Dear sir 1-my father is 73 year have gone bypass surgery on 1995 in hospital, delhi. Triple vessel artery disease. Graft were implated. Having diabetic, bp. By Dr. Gk mani. 2-now on 15-jan-015 he again admitted to medical college, lucknow. For heart problem. Angio was done. All graft blocked. Medicine prescibed. Shock also given during angio. Dischrged. 3-(23-01-015)----150/80 pulse-100----(14-04-015)----110/60, pulse---90 4-renal dysfunction ---(14-02-015)---serum creatine---1.66, serum urea---84mg/dl---(14-04-015)--serum creatine---1.10, serum urea---34mg/dl 5-renal dysfunction---(02-03-015)---serum creatine---3mg/dl, serum urea---112mg/dl--(14-04-015)--normal 6-(04-03-015)---uric acid---9.8----(14-04-015)----2.5mg/dl 7-(18-03-015)---serum creatine---1.6, serum urea---46mg/dl---(14-04-015)--serum creatine---1.10 8-stomach pain on (18-03-15)---done ultrasound---given inj voveran for pain---found 4-6mm multiple stone in gallbalder. Dr. Advise for operation. Report shows---mild hepatomegaly, choleithiasis, fatty pancreas, hypoechoic bowel wall thckening in left illacfossa, inflammatory. There is pain frequently. And voveran inj have to be given. For pain relief. Now problem:-(19-04-015) 1-pain (inj voveran given. But again pain. For gallbalder. 2-constipation 3-uneasyness. Feeling heavyness. In chest. When. Bending forward I cannot breathe through my nose it get stuffed. When I lie down to sleep at night my both nose I cannot breathe through it. My chest hurts as well. Whats the cause. Sometimes I get lightheadedness and dizziness? My ct scan on brain and sinus is normal in the ct results.

Trouble in breathing while bending and lying down is not uncommon in heart disease. But such people get excessively breathless with walk, work or lifting weights. With symptoms of'chest hurt' and dizziness better to get heart and lungs examined.

My mother (58+ yrs) is under treatment for Hypothyroidism (Eltroxin 50 mcg), Hypertension (Olmezest 20 mg) & Osteoarthritis (stage-2) of knee joints. Since past couple of months, she faced the problem of seeing every distant object in black color as well as in blurry image. Recently she visited an eye doctor, who suggested this is because of the eye gel getting dried up. She had cataract also, but that's mostly at initial phase. 1) What could have caused this eye gel to be dried ? Is it something serious & related to other medical problems ? 2) They have prescribed 2 eye drops - Aqualube & Nepaflam daily 3-4 times for 3-months. Is it safe to to use Nepaflam for such a long period ?

Dryness is caused by hypothyroidism itself. There is a change in composition of mucous that is secreted by special glands in the conjunctiva tissue of the eye. She has to use lubricating eye drops on a daily basis at a frequency that depends upon the ambient temperature, humidity and wind factor. Generally speaking use drops for comfort and repeat when discomfort starts. Lubricating drops are safe and preservative free formulations are also available. Nepaflam has to be stopped after 3 weeks.

These reports suggests that time he come to be alert and try to control your oily diet although your good cholesterol is in normal range but it is low borderline range and simultaneously your bad cholestrol is increasing in your body saturated fat and cholesterol in the food you eat make your blood cholesterol level go up. Saturated fat is the main culprit, but cholesterol in foods also matters. Reducing the amount of saturated fat and cholesterol in your diet helps lower your blood cholesterol level. So from today onwards use only olive oil and mustard oil in home and say bye bye to outside food in all forms and by doing this you can also control your weight to below 72 kg and your sgpt will come down a your digestion will improve by eating home made easily digestable food.

Dear lybrateuser, what you are describing does not appear to be cardiac pain and looks more like a muscular pain but still to be sure you can get an ecg done and contact again with report.

There is possibility of head spinning due to high blood pressure. But as per your age the possible cause are * cervical spondylosis * low blood pressure * anaemia * any problem of ear * stress.

This must be due to lung and pleural infection postoperatively. Due to which the collection fluid in the pleural cavity. You should consult your treating surgeon or physician. For the same.

Dear lybrateuser, -first try to stop smoking completely since one or two cigarettes occasionally can lead you to more numbers slowly -slowly -for your problems take a healthy diet & do regular exercises --eat vegetables, fruits, whole grain foods like oats, dalia, dairy products like milk, eggs, paneer, nuts, non-veg like fish, chicken, have yoghurt daily -start doing exercises to increase your strength & stamina, do brisk walking, jogging, swimming, yoga, after sometime your problems will go away.

You can control cholesterol / hdl / ldl/t.g. and obesity by using following flax oil, omega-3 fatty meal or cap., take . green veg., whole grain foods, spicy foods, Use GARLIC in foods daily ,. Cod liver oil. Low sugar foods, low calories foods. Change own life style . do meditation,yoga, Go walk daily in morning and evening, do any one physically activity daily i.e. play any game., Avoid fast foods, oily foods, alcohol,smoking, fatty foods, non-veg., avoid stress, take plenty of water in day time. These are helful also reduced obesity. one important that is take 1 glass of water before bed at night. Because it protect from heart fail during sleep at night. Welcome in lybrate.

