Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Requires detailed examination & investigations. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid, vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful. Use no pillow under the head. Do hot fomentation paracetamol 250mg od & sos x 5days. Bio d3 max 1 tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that the patient is not allergic to any of the medicines that patient is going to take. If it does not give relief in 4-5 days, inform.

Dear Dr, I feel hitching/ burning on my foot when I go to sleep and this is continue for some time and then NO problem. However can feel mild pain when walking. My BP is 140/85 and No sugar.I am 50 and have a job of sitting 6-8 hrs a day.

You need to maintain proper posture while working or studying if you use computers for long time, your table and chair should be ergonomically placed monitor level should be at the level of eye knee and hip at right angle do neck isometric exercises take vitamin d supplement normally if you follow all this, you should be better.

In high uric acid avoid few diets i. E. High purine-rich foods, milk products, protein foods, meat, dried beans, dried peas, mushrooms, whey proteins which are used in zym, alcohol, diuretic medicines. Eat balanced diet, eat banana, take amino acid rich diets, l- glutamine like supplements, whole grain, oats, green veg, low cholesterol foods, plenty of water in day time, do test kidney function test every 6 months to monitor levels of uric acid. Consult doctor always for medicines. Do not take medicines by self. Take tab. Zyloric 100 mg once a day. Tab. Ulracet sos for pain.

The pain could be because of early arthritic changes.Wear knee support while walking. Take 7 sessions of S.W.D followed by strengthening exercise of quadriceps.

Pain of neck, back,hanf and head. More on right sides since 2 years. Mri and xray it shows normal. Tried physio neck stretching and traction and also following good posture at work. Day by day pain is increasing its unbearable. I think I had strained muscles which has become chronic. Is there any treatment? Will chronically strained muscles ever heal to normal again? In any sort of unbearable pain, exercisetherapy is not recommended. It will worsen up the condition. First of all take your reports to a good Radiologist. Diagnosing the condition is the utmost important step only then a proper treatment will take place. If the tests show nothing and you are in pain, there is something wrong. A good doctor doesnot just rely on the reportsbut view images himself and evaluates what he sees based on his examination and the patient's symptoms and signs.

(from mankind) 1 tab bd x 5 days make sure that, there is no allergy to any of the medicines. For emergency treatment contact your nearest hospital or family doctor. Hello lybrate-user, knee pain occur after 30-35 are due to degenerative changes in bone. It may be due to osteoarthritis or rheumatoid arthritis. So consult a physio and learn some strengthening exercise and start doing it guided by them. From xray images and symptoms of patient we can come into conclusion either it is due to osteoarthritis or rheumatoid arthritis. So in both the condition knee strengthening exercise plays important role. There are also ultrasound, ift, swd modality availablele in physio setup to get you releive from pain. So it would be better if you visit a nearby physio and start doing exercise guided by them.

What You Should Know About Spondyloarthritis?

The bones and joints orchestrate very well with each other for smooth movements of various joints and the spine. However, due to various reasons, some identified and others unknown, there could be inflammation of the areas where the bones join the muscles leading to impaired movement. The range of motion could be reduced or it could be painful, thereby, limiting movement. Spondyloarthritis includes a group of inflammatory diseases that involves the bones and joints and enthuses which are areas where the ligaments and tendons attach to the bones. This causes conditions like plantar fasciitis, ankylosing spondylitis, psoriatic arthritis and reactive arthritis. The term spondyloarthritis is a blanket term to describe any of the above-mentioned conditions.

Read on to know a little more in detail about it:

Causes: While the exact etiology is not identified for sure, most patients with spondyloarthritis have an autosomal dominant gene (HLA-B27). Chronic infections could trigger the onset of the disease too.

Symptoms:

1. In all forms of spondyloarthritis, there is always inflammation of the spine, joints and entheses. If the fingers or toes are involved, there could be swelling leading to what are typically known as 'sausage digits'.

2. There is sacroiliitis which is inflammation of the joint between the pelvis and the spine, and spondylitis which is inflammation of the joints between the spines. 3. Family history and autosomal dominant gene.

Diagnosis:

A detailed medical history and physical examination form the cornerstone of a confirmed diagnosis. In addition, radiographic examination and diagnostic blood tests also could be done to confirm the diagnosis. Radiographs, especially, of the spine and the low back show typical changes including sacroilitis in suspected cases of spondyloarthritis. If the spine does not show these changes, but there is a high likelihood of spondyloarthritis, then MRI (magnetic resonance imaging) can be used to confirm the diagnosis.

As a final step, a blood test for the HLA-B27 gene can also be confirmatory. However, it is not true that a person with the gene will definitely develop the disease.

Treatment:

The most important aspect of treatment is the use of pain medications and anti-inflammatory drugs (NSAIDs) which help manage pain and inflammation.
Corticosteroid injections may sometimes be given directly into the joint spaces or the membranes to provide quick relief of these symptoms.
ESWT / RSWT has a definite role to cure enthesopathy. Plantar fasciitis, and achillodynia can be cured with ESWT. Even small joints of hand and feet are amenable to treatment with RSWT. It is nonoperative and noninvasive treatment and has no side effects.
Very rarely, stronger drugs such as methotrexate or sulfasalazine may be used.
Another set of drugs known as TNF-blockers (tumor necrotizing factor blockers) are also shown to be highly effective in controlling the pain of the spine and joints.
In very severe cases, surgery may be required to correct joint damages or correct severe degenerative changes.
Exercise plays a very vital role in controlling the issue and improving range of motion.

Following medicines of ayurveda classics are used for cervical spodylosis and backache:

Ekangavira rasa, pravala pisti, brihat vata chintamani ras, svarna-maksika bhasma, trayodasanga guggulu, chandra-prabha vati, pidantaka rasa. The vehicle (anupana) is honey. As given in my tips earlier, harsingar/ parijat (night jasmine) leaves decoction used with these will miraculously relieve and cure the patient barring acute prolapse conditions for which surgery is indicated. But surgery of spine should be got done in extreme necessity only by experts.

As everybody knows that such diseases are technically pain management techniques but medicinally, in allopathy, only some pain killers are given to get instant relief. Alternatively, above ayurveda medicines balance the vata vitiation. When used with other pain management support techniques, patient comes out of chaos very soon.

You could be having planter fascitis which is a fairly common condition at your age. Put you foot in warm water for 10-15min daily. Get the help of a physiotherapist who can teach you stretching exercises for the foot. He can also give you ultrasonic therapy. Take calcium supplements.
