.

When should we go for a knee replacement? Is there any particular age?

 Well, today’s modern time age is not the criteria for undergoing a knee replacement surgery. Your level of pain, as well as your level of disability, decides when you should go in for a knee replacement. Most of the patients, they are very fearful.

Will there be a lot of pain during the surgery?

Well, with modern pain relieving medications and procedures, nowadays the knee replacement surgery is as pain-free as possible. Even the recovery part, it is a multimodal pain relief which we employ including epidural analgesia’s, patient control analgesia’s, patches and IV medication. Generally, the patient is pain-free, even during the surgery as well as post the recovery. The recovery part starts from the day of the surgery. On the day of the surgery, we make you do some exercises. The next day we make you stand and the second to third, we make you walk with the help of a walker and gradually as the recovery progresses with the help of a stick. We may even allow you to climb and come down from the stairs, depending upon your level of recovery. We generally, advice avoiding contact sports. By contact sports, I mean playing football hockey etc after knee replacement surgery. But most of the activities like swimming, cycling and doing brisk walking you are able to do it very efficiently. You are able to bend your knees after the surgery. Your knee replacement surgery consists of a metal implant which is there implanted in the thigh bone. This is your thigh bone. As well as a metal and a plastic implant


which is implanted on your leg bone. So, this again is your new knee. Of course, you will be able to bend your knees like this, straighten up your knees like this. You will be able to do your normal activities like bending. Your level of bending depends upon how well your leg muscles touch your thigh muscles. This will be the amount of bending that you will be having, which is generally equal to your preoperative status. So yes you will be able to bend and straighten your knees after your knee replacement surgery. You will be able to sit cross legged, but generally, we advise you to not sit cross legged because when you get up it puts a lot of pressure on the knees. But you can sit easily cross legged on the bed because getting up from a bed does not require a lot of pressure on your knees. We employ a computer navigation which is highly accurate even in few degrees. The life and with modern day implants is on an average 15 to 20 years.

Identify the mid point between the ankle bone and tendons and give pressure with a blunt edge on either side of your foot for 2 minutes. Also give pressure in the web between 2nd and 3rd finger from your toe for 2 minutes. Finally give a good massage for both feet in hot water. Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Please swing 20 times morning and evenings. This will relieve your problem. Consult after 10 days.

Hello. Maintain a right standing and sitting posture, keep your back always straight, avoid carrying weight on back. Hold or carry weight divided equally in both the hands or shoulders (to avoid over straining on one particular hand or shoulder). Lie down on bed on your back without pillow. Exercise: Practice Shavasana or Corpse Pose as it looks like a dead body. If practiced correctly and with full efforts, can relax every body part, relaxing not only the body but also the mind: Spread the legs one to two feet apart, the toes are turned outwards, the heels facing each other, a comfortable distance apart. Bring the arms a little away from the body, palms turned upward. Relax the neck and allow it to turn to the side if it is more comfortable. Close the eyes and focus the attention on the body, breathing normally.

Hi, your t score in bmd is low. You need to take adequate calcium and vitamin d supplement to increase it. For now i'll suggest calciferol 60000 unit /week for 8-12 weeks. Though there are better drug such as biphosphonates but that to be taken after evaluation and consultation of specialist.

Postural correction- sit tall, walk tall. Extension exercises x 15 times x twice daily. Apply hot fomentation twice daily. Avoid bending in front.

Most leg pain results from wear and tear, overuse, or injuries in joints or bones or in muscles, ligaments, tendons or other soft tissues. Leg pain can also be caused by blood clots, varicose veins or poor circulation. To relieve mild pain: stay off your leg as much as possible apply an ice pack or bag of frozen peas to the affected area for 15 to 20 minutes three times a day elevate your leg whenever you sit or lie down please go to a physiotherapist if: you have pain during or after walking you have swelling in both legs your pain gets worse your symptoms don't improve after a few days of home treatment you have painful varicose veins if you feel the pain is from muscle cramp then you need to drink a lot of water because poor fluid intake (dehydration) may predispose to cramps also low blood levels of either calcium or magnesium directly cause cramps. Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. If your pain is only a localized one, and since 2 days, you can start with - ice packs at painful site to reduce muscle spasm - gentle anti inflammatory medication like tab etoshine, tab diclofenac etc which suits your body - a muscle relaxant can be added to provide relaxation to muscle spasm - analgesic spray like dynapar qps can be used for local application after fomentation 3-4 times in a day - rest for 4-5 days, use hard mattress, avoid lifting heavy weight and bending forwards acutely we will initiate your physical therapy programme once your pain subsides.