To control & maintain Blood pressure to normal these are best methods to follow:.• Do physical exercise like jogging, running or brisk walk for atleast 20-30 minutes. • Maintain your cholesterol level to normal & eat least amount of butter, cream or fatty things. • Eat less amount of pickle, salt and fast food as they contain high sodium levels. • Avoid drinking alcohol and smoking. • Take less amount of caffine containing coffee, tea or cold drinks because caffine causes increase BP. • Drink 8 to 10 glass of water daily • Take 6 to 8 hrs of sleep daily. Do not take too much stress. • Do long breathing pranayams (Anulom- Vilom, Bhramari) daily for 10 - 15 minutes. It will reduce your stress level in your body. • Eat healthy diet such as green leafy vegetables, fruits. • Take 5 ml of GULAB Jal added in a glass of water daily once a day.

Sexual problems might mean you have a broken heart, literally. The most common sexual problem in men is erectile dysfunction (ED). Erectile dysfunction affects >30% of men between 40 and 70 years of age. There are several different causes of ED, including depression, low testosterone, neurological problems, and some medications, such as beta blocker but the most common cause is a problem with the blood vessels called atherosclerosis.

Erectile dysfunction is frequently a sign of atherosclerosis, that can be the first clue to heart disease in men. It usually comes 3 to 5 years before a heart attack, so after ED is diagnosed, there is time to treat atherosclerosis and prevent a heart attack.

Sexual Activity and Cardiovascular Disease

Decreased sexual activity and function are common in patients with CVD and are often interrelated to anxiety and depression. In fact research has proved that there is a strong link between erectile dysfunction and heart disease. Studies have shown that if a man has ED, he has a greater risk of having heart disease. For instance, in one study, 57% of men who had bypass surgery and 64% of men hospitalized for a heart attack had had ED.

Several having ED can predict that a man will probably have heart disease symptoms within five years. In fact, having ED is as much a risk factor for heart disease as a history of smoking or a family history of coronary artery disease.

The Stockholm Heart Epidemiology Programme (SHEEP) study of post-MI patients (50% women) found that those who were sedentary had a higher risk of MI with sexual activity (relative risk 4.4) than did those who were physically active (relative risk 0.7).

How is the Heart and ED Related?

Deposition of cholesterol in the arteries causing atherosclerosis is taken as the most important reason for ED as the plaque formation reduces the blood flow in the penis, thereby affecting the erection process. However the latest studies show that ED is also due to the dysfunction of the inner lining of the blood vessels (endothelium) and smooth muscle. This causes inadequate blood supply to the heart and the penis which also aids in arthrosclerosis. Hence ED and heart disease are found to be inter related.

What to do?

If you are suffering from ED or heart disease , it is best to discuss your concerns with a specialist - for the heart a cardiologist and for the ED a urologist

Eat a high-fiber diet. Get insoluble and soluble fiber. Limit foods that are high in fat. Choose lean meats. Incorporate probiotics into your diet. Eat on schedule. Stay hydrated.

Mr. lybrate-user, If the heart specialist has advised by pass surgery then most probably surgery has to be done. Otherwise, heart muscle may not get proper blood supply leading to myocardial infarction (heart attack). Thanks.

Approach to Ramadan Fasting In Patients with Heart Diseases

Holy month ramadan is auspicious for worldwide muslims and most of them starts fasting early in the morning up to the sunset in the evening during this entire month.

Heart disease patients tend to fast during ramadan but they have many questions:

1. Cardiac patients can fast?

The holy Quran exempts sick people from fasting. Fasting is allowed only for stable heart disease patient and it's strictly the discretion of the treating cardiologist.

Common types of cardiac conditions as follows:

A. Patients who had heart attack or recent stenting or open heart surgery

Many studies comparing ramadan with other months in cardiac patients shows there is no significant differences in the incidence of heart attack or typical cardiac chest pain during ramadan. Patients with recent acute heart attack, heart attack equivalent chest pain with high cardiac enzymes, recent stenting, recent open heart surgery should strictly avoid fasting.

B. Patients with low pumping of heart i. E, reduced ejection fraction 

Low pumping of the heart is one of the common cause of frequent hospitalisations among cardiac patients. Many studies done in the middle east shows there was no significant difference in the number of hospitalizations for patients with low heart pumping during ramadan. Its almost similar to other months without fasting. But in this category certain groups of patients cannot fast especially patients with difficulty in breathing, associated swelling of legs, associated cough and sudden increase in weight.

C. Patients with high blood pressure.

Most of the high blood pressure patients on controlled medications can fast during ramadan as it will not affect the blood pressure readings and it was proved in many studies. Certain group of patients with resistant blood pressure who requires multiple drugs for controlling should consult treating doctors before ramadan for appropriate dose adjustments. If dose adjustment is not possible then for such patients it is advisable to abstain from fasting to avoid cardiac and other complications.

2. How to take their cardiac drugs during this month?

Stomach ulcer and related complications are common during ramadan. Hence drugs which can cause gastric irritation (like aspirin) should be taken as per the advice of treating doctor. Before ramadan you should consult your doctor and make sure regarding the dose and timing. Medications like water pill (lasix)to be avoided during fasting.

3. Lifestyle during ramadan

Avoid over eating while breaking the fast

Avoid exercise after a large meal

Stop smoking

4. Can cardiac patients do hajj pilgrimage?

Cardiac patients who is certified as stable and fit by the treating cardiologist can go for hajj

Full medical workup is mandatory before hajj.

Patients with recent acute heart attack, heart attack equivalent chest pain with high cardiac enzymes, recent stenting, recent open heart surgery are unfit for hajj.

Yes he should take bp medicine regularly as bp is normal because of medicine bp monitoring is required daily and consult your physician he may reduce dose of bp medicine after seeing bp monitoring chart.
