Diabetic Nephropathy - Who is at Risk of Acquiring it?

MBBS, MD - Medicine, DM - Nephrology

Diabetic Nephropathy - Who is at Risk of Acquiring it?

Diabetes is a lifestyle disease that is very prevalent in the current generation. According to a report by the World Health Organisation (WHO), the number of people with diabetes has risen from 108 million in 1980 to 422 million in 2014. The global prevalence of diabetes among adults over 18 years of age has risen from 4.7% to 8.5% from 1980 to 2014. Diabetes should not be taken lightly as it can affect other body organs besides the pancreas, such as the kidneys. Diabetic nephropathy refers to damage to your kidneys caused by diabetes, and may also lead to kidney failure in various cases.

Causes of diabetic nephropathy
Not everyone who suffers from diabetes will suffer from diabetic nephropathy. The main function of the kidneys is to filter waste from your blood. However, this is not always the case. Due to high a concentration of blood sugar caused by diabetes, several blood vessels in the kidney are destroyed, thereby, preventing the kidneys from carrying out its job. Over time, the kidneys may stop working altogether, resulting in kidney failure. If you are diabetic, the probability of you suffering from diabetic nephropathy will increase if you smoke or have high blood pressure or high cholesterol.

Catching it early
Today, diabetic nephropathy is the leading cause of chronic kidney disease in the US and other western countries. Since the symptoms of this condition aren’t visible in its early stages, you need to undergo urine tests on a regular basis. If detected early, the effects of diabetic nephropathy can be reversed. Common symptoms of this disease include the following:

  • Swelling in legs and feet
  • Detection of albumin in urine
  • Decline in glomerular filtration rate (GFR)
  • Elevated arterial blood pressure

What are the treatment options?
Medication to lower the blood pressure is the most commonly prescribed form of treatment suggested by doctors to prevent or slow the damage to your kidneys. There are other lifestyle changes that you may have to undertake in order to keep a check on the condition:

  • Limit sugar intake and keep blood sugar levels within the target range
  • Exercise regularly and work towards keeping your heart healthy
  • Limit protein intake, as it could put extra pressure on your kidneys
  • Limit salt intake to control blood pressure
  • Quit smoking and drinking

Diabetes is an unfortunate condition to have to live with. But don’t let it bog you down! With these simple lifestyle changes and taking care of your general well-being, you can beat the disease and emerge a winner!

Hi, I would not like to counsel you without the reports as I need to understand his case better before I can advise further. But seeing that the creatinine is so high, I feel that he needs to be on dialysis and a transplant will do him a world of good, but it has its own risks and complications.

If there is no other problem except the above, it indicate you are not taking sufficient quantity of water. Take minimum of 3 litres of water in a day. Avoid hot and spicy food. If you move in sun a lot then increase water intake to 4 litres a day. Protect yourself from hot sun. Get your urine tested for routine examination, if any finding come back again.

Yes urine retention gives sexual excitement/ pleasure, because of its pressure on sex organs. But it is not a good habit to retain waste material in body, it may cause uti etc or some other problems in future. You must void urine whenever you feel pressure.

high blood pressure combined with diabetes going on for a number of years are invariably dangerous to the kidney. These conditions lead to a chronic kidney damage then to chronic kidney failure. The treatment options are limited. Most important thing is to have strict sugar and bp control. Because if these diseases are ot fully controlled then damage is progressive, can cause complete failure. So make sure that his sugar levels and bp are as normal as possible. Reduce excess salt and protein intake. Drugs like nsaids (pain killers) should be avoided. If need be dialysis should be used to help kidney functioning.

I am a 65 years old female. I am a diabetic patient but the sugar is under control. I suffer from kidney (urine) infection for the past 3 years. Despite several allopatic medicines the infection is not subsiding. The infection is caused by e-coli bacteria. Kindly prescribe the right medicine.

MBBS, DGO (cal)
General Physician,
Diabetics are prone to different systemic and local infection. Female are very prone to urinary tract infection, which turns to be a chronic one due to negligence or ignorance in the part of patient who do not comply with medical advice. Once it become recurring infection it becomes very problematic but not impossible to cure such condition. What you need is to talk to a urologist / nephrologist for following: a) to assess kidney function, b) to find out whether there is any persistent infection and if yes, causative organism and its drug sensitivity. C) then treat the condition with appropriate medicine for at least 2 - 3 weeks. D) and follow-up treatment for a prolonged period, at least for 3 - 6 months (may be more) to prevent infection. E) side by side good blood sugar control is to be achieved. This part is your diabetologist's job. So, hope you could understand. Please do as I said.

Kidney Stones Risks and Their Prevention

MBBS, DNB (General Medicine)
General Physician, Delhi
Kidney Stones Risks and Their Prevention

Kidney Stones

What are kidney stones

Kidney stones are tiny, hard masses that form inside the kidneys. These stones are made of acid salts and minerals.

Causes

Kidney stones may get created when an imbalance of minerals, salts, water and other materials found in urine occurs. Things that cause the imbalance include:

Insufficient drinking of water
Different types of medical conditions

A person can form kidney stones due to his/her family history as stones frequently occur within the family over several generations.

In rare cases, kidney stones are formed due to excessive production of a hormone by the parathyroid glands.

Symptoms

Symptoms of a kidney stone may not be found until it proceeds to the ureter or moves around inside the kidney. When any of these occur, the following symptoms can be noted:

Pain that advances to the groin and lower abdomen
Acute pain in back and side, beneath the ribs
Brown, pink or red colored urine
Pain on urination
Vomiting and nausea
Foul-smelling or cloudy urine
More urination than normal
Continual need of urination
Urinating little amounts of urine
Fever and chills when there is an infection
Diagnosis

If the presence of a kidney stone is suspected by the doctor, the following diagnostic procedures and tests may be prescribed:

Blood tests
Imaging tests
Urine tests
Examination of passed stones
Diagnosis

Treatment

Treatment for kidney stones differs, depending upon the cause and type of stone. Most kidney stones can be cured by home treatments and don't require invasive treatment. Home treatments include:

Drinking enough water
Use of pain relievers

If conservative measures fail to treat kidney stones, extensive treatment may be required. Such procedures can include:

ESWL (extracorporeal shock wave lithotripsy)
Surgery
Use of ureteroscope
Parathyroid gland surgery

Prevention and controllable risk factors

A merger of medications and lifestyle changes may help prevent the formation of kidney stones.

Medications can help maintain the balance of acid and minerals in the urine. They can also be helpful for those having certain types of stones.

Lifestyle changes too can help prevent this disease. They include:

Drinking water round the clock.
Having diets low in animal protein and salt.
Avoiding oxalate-rich foods.

Controllable risk factors that elevate the risk of forming kidney stones include:

Intake of diets high in sodium, protein and sugar. A high-sodium diet is especially risky as excessive sodium in your diet increases the quantity of calcium your kidneys must filter, thus significantly boosting your risk of kidney stones.
Dehydration: Inadequate intake of water increases your risk of kidney stones. People who sweat a lot and those living in warm climates are at higher risk than their counterparts in other, more comfortable regions.
Being obese: Several studies have linked weight gain, large waist size and high BMI (body mass index) to an increased risk of kidney stones.

A merger of medications and lifestyle changes may help prevent the formation of kidney stones.

Medications can help maintain the balance of acid and minerals in the urine. They can also be helpful for those having certain types of stones.
