We are married couple (32 age Male, 29 age Female). We have been trying to conceive for last 4 months. We need to use lubrication (Vaseline) during the sex. What are some of pregnancy friendly lubricant that I can buy in local stores.

MD - Obstetrtics & Gynaecology, MBBS
Gynaecologist,

If you need to use vaseline during sex, and you are trying to conceive, then be aware that vaseline is spermicidal and hence acts as a obstacle to conceive. Most lubricants are spermicidal and hence the nest would be to try and get as much natural lubrication as is possible. Before penetration, during foreplay please get famililiar with the private parts of the female. At the topmost part of the external vulva, is a fold of skin between which is enclosed a small bead like organ called clitoris. If the clitoris is massaged it will result in engorgement of the erectile tissue in the female and hence in preparation good lubrication will be thrown by the accessory glands. This will solve most of the problems as natural lubricant is very sperm friendly. During sex if you have routine man on top position it prevents the clitoris from being stimulated. Lubrication is a sign of arousal. If the woman is aroused her vagina gets lubricated, in preparation for the coitus naturally by the secretions of the accessory sex glands I would suggest you bring your partner at the edge of the bed, ask her to hold her legs apart and you can stand by the bed, between the legs and enter the vagina this way. In this position the visual stimulus to your erection and also your hands are free to stimulate the clitoris to get her aroused and hence more lubricated.

Dyspareunia is a term used for pain felt in the genital area or pelvis during or after having sex (intercourse). Nobody really knows exactly how common it is, as many women never seek medical help. However, questionnaires asking women if they have symptoms suggest that somewhere between 1 and 4 out of 10 women experience it. Most commonly, this is early in their sexual lives or around the menopause. There are many causes of dyspareunia, most of which are not serious or damaging in nature, but all can be detrimental to your sex life and ultimately may lead to relationship difficulties. It can be a vicious circle, with pain leading to nervousness about having sex, and nervousness leading to dryness and further pain. It's also not uncommon for dyspareunia to remain after the cause has been treated, particularly if things have been left untreated for a while. For this reason, it is important to seek help early, so that treatable causes can be discovered and managed Superficial dyspareunia This is pain felt in the lips of the vagina (labia), at the vaginal entrance (introitus) and the lower part of the vagina. It typically begins with penetration or very early on after intercourse has begun. It is sore and instant. It is usually quickly relieved by stopping penetration, although you may be sore to the touch afterwards for a little while. The causes of superficial dyspareunia are usually local problems of the labial, vaginal and perineal skin (the perineum is the area of skin between the vaginal opening and the back passage). Deep dyspareunia This is the term for pain felt deeper in the pelvis during or after intercourse. It can also spread to involve the fronts of the thighs. It may be sharp or dull, may stop when penetration stops or can continue for minutes or even hours. The causes of this type of pain usually lie rather deeper in the pelvis. Causes of superficial dyspareuniaSkin bridge Formation of a little bridge of scar tissue at the very back of the vaginal entrance (introitus) is common after childbirth, particularly if you have had a cut between the vagina and back passage (anus) - a procedure called episiotomy. The scar tissue contains nerve endings and is very sensitive, and so it causes pain on penetration. The pain tends to become worse as having sex (intercourse) continues, as the skin bridge becomes rubbed by the continuing rubbing of penetration. Intact hymen The hymen is a membrane that lines the vaginal opening. Early in your sex life the hymen is broken down by the act of having sex. In many young women it will already have been stretched by use of tampons. The name 'intact hymen' is misleading, as the hymen does in fact have a small hole in it from birth. This hole becomes larger little by little as girls grow older. However, the hymen can be quite thick and the hole not quite large enough. This can mean that early in her sexual life a woman my feel pain from the hymen as it is forced open the first time she has intercourse. The pain is superficial, felt at the entrance to the vagina as soon as penetration is attempted and may prevent it from taking place.

Dear Jagdish, any kind of bleeding in pregnancy is not a happy situation. We wait for heart beat in a fetus till it is 6mm or more or pregnancy is more than 6 weeks. Till that time please follow what your doctor has advised and then go for another scan and then take decision.

Mirena Insertion - Understnding the Procedure and Results

MD, MBBS
Gynaecologist, Gurgaon
Mirena Insertion - Understnding the Procedure and Results

Mirena is a contraception that can be inserted into the uterus for long term use. This insertion is done with a proper clinical procedure. The Mirena is a device that may be classified as a hormonal Intrauterine device (IUD). This device releases a kind of progestin, which prevents the sperm from travelling to the egg to fertilise it. This hormone basically makes the cervical mucus thick and makes the uterine lining thin, so that ovulation gets suppressed.

Let us get to know more about the procedure and the results of Mirena Insertion.

Reasons: There are a variety of reasons why women use this kind of long term contraception. After insertion, one does not need to have any interruptions in the act of sex in order to look for and use contraception. It also reduces the risk of developing pelvic inflammatory disease. This kind of disease is usually caused by sexually transmitted diseases. This kind of device can also help in bringing down the pain experienced during menstrual cycles. It also reduces the risk of falling prey to endometrial cancer as well as cervical cancer.

Eligibility: You are not an eligible candidate for this kind of procedure if you have suffered from breast cancer, cervical cancer or liver disease. Also, if you suffer from diabetes or other cardiovascular diseases, then it is not safe to go through this procedure.

Procedure: The procedure will be carried out at the doctor’s clinic. The gynaecologist will insert a speculum into your vagina. Then, the doctor will continue to clean the area including the cervix with the help of an antiseptic solution. Thereafter, a special instrument will be used in order to align the uterine cavity with the cervical canal. During this process, the doctor will also measure the depth of the patient’s uterine cavity. The device will then be folded and placed within an applicator tube, which will be inserted into the cervical canal. Then, the applicator tube will be removed. After this, the device will remain in place.

Result: Once the device has been inserted, it will be effective for a period of at least five years. Every month, you will need to get the strings of the device checked so that they are not protruding from the cervix. You may experience side effects like pain during sex as well as others like headaches and persistent migraines. These must be reported to the doctor so that the doctor may deal with any complications.

Removal: If there are any kinds of complications and risks that may develop after the procedure, the doctor can remove this device. This can be done with the help of forceps. The device can also be removed and replaced after five years. If you wish to discuss any specific problem, you can consult a gynaecologist.
