Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Spiking of tibial spine with marginal osteophytes are indication of degenerative changes in your knee region. Treatment of osteoarthrosis depends upon what grade of degeneration has occurred, what is your knee range of movements and what medications like analgesics do you use on a daily basis? recommendations for early oa are: - start doing daily brisk walks for at least 2-3 km in a day in mornings. - intensive knee - quadriceps and hamstring strengthening exercises should be initiated initially in presence of a trained physical therapist and then at home on your own - avoid sitting cross legged, not to use indian style washrooms, do not do anything which eccentrically loads the knee and increases cartilage breakdown. - adequate calcium and vitamin d metabolism in terms of appropriate intake is important to keep bone metabolism ok - you can take short course of anti inflammatory medication like tab etoshine on sos basis especially when there is an episode of inflammatory pain, swelling etc. Do not hesitate to contact me if you need any further assistance.

Dear at this age shaking of hand may be due to anxiety or excessive alcohol use or thyroid disorder. Get tsh, t3, t4 done, stop alcohol if abusing. If you feel yourself worried always, can not relax, have dry mouth, palpitation, excessive sweating, churning sensation in stomach, dizziness etc then you may be suffering from anxiety. Treatment of anxiety with medicines may also cure this shaking of hands.

Pain In Fingers - Can It Be A Sign of Tendinitis?

Tendinitis is a discomfort of the tendon. It results in pain outside the joint. There are many tendons in the body, but this condition is mainly witnessed in wrists, heels, shoulders and elbows. Some frequently used names for different kinds of tendinitis includes pitcher’s shoulder, tennis elbow, jumper’s knee, golfer’s elbow and swimmer’s shoulder.

Symptoms of tendinitis:
Symptoms of tendinitis are witnessed at the point of contact between bone and the muscle. Some of the visible signs of tendinitis are tenderness, swelling and a dull ache while moving the affected joint. Even constant pain can be felt in some cases.

What are the causes of tendinitis?
Tendinitis is frequently observed among sportsmen and athletes. If a job involves repetitive motion of a particular joint, the stress on the tendons can result in tendinitis. It is therefore very important for sportsmen to use the right technique in order to refrain from tendon related problems. Sudden accidents can also result in an injury to the tendon.

Risk factor of tendinitis:
Risk factors for this condition include job, occupation, and sports. As a person gets older, tendons tend to lose their flexibility which can lead to an injury of the tendon. If the job of a person involves frequent reaching of the overhead, vibration, repetitive motion and forceful exertion, he might face an injury to the tendon resulting in tendinitis. Last but not the least, tendinitis is very common among players who are involved in sports such as basketball, tennis, golf, baseball, running , cricket and bowling.

What are the complications involved?
Tendinitis can aggravate to tendon rupture of proper treatment is not done on time. Tendon rupture needs a surgical procedure to fix. If tendon pain lasts for few weeks at stretch, there is a good chance that the condition has turned into tendinitis.

Diagnosis:
Diagnosis of tendinitis can be done with the physical exam itself. A doctor might want to look at an X-ray scan to negate other condition that might be causing the pain.

Treatment:
Treatment includes the prescription of pain relievers, such as ibuprofen and aspirin. Tropical creams can also be prescribed to relieve the pain. Doctors often suggest corticosteroids be injected along the tendon in order to relieve tendinitis. If the condition is not very serious, a doctor might suggest specific exercise such as eccentric strengthening to fix the condition.

If the condition is severe, the doctor might want to go for a surgical procedure to fix tendinitis. Especially in conditions where there is a tear in the tendon and the bone. Another invasive technique to treat tendinitis is the focused aspiration of scar tissue. This procedure is performed under anesthesia thereby causing less pain to the patient. This is Dysmenorrhea.It looks that you are living in a comfort zone and not doing any physical work. Ta k e cap.Evecare morning evening after meal. Do sweep flooring daily sitting on legs. Use indian style sheet in toilet.

Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Dear lybrate-user back pain is very vague term and we need more information to understand you. Simple solution is to keep fit with aerobic exercise and so yogasan and pranayam and later on weight training. Avoid anything which gives more pain like bending forward or lifting weights.

