Multimodal Pain Relief After Total Knee Replacement

Knee replacement can be extremely painful. Previously, opioids or narcotics were administered for pain relief. But excessive addition of opioids is not exactly effective for controlling pain.

Multimodal pain management has become an important part of the perioperative care of patients undergoing total joint replacement. The principle of multimodal therapy is to use interventions that target several different steps of the pain pathway, allowing more effective pain control with fewer side effects. Many different protocols have shown clinical benefit. The goal of this review is to provide a concise overview of the principles and results of multimodal pain management regimens as a practical guide for the management of joint arthroplasty patients.

Multimodal denotes administering two or more than two types of medications that work with different mechanisms. The following are the techniques used:

  1. Pre-operative Femoral Nerve Block: Prior to the surgery, a catheter is placed beside the femoral nerve for blocking it. This nerve is located in the upper thigh. Medication is delivered through the catheter for the nerve to be numbed for 24 hours. Thus, pain signals to the brain are blocked. This method reduces the use of narcotics and the consequent side effects.

  2. Patient Controlled Analgesia (PCA): This method is also known as ‘Pain Pump’. An intravenous pump is used to administer pain relief medications, such as oxymorphone or morphine, after the surgery. The control button of the machine could be pressed, by the patient for 6 to 10 times per hour. The machine is used for two post-operative days.

  3. Oral Medications: The oral medications include Non-Steroidal Anti-Inflammatory drugs or NSAID; such as Celebrex which is similar to aspirin, structurally. Alternatively, acetaminophen, such as Tylenol or its equivalent composition, can also be used.

  4. Acetaminophen: It acts on the Central Prostaglandin Synthesis and relieves the patient of pain through multiple mechanisms.

  5. Epidural Analgesia: It produces lower pain scores and involves less time for achieving physical therapy goals. However, this is subject to side effects such as dizziness, urinary retention and itchiness.

  6. Gabapentinoids: These medications include membrane stabilizers such as Gabapentin and Pregabalin.

The objective of multimodal treatments is to provide quick relief to the patient and immediately so. Earlier the rehabilitation, more successful will be the knee replacement surgery.

Alcohol - How It Can Induce Sleep Disorders?

Sleep disorders are often associated with substance abuse. The problems in sleep arise as a result of direct effects of a particular substance. Substance induced sleep disorders can be caused by many drugs, regardless of whether or not the substance is medically prescribed, legal or socially acceptable. It can also be caused due to alcohol abuse.

If you experience problems in sleep on not having resorted to any substance or even if you are trying to quit, it is called ‘withdrawal’. The severity of the sleep disorder depends on the type of substance used, and not much on whether or not the drug is illegal. A number of prescription as well as non-prescription drugs are capable of causing sleep disorders. However, the severity of the sleep disorders tends to vary among individuals.

Problems in sleep that can be caused due to prescription drugs such as:

  1. Hormonal drugs e.g. oral contraceptives
  2. Drugs for high blood pressure e.g. beta blockers
  3. Inhalable respiratory drugs
  4. Steroids, such as prednisone
  5. Medications for attention deficit hyperactivity disorder
  6. Diet pills
  7. Seizure medications
  8. Some antidepressants

Non-prescription drugs that can cause problems in sleep:

  1. Medications along with caffeine: These may include many cold and cough medications.
  2. Pseudoephedrine
  3. Illegal drugs such as cocaine, amphetamines as well as methamphetamines.
  4. Nicotine is capable of disrupting sleep and reducing the total sleep time.

Alcohol is often regarded to be a calming drug or a sedative. While alcohol is capable of inducing sleep, the quality of sleep tends to be fragmented during the second half of the sleeping period. Consumption of alcohol usually tends to increase the number of times you wake during the latter part of the sleep. This is because the relaxing effect of alcohol wears off eventually. Alcohol tends to keep you in the lighter sleep stages more and thus, is at fault of not inducing the much required deep sleep.

If you consume alcohol just before you go to bed, the sleep inducing capacity might decrease as the effects that are disruptive continue to increase. The alcohol induced sleep disorders may lead to sleepiness and fatigue during the daytime. With age, these problems tend to grow in severity. Consumption of alcohol before bedtime, among the elderly, might lead to unsteadiness while walking with a higher risk of injuries and falls.

Go for a routine health check up and (also check blood sugar and BP). I don't think there is any serious problem but let the doctor decide it based on examination and some tests. On your part you need to become aware about the basics of health and not take life for granted. Start exercising every day (add yoga as it can be relaxing and empowers the nervous system ). Eat healthy food. Avoid all junk food and cold-drinks. Eat more of fruits and vegetables. Avoid bad habits. Sleep for 7-8 hrs everyday.

I am having pain in right shoulder my pharmacist gave me an injection of deca50 mg with diclofenac q. First of all you have to be assessed for any of the muscles or soft tissues of your shoulder. Then your movements have to be checked properly. And for this you need expert consultation. Then only reach to some decision else you would be prescribed 1 thing or another by different people no matter whether you get relief or not.

Many times all tests are negative and the patient still suffers. You are suffering from indigestion and due to which there is a toxin called AMA being formed in your stomach. This is accumulating in joints leading to swelling and stiffness. So digestion needs to be rectified first along with panchakarma treatment. Pls see me personally.

Due to diabetes the vessels of body and muscles becomes stiff. It means it is unable to move or touch. It's called frozen shoulder. Some mesh type structure form around the working joint which causes pain while movement. Apply the hot water bag over shoulder. So the hand exercise slowly. Although it is painful do some movements . You have to go for traction and also you can do IFT. Please consult the near by therapist for quick relief. It is also suggestible to go for cervical muscle strengthening exercises.

Hello sir, I have sleep disorder problem. My age is 35 years. My EEG report is normal. From last 4 years I am taking mirtazapine 7.5 mg (1/2 tablet per night). With this tab. I sleep very well 8-9 hrs. Many times, I try to sleep without this tab. But sleep only 3-4 hrs with disturbing (awake 4-5 times during night) and feels laziness/tiredness in day which disturb my job/routine. So I am taking the mirtazapine continue. I also exercises daily at evening, take 1 glass of milk at night, Drink 8 glass water daily, walking after dinner 2 km but not sleeps properly. I read on internet that sleep medicine increases B.P. And lack of sleep also increases B.P. L want to know the following:- 1. Which is more predominant for high BP (lack of sleep/medicine for sleep). 2. What is long term side effects of mirtazapine, if I continue it. 3. If any serious side effects, please suggest any other medicine (allopathic/Ayurvedic) which has low side effects than mirtazapine. Azlu probably you are facing stiffness resulting in pain due to accumulated lactic acid in group of muscles you need to do some physical exercise on daily basis along with it need glucose rich diet and for now avoid protein rich diets for few days along with it take a medicine Mag Phos 6x thrice daily for 5 days and consult back.
