Simmyji,First thing is what type of diabetes is having T1 or T2? If it is T1 then only treatment is taking Insulin injections. If T2 then looking at his glucose levels one can think of adjusting the drug treatment. But usually insulin is given for improperly controlled diabetes and also for those who have very high sugar levels or have HBA1c% more than 8 to 9 %.

Mr. Lybrate-user, diabetes can only be treated by either oral anti-diabetic drugs or insulin injections or both. Plus diet restriction and regular exercise. Unfortunately there are no other drugs in any branch of medicine which control blood sugar. So my suggestion is to continue present treatment. I have seen patients in my practice who stopped modern medicine and started other therapies and have come back with exceptionally high blood sugar. Now the choice is yours.

Mr. lybrate-user, Thanks for the query. I have gone through the details given. Since she is having diabetes for last two years, there must be some treatment going on. So please tell me the details of that. Plus give following information: Fasting, PP glucose levels, HbA1c%, diet pattern, extent of daily exercise, weight and height. To treat diabetes appropriate drugs are to be given. Plus there has to be a strict control on diet and every day at least 1 hour brisk walking should be done. Patient should avoid sweets, deep fried foods, junk foods, pastries, bakery items and colas and foods made from maida. Thanks.

I am taking insulin basalog 20 units in a day I having a faasting sugar of 150 should I increase the dose

Your sugar is not very high do regular exercise and diet control and check your HBA1c levels every 3 months.

1. Do regular exercise for 30 minutes daily like brisk walking 2. Reduce weight if overweight 3control diet a) avoid sweets, sugar/honey, milk, milk product excess calorie and carbohydrate rich diet like white bread, cereal, rice, pasta b) avoid all refined food like maida, pasta, starchy food like rice, potato, bakery item, processed food, c) include more fiber like pulse, green leafy vegetable, cucumber, carrot, tomato, j legume. D) avoid sweetened fruit like banana, mango, litchi, chickoo, sapota, seethaphal jack fruit, grape etc. Can have fruits like guava, pineapple, papaya, orange, mousambi, strawberry, watermelon, pomegranate, jack fruit, sweet potato, a handful of walnut daily (akhrot) e) avoid white/polished rice, take unpolished rice like brown rice/wheat/jowar/ragi, moong and chana dal. F) avoid ghee/ butter./coconut oil/excess salt intake like papad/pickle. G) avoid red meat. Take fish/chicken. H) avoid alcohol and carbonated drinks like soft drink 4. Adhere to a strict diabetic diet by consulting dietitian 5. Monitor your fasting, pp blood sugar 3 monthly and hba1c six monthly 6. Consult diabetologist for advice and treatment.

Hi doctor I have been suffering from diabetes for the last six years but despite it I keep fast in the month of Ramadan (Ramzan. 1 week ago I felt low back pain that was unaffordable. My doctor suggested me to not to keep fast and said keeping stomach empty for diabetes patients may cause of increase of sugar level. I just want to know can I keep fast or not.

Dear lybrate-user, best wishes for the holy month of ramadan! you are very young and your diabetes should not be a problem for observing fast, in ramadan this year days are very long and hence dose of medicines need to be adjusted so well that you should not experience low sugar. My father went through a test B.M.D[DEXA] [LUMBER SPINE], his LUMBER spine T-Score is -2.3, Result is OSTEOPENIA, His Vitamin D test (Method: CLIA, SAMPLE: Serum) score is 39.40. His age is 77, He is also a diabetic patient since 20 years. So please advise me treatment for his problem .

Mr. lybrate-user, At 77 years, it is difficult correct osteopenia. He can be put on Vitamin D 3 and Calcium supplements, plus some regular exercise. Brisk walking is an ideal one. A healthy lifestyle benefits osteopenia patient very effectively. Diet containing dairy products, like cheese, curds, milk is useful. Plus consuming green vegetables also helps. Most important is to keep active. Thanks.

How much does it matter if I have metformin before or after meal? also if i'm having it 1000 mg od or 500 mg bd?

If only metformin is taken it should be after food. If sustained release tablet is there then od may ok. If not twice a day is better. Side effects are less if taken after food.

It might be due poor postural use of this limb. Do some light stretching exercises and warm foamentation. Light activities.