Dear, Osteoarthritis is age-related wear and tear of your joints and is expected with aging in almost everyone. Most of older gentlemen have weakness of the muscles around the knee -the quadriceps, hamstrings and calf muscles. That adds up to the strain on the knees and increases the pain. The treatment is multidirectional. Avoid climbing up and downstairs, low dose pain killers (as advised by your family practitioner) will reduce your acute pain. Do not squat for your toilet. Use the higher (western) commode. Start on a few sessions of physiotherapy to strengthen your muscles. Make a practice of regular walking. If pain is excessive make use of a walking stick support. After 7 years of long diagnosis doctors finally declared that my uncle is suffering from Amyloidosis. Initially he was wrongly diagnosed as Rheumatoid Arthritis patient by the doctors of CMC and Gangaram Hospital. Doctors prescribed him to take 24 nos of Bortezomib 2 mg injection. But after taking all the injections there is not any remarkable improvement. I want to know is there any treatment available in our country to cure it? Kindly help us. Thanks.

I must say that precious 7 years wasted. It had been better to use Ayurveda in such cases. Even if medicine continues through out life, the disease won't advance. But it's not fault of doctors. Allopathic has the limit in 'kapha ama vitiation'. Even now, even if u go by modern med (though no treatment for it in allopathic), I suggest you to give him parallel Ayurveda. There will be no interaction. If there's slightest improvement in 6 months, continue the medicine. It's not advisable to comment how much time would it take. Treatment will be in phases.

How To Treat Fractures?

Fractures are simply a break in a bone. They can be caused due to injury, (traumatic fractures) or a pre-existing condition like osteoporosis that causes weakening of bones (pathologic fractures). There are many ways to classify fractures. All fractures fall into the major categories of simple and compound fractures. Simple fractures are fractures where bones remain inside the skin and don’t jut out. They are also called closed fractures.

Compound fractures, also called open fractures, are broken bones that penetrate through the skin. These types expose the bone and deep tissues to the environment. Compound fractures are more serious of the two. The healing here may be affected due to deep infections for which antibiotics need to be used. There are many different sub types of fractures and we’re only going to skim through them here.

  1. Comminuted fractures: Severe fractures in which a bone breaks into several smaller pieces.
  2. Avulsion fractures: A small piece of bone is completely torn off from the main bone due to fierce pulling off a part of the body.

Other types of fractures are characterised by the many different angles the bone breaks into like transverse, oblique and spiral fractures.

Diagnosis
When a bone is broken there are symptoms like swelling that doesn’t subside on its own and pain. In such a case it’s imperative that one goes to a doctor for a diagnosis. Doctors can usually recognise most fractures by examining the injury and taking an X-ray. The X-ray also provides a clear idea about the type of fracture and the degree of displacement of the bone. And, it’s important that the patient doesn’t wait too long before approaching a doctor. This is because bones begin to heal very quickly after a fracture and the bone tissue will heal using any tissue available. This can lead to a misalignment of broken pieces of bone and cause disability and loss of function.

There are cases when X-ray may not show a fracture. This is especially common in fractures in the hip and wrist in older people. For diagnosing these, doctors will get some other tests done such as a computed tomography (CT) scan, magnetic resonance imaging (MRI), or a bone scan.

Treatments
Fractures have to be treated by doctors. The doctors set the fractured bones in their proper place and hold them there so that they can heal. Setting a bone is called "reduction." Reduction without surgery is called "closed reduction." But if the fracture is serious, it’s going to require surgery with bone repositioning, called open reduction.

In extreme cases, pins, plates, screws, rods, or glue are used to hold the fractured bones in place, inside the body. Once the bone abutment has been treated, the bone is immobilised to allow the broken pieces to heal. In most cases, the fractured part is set in a rigid cast. The fractured ends of the bone can be fixed into place using metal pins connected to an external frame. This is removed after the bone has healed. If you wish to discuss about any specific problem, you can consult an orthopedist.

Is it since birth? or developed later in life. It could be due to cervical spondilosys rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Any way take bio d3 max 1tab odx10days ibuprofen 200mg od & sos x 5days do neck & back (spine) exercises contact me again if need be. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take.

Dear, low back pain are often referred for physical therapy for four weeks as an initial conservative (nonsurgical) treatment option before considering other more aggressive treatments, including back surgery. Most probably you are suffering from oa of knee. Knee surgery-total knee replacement (tkr) is quite safe & gives uniformly good results. We have well equipped center in delhi. Do ask for detailed treatment plan. Surgery should be done when patient is still active & will be able to take benefit of surgery. Do not ignore. It could be beginning of a serious problem. Kindly show me x- rays before a final decision can be taken. You will be thoroughly investigated before surgery is under taken. Risk in surgery is less than the risk in crossing road.
